## Procedure steps

### 1. Confirming the diagnosis and defining the type

The diagnosis is confirmed by hemoglobin electrophoresis or chromatography together with a blood count, blood film and reticulocyte count. Full disease is distinguished from carrier trait, and any coexisting thalassemia is sought. Family members are tested when appropriate, and genetic counselling is offered before marriage and pregnancy.

### 2. Building the prevention plan

Vaccination against pneumococcus, Haemophilus, meningococcus and influenza is completed, antibiotic prophylaxis is prescribed for children according to protocol, and folic acid is given. The patient and family are taught the triggers of a crisis: dehydration, cold, infection, over-exertion, smoking and high altitude, and how to manage pain early at home.

### 3. Disease-modifying treatment

When crises recur, after acute chest syndrome, or with other risk markers, a treatment that reduces sickling and raises fetal hemoglobin is prescribed, with regular blood counts to guide response and safety. The decision, dose and duration come from the hematologist alone, and side effects and pregnancy planning are discussed before starting.

### 4. Managing the acute crisis

A crisis is treated with early, adequate, stepwise analgesia, fluids, oxygen if saturation is low, and warmth, while a search is made for infection or another trigger and the chest is assessed. Transfusion or exchange transfusion is decided for specific indications such as acute chest syndrome, stroke or a sharp fall in hemoglobin, not for every pain episode.

### 5. Long-term complication surveillance

Transcranial Doppler is performed in children to estimate stroke risk, with periodic eye examination for sickle retinopathy, monitoring of kidney function and urinary protein, lung and heart assessment, and evaluation of hip pain for avascular necrosis of the femoral head. Regular transfusion programmes or stem cell transplantation are considered for selected patients in a specialist centre.

## About this service

Sickle cell disease is an inherited hemoglobin disorder in which red blood cells take on a rigid crescent shape when oxygen is low or the body is dehydrated or cold. The cells then break down early and stick inside small blood vessels, blocking them. The result is a combination of chronic hemolytic anemia and acute pain crises in the bones, back, chest and abdomen. Over time, organs that depend on delicate blood flow may be damaged, including the spleen, kidneys, eyes, lungs, brain and the head of the femur.

Treatment rests on two arms, neither of which replaces the other: preventing crises and complications, and treating an acute crisis promptly and adequately. Prevention includes vaccination against encapsulated bacteria, antibiotic prophylaxis during childhood according to protocol, folic acid, drinking enough fluids, avoiding severe cold, extreme exertion and high altitude, and treating any infection early. When indicated, a disease-modifying treatment that reduces the frequency of pain crises and acute chest episodes is added, chosen by a hematologist with regular laboratory monitoring.

An acute crisis is a genuine medical emergency that needs early and sufficient analgesia, fluids, assessment of the trigger, and a search for infection or low oxygen. Among the most dangerous events are acute chest syndrome, stroke, splenic sequestration in children, and severe infection due to poor spleen function. For this reason, fever in a person with sickle cell disease is treated as an emergency rather than a passing cold. Complications are monitored with investigations such as transcranial Doppler in children and regular eye, kidney and lung assessment.

The limits of treatment must be stated plainly. No medicine changes the gene, and available treatments reduce crises and protect organs but do not prevent every crisis. Painkillers treat the present pain and do not prevent the next episode; hydration and warmth are preventive measures, not treatment; and transfusion is not given routinely for every pain crisis but for specific indications. One harmful myth is to regard crisis pain as exaggeration, a psychological problem, or drug-seeking behaviour, a belief that delays necessary pain relief. Stem cell transplantation and some newer therapies may be curative or disease-altering in selected patients, and that decision comes from a specialist centre after full examination and assessment.

## Preparation

Bring a record of previous crises and your current medicines including analgesics and folic acid, along with recent laboratory results and any eye or Doppler reports. Tell the doctor about current fever, cough or chest pain, about pregnancy or plans for it, and about any drug allergy. Before any operation, anaesthetic, endoscopy or dental extraction, inform the team of your diagnosis, because anaesthesia, dehydration and cold can trigger a crisis and special preparation is needed. Drink enough fluids before the visit, avoid prolonged fasting or cold exposure on the day, and carry a card stating your condition and blood group.

## Aftercare

Stick to your daily prevention plan, vaccinations and follow-up appointments, drink enough fluids especially in hot weather and after exertion, dress warmly in the cold, and do not smoke. Treat pain early according to your doctor's plan rather than waiting for it to worsen, and keep a record of crises and their triggers. \*\*Attend the emergency department immediately if you have a temperature of 38 degrees or more, chest pain, breathlessness or cough, weakness or numbness on one side, slurred speech or confusion, a sudden severe headache, a prolonged painful erection, severe abdominal pain with distension and rapid pallor in a child, pain not responding to your home plan, or jaundice and dark urine with severe fatigue.\*\* If crises become frequent, do not rely on painkillers alone; return to your doctor to review the plan.
