## Procedure steps

### 1. Characterising the pain and localising the root

The doctor asks when the pain began, its path, how far down the leg it travels, which movements aggravate it such as coughing or prolonged sitting, and exactly where the numbness lies, because the distribution of symptoms indicates the affected nerve root before any scan.

### 2. Neurological examination and ruling out mimics

Straight-leg raising, foot and ankle power, sensation and reflexes are tested, the hip and sacroiliac joint are examined, foot pulses are checked, and the doctor asks about diabetes and about red flags such as fever, weight loss or unremitting night pain.

### 3. Guided conservative treatment

A plan combining staying active with regular walking instead of prolonged rest, physiotherapy with exercises chosen for the affected root and your tolerance, adjustment of sitting and lifting, and medication prescribed for a limited period. The plan is reviewed after a few weeks against pain and function.

### 4. Imaging and injection if pain persists

MRI is requested when disabling pain persists or weakness or a red flag appears, not in the first week of simple pain. An image-guided injection around the root may be offered to calm inflammation and make rehabilitation possible, with its benefit reviewed after two to four weeks.

### 5. Surgery when needed and follow-up

For progressive neurological weakness or unresponsive disabling pain, the compressing portion of the disc is removed through a small incision or endoscope and the root is freed. Walking usually starts the same day, followed by graded rehabilitation and follow-up of sensation and power, which may take weeks to months to recover.

## About this service

Sciatica is not a disease in itself but a description of pain that radiates from the lower back or buttock down the leg along the path of the sciatic nerve, sometimes reaching the foot and accompanied by numbness, pins and needles or weakness. It usually reflects pressure on or irritation of a nerve root in the lower back. The commonest source is herniated disc material, followed by narrowing of the nerve exit or the spinal canal, with less common causes including masses, infection and piriformis-related irritation.

The first step is therefore diagnosis rather than treatment: identifying which nerve root is affected from the distribution of pain, the areas of numbness, the pattern of weakness and the reflexes, and excluding conditions that mimic sciatica, such as hip and sacroiliac joint pain, peripheral neuropathy in people with diabetes, and reduced arterial circulation in the leg.

Most acute sciatica improves gradually with conservative care: staying active within the limits of pain rather than resting in bed, physiotherapy with targeted exercises and correction of movement and lifting mechanics, and medication prescribed by your doctor to control pain and inflammation or to settle nerve pain. When disabling pain persists despite this, an image-guided injection around the nerve root is considered, or surgery to remove the compressing portion of the disc. Surgery is brought forward when there is progressive neurological weakness.

The limits should be stated plainly. Painkillers reduce pain but do not repair the source of compression. No exercise, traction device or machine puts a disc back in place in the literal sense, although many herniations do shrink on their own over time. An injection calms inflammation around the nerve but does not dissolve the herniation. Surgery treats leg pain caused by compression far better than it treats chronic back pain, and it does not prevent a new problem developing at another level.

Two common myths need correcting: prolonged bed rest slows recovery rather than speeding it, and a disc herniation on an MRI report is not by itself a reason for surgery, since many people with no pain at all have herniations visible on imaging. The decision is built on matching symptoms to the clinical examination, not on the report alone.

## Preparation

Before your visit, note when the pain started, where it ends in the leg, what aggravates and relieves it, and whether your foot has become weak or you have caught it while walking. Bring any earlier imaging and reports. Tell the doctor about all your medicines including blood thinners and aspirin, about any stomach ulcer, kidney disease or drug allergy before anti-inflammatories are prescribed, and about diabetes or liver disease. Say if you are pregnant or breastfeeding. Wear comfortable clothing that allows your back and legs to be examined and lets you bend and walk. If you are a candidate for an image-guided injection, ask about fasting and which medicines to pause beforehand, and arrange for someone to drive you home. Bring your questions written down and ask openly about the alternatives at each step.

## Aftercare

Keep moving with short frequent walks, and avoid sitting for more than half an hour at a stretch, bending combined with twisting, and lifting heavy loads while the pain is acute. Sleep in whatever position hurts least rather than following rigid rules, and use warm or cold packs according to what soothes you. Do your home physiotherapy exercises daily, since they are the backbone of recovery, and return to work and sport in stages. After surgery, follow your surgeon's restrictions precisely, care for the wound, and avoid long car journeys in the first weeks.

\*\*Go to the emergency department immediately for progressive leg weakness or a dropping foot, numbness between the thighs and around the anus, urinary retention, leakage of urine or stool, or severe pain in both legs at once — these are cauda equina symptoms and are treated as a surgical emergency.\*\* Contact your doctor quickly for fever, unintended weight loss, night pain that no position relieves, or a past history of cancer.
