العلاج متدرّج: يبدأ بمضادات تعرّق طبية تحتوي أملاح الألومنيوم تُطبَّق على جلد جافّ تماماً قبل النوم لا في الصباح؛ ثم الرحلان الأيوني بالماء على جلسات متكرّرة للكفّين والقدمين؛ ثم حقن توكسين البوتولينوم في الإبط أو الكفّ الذي يوقف إشارة الغدد العرقية موضعياً؛ وهناك أدوية فموية مضادة للكولين تُصرف بإشراف طبي لأن لها آثاراً جانبية مثل جفاف الفم وتشوّش النظر واحتباس البول ولا تناسب كل مريض؛ وأجهزة تعتمد الطاقة الحرارية لتقليل الغدد في الإبط؛ وأخيراً قطع الأعصاب الودّية بالمنظار في حالات شديدة جداً بعد فشل ما سبق.

حدود العلاج: \*\*الهدف تقليل التعرّق إلى مستوى محتمَل لا إلغاؤه، فالتعرّق وظيفة تبريد ضرورية وإلغاؤه كلياً غير مرغوب ولا آمن. ونتيجة الحقن مؤقتة تستمرّ عادة عدّة أشهر ثم تحتاج إعادة، ومضادات التعرّق تحتاج استمراراً وانتظاماً، والجراحة قد تسبّب تعرّقاً تعويضياً في مناطق أخرى من الجسم لا يمكن التراجع عنه.\*\* ولا يوجد علاج يُنهي المشكلة نهائياً بجلسة واحدة، والترشيح لكل خطوة يحدّده الفحص السريري ودرجة التأثير على حياتك.

وتوضيح مهم كثيراً ما يُخطئ فيه الناس: مزيل العرق ومضاد التعرّق شيئان مختلفان؛ الأول يعالج الرائحة الناتجة عن البكتيريا ولا يقلّل كمّية العرق، والثاني يسدّ قنوات العرق مؤقتاً فيقلّل الكمّية. وفرط التعرّق ليس دليل ضعف أعصاب ولا نقص فيتامينات ولا قلّة نظافة، ولا يُعالج بالحميات ولا بالخلطات العشبية.

## خطوات الإجراء

### 1. التقييم وتحديد النوع واستبعاد الأسباب الثانوية

يسأل الطبيب عن عمر البداية والمناطق المصابة وهل التعرّق ليلي أم يتوقّف في النوم، وعن التاريخ العائلي والأدوية والأعراض المصاحبة كفقدان الوزن والحرارة والخفقان، ويفحص الغدة الدرقية والعقد اللمفية، ويطلب فحوصاً موجَّهة فقط عند الشكّ بسبب ثانوي.

### 2. تحديد المناطق ودرجة التأثير على الحياة

تُحدَّد المناطق الأكثر تأثّراً بدقّة، وقياس الأثر على العمل والدراسة والكتابة والقيادة والحياة الاجتماعية، لأن شدّة الأعراض هي التي تحدّد الخيار المناسب. وقد يُستخدم اختبار بسيط بمسحوق ملوّن لتحديد مساحة التعرّق قبل التخطيط للحقن.

### 3. البدء بالعلاجات الموضعية والرحلان الأيوني

يُوصف مضاد تعرّق طبي مع تعليمات دقيقة: جلد جافّ تماماً، تطبيق ليلي، وغسل صباحي، وتقليل التكرار عند التهيّج. وللكفّين والقدمين تُجرى جلسات الرحلان الأيوني المتقاربة أولاً ثم تتباعد للصيانة، وتُشرح لك مدّة الالتزام اللازمة قبل الحكم على النتيجة.

### 4. الحقن أو الخيارات الأعلى عند الحاجة

إن لم تكفِ الخطوات السابقة تُطرح حقن توكسين البوتولينوم في الإبط أو الكفّ بعد شرح أن الأثر مؤقّت ويحتاج إعادة، وأن حقن الكفّ قد يسبّب ضعفاً مؤقّتاً في قوّة القبضة. وتُناقش الأدوية الفموية بإشراف طبي وآثارها الجانبية، وتُترك الجراحة للحالات الشديدة جداً بعد استنفاد البدائل.

### 5. المتابعة وخطة الصيانة

تُقاس الاستجابة بعد أسابيع، ويُعدَّل الجدول: مواعيد إعادة الحقن، وتكرار جلسات الرحلان، واستمرار المستحضر الموضعي. وتُضاف تدابير عملية: أقمشة قطنية وجرابات ماصّة وتبديل الأحذية والعناية بالقدم لمنع الفطريات والرائحة التي كثيراً ما ترافق فرط التعرّق.

## Procedure steps

### 1. Assessment, type and excluding secondary causes

The doctor asks about the age of onset, which areas are affected, and whether sweating occurs at night or stops during sleep, along with family history, medicines and accompanying symptoms such as weight loss, fever or palpitations. The thyroid and lymph nodes are examined, and targeted tests are ordered only if a secondary cause is suspected.

### 2. Mapping the areas and measuring the impact

The most affected areas are mapped precisely and the impact on work, study, writing, driving and social life is measured, because symptom severity determines which option fits. A simple coloured-powder test may be used to outline the sweating area before planning injections.

### 3. Starting topicals and iontophoresis

A medical antiperspirant is prescribed with precise instructions: completely dry skin, application at night, washing off in the morning, and reducing frequency if irritation appears. For palms and soles, iontophoresis sessions are given close together at first and then spaced out for maintenance, and you are told how long to persist before judging the result.

### 4. Injections or higher-tier options when needed

If earlier steps fall short, botulinum toxin injections into the armpit or palm are offered, with a clear explanation that the effect is temporary and needs repeating, and that palm injections can cause temporary weakness of grip. Oral medicines and their side effects are discussed under medical supervision, and surgery is left for very severe cases once alternatives are exhausted.

### 5. Follow-up and a maintenance plan

Response is measured after some weeks and the schedule is adjusted: when to repeat injections, how often to run iontophoresis sessions, and how to continue the topical product. Practical measures are added — cotton fabrics, absorbent socks, rotating footwear and foot care — to prevent the fungal infection and odour that often accompany hyperhidrosis.

## العناية بعد الإجراء

طبّق مضاد التعرّق على جلد جافّ تماماً قبل النوم واغسله صباحاً، ولا تستخدمه على جلد محلوق حديثاً أو متهيّج. وإن ظهرت حكّة أو حرقة قلّل عدد الليالي بدل التوقّف الكامل، وأخبر الطبيب. بعد الحقن تجنّب الحرارة الشديدة والساونا والرياضة العنيفة وتدليك المنطقة في اليوم الأول، وتوقّع أن يبدأ الأثر خلال أيام لا فوراً. اعتنِ بقدميك: تنشيف جيد بين الأصابع، جرابات ماصّة، وتبديل الأحذية للوقاية من الفطريات. \*\*راجع فوراً عند: ضعف واضح في قوّة القبضة أو تنميل مستمر بعد حقن الكفّ، أو تدلّي الجفن أو ضعف في تعبير الوجه بعد حقن الوجه، أو صعوبة بلع أو تنفّس أو ضعف عضلي عامّ بعد الحقن، أو احمرار وتورّم ساخن وتصريف قيحي في موضع الإبرة مع حرارة. وراجع أيضاً على وجه السرعة عند تعرّق ليلي غزير مع فقدان وزن أو حرارة أو كتل في الرقبة أو الإبط، فهذه علامات تستدعي البحث عن سبب داخلي.\*\* ولا تكرّر الحقن قبل موعده أملاً بنتيجة أقوى.

## About this service

Hyperhidrosis is sweating far in excess of what the body needs for cooling — enough to soak clothing, interfere with writing, handshakes and work, and affect social life. It comes in two forms. Primary focal hyperhidrosis affects the palms, soles, armpits and face, usually starts in childhood or adolescence, often runs in families, and stops during sleep. Secondary hyperhidrosis is generalised or occurs at night and results from another cause that must be sought before any cosmetic treatment.

Assessment begins by separating the two, \*\*because heavy night sweats or newly generalised sweating — especially with weight loss, fever, enlarged lymph nodes or palpitations — calls for prompt investigation to look for a cause such as thyroid disorder, diabetes, infection, hormonal change, a new medicine or other conditions needing an entirely different treatment.\*\* Primary focal hyperhidrosis, by contrast, is diagnosed clinically from the history and examination and does not usually require extensive tests.

Treatment is stepwise. It starts with medical antiperspirants containing aluminium salts, applied to completely dry skin at bedtime rather than in the morning. Next comes tap-water iontophoresis in repeated sessions for palms and soles, then botulinum toxin injections into the armpit or palm, which locally block the nerve signal to the sweat glands. Oral anticholinergic medicines are prescribed under medical supervision because of side effects such as dry mouth, blurred vision and urinary retention, and they do not suit every patient. Energy-based devices can reduce underarm glands, and endoscopic sympathetic nerve division is reserved for very severe cases after the earlier steps have failed.

The limits matter. \*\*The aim is to reduce sweating to a tolerable level, not to abolish it: sweating is an essential cooling function and eliminating it completely is neither desirable nor safe. Injection results are temporary, typically lasting several months before repeat treatment, antiperspirants must be used consistently, and surgery can cause compensatory sweating elsewhere on the body that cannot be reversed.\*\* No treatment ends the problem permanently in a single session, and eligibility for each step depends on clinical assessment and on how much your daily life is affected.

One distinction people frequently get wrong: a deodorant and an antiperspirant are not the same thing. The first tackles odour produced by bacteria and does not reduce the volume of sweat; the second temporarily blocks the sweat ducts and does reduce it. Hyperhidrosis is not a sign of weak nerves, vitamin deficiency or poor hygiene, and it is not treated by diets or herbal mixtures.

## Preparation

Come ready to describe the problem precisely: when it began, which areas are worst, whether you sweat during sleep, how many times a day you change clothes, and how it affects your work or studies. List all your medicines and supplements, since some cause sweating as a side effect, and stop nothing on your own. Mention chronic conditions, especially thyroid disease, diabetes and heart disease, any allergies, and any accompanying symptoms such as weight loss, fever, palpitations or heavy night sweats, which take priority in assessment. Women should mention pregnancy or breastfeeding. If you are a candidate for underarm injections, do not shave on the day itself — two or three days beforehand is better — apply no antiperspirant or perfume before the appointment, and wear loose clothing that gives easy access to the area.

## Aftercare

Apply antiperspirant to completely dry skin at bedtime and wash it off in the morning; never use it on freshly shaved or irritated skin. If itching or burning appears, reduce the number of nights rather than stopping altogether, and tell your doctor. After injections, avoid intense heat, saunas, strenuous exercise and massaging the area for the first day, and expect the effect to begin over days rather than immediately. Look after your feet: dry carefully between the toes, use absorbent socks and rotate footwear to prevent fungal infection. \*\*Seek review immediately if: grip strength is clearly weak or numbness persists after palm injections; the eyelid droops or facial expression weakens after facial injections; you develop difficulty swallowing or breathing or generalised muscle weakness after injections; or an injection site becomes red, hot and swollen with pus and fever. Seek prompt review as well for heavy night sweats with weight loss, fever, or lumps in the neck or armpit, which call for a search for an internal cause.\*\* Do not bring injections forward hoping for a stronger effect.
