Adrenal Gland Insufficiency Treatment

## Procedure steps

### 1. Confirming the diagnosis and its type

Cortisol is measured in the early morning together with adrenocorticotrophic hormone, a stimulation test is performed where needed, and electrolytes and blood glucose are checked. The cause is then investigated, whether autoimmune, inflammatory, drug related or pituitary in origin, with imaging of the adrenals or pituitary as appropriate.

### 2. Starting glucocorticoid replacement and timing it

Glucocorticoid replacement is started at a dose your doctor determines, distributed through the day to imitate the body's natural rhythm, with a clear explanation of why no dose may be missed. Response is judged by symptoms, weight and blood pressure rather than by a single blood test.

### 3. Adding mineralocorticoid and monitoring electrolytes

In primary insufficiency a mineralocorticoid is added, with follow-up of sodium, potassium and blood pressure sitting and standing, and a discussion of salt requirements especially in hot weather, with exertion and with heavy sweating. The plan is adjusted according to results and to the season.

### 4. Training in sick-day rules and the emergency injection

You and the people you live with learn how the dose is increased with fever, infection and before surgery, endoscopy or a dental extraction, and how to use the emergency injection during vomiting or diarrhoea, and you carry a medical identification card or bracelet stating the diagnosis. This training is a compulsory part of treatment, not an optional extra.

### 5. Regular follow-up and adjusting the plan

The plan is reviewed every few months once stable and whenever something changes: pregnancy, surgery, intensive exercise, travel across time zones, or new medicines that interact with replacement therapy. Bone health, weight and blood pressure are also monitored over the long term.

## About this service

Treatment of adrenal insufficiency means replacing the hormones the adrenal cortex can no longer produce in adequate amounts, above all cortisol, which maintains blood pressure, blood sugar and the body's ability to withstand stress. There are two forms: primary insufficiency, in which the adrenal gland itself is damaged, and secondary insufficiency, in which the problem lies in the pituitary gland or follows prolonged steroid use that was stopped too quickly, a very common and frequently overlooked cause.

Symptoms develop slowly and mimic many other illnesses: profound fatigue that sleep does not fix, poor appetite and weight loss, dizziness on standing with low blood pressure, nausea and abdominal pain, salt craving, and in primary insufficiency darkening of the skin and mucous membranes. Diagnosis is never based on symptoms alone; it rests on a morning cortisol measurement together with adrenocorticotrophic hormone, is confirmed by a stimulation test, and is followed by investigation of the cause and type.

Treatment is regular glucocorticoid replacement distributed through the day to imitate the natural rhythm, with a mineralocorticoid added in primary insufficiency alongside monitoring of sodium, potassium and blood pressure. Doses are set and adjusted by your doctor, and the most important part of the plan is teaching you sick-day rules: how the dose is increased during fever, infection, vomiting or diarrhoea and before surgery, and how to use an emergency injection when tablets cannot be kept down. This training genuinely saves lives.

The limits are clear. Replacement compensates for the deficiency; it does not cure or regenerate the gland, and it is usually lifelong, except in some cases caused by external steroids where the gland may recover gradually under medical supervision. One of the most harmful myths is that so-called adrenal fatigue is a medical diagnosis treatable with supplements that stimulate the gland. It is not a recognised diagnosis, no supplement stimulates the adrenal gland, and relying on such products delays diagnosis of genuine insufficiency or of other diseases and exposes patients to real danger. Proven insufficiency is treated with hormone replacement only, and diagnosis is established by clinical examination and blood tests rather than by reading.

## Preparation

Book an early morning appointment where possible, because cortisol measurement depends on the time the sample is taken. Bring a written list of all your medicines and supplements, and describe in detail any steroid use over recent months in any form: tablets, asthma inhalers, drops, potent creams, or injections into joints or muscle, since this is among the commonest causes of secondary insufficiency and is often forgotten. Ask before the blood test about the timing of your morning dose, and never stop or adjust it yourself. Record your blood pressure, weight and pattern of fatigue and dizziness for a week before the visit. Tell your doctor if you are pregnant or planning pregnancy, and if you are taking supplements marketed as adrenal stimulants, and bring all your previous reports and results.

## Aftercare

Never stop your medicine or miss a dose, even if you feel completely well, because stopping abruptly can cause a life-threatening adrenal crisis. Always carry spare tablets and your emergency injection if one has been prescribed, together with a medical identification card or bracelet stating your diagnosis, and show it to any doctor, dentist or paramedic before any procedure. Apply the sick-day rules your doctor trained you in during fever, infection, diarrhoea, surgery or severe physical stress, and call your doctor if you do not improve. Do not buy supplements marketed as adrenal stimulants. \*\*Go to the emergency department immediately and tell staff that you have adrenal insufficiency if you have vomiting or diarrhoea that prevents you taking tablets, profound weakness, dizziness and low blood pressure, severe abdominal pain, a high fever, confusion, fainting or a seizure, since this is an acute adrenal crisis that needs injected treatment without delay.\*\*
