\*\*إن خرج سنّ دائم كاملاً من مكانه فالدقائق هي ما يحدّد إمكان إنقاذه: أمسكه من التاج لا من الجذر، ولا تفرشه ولا تحكّ الجذر ولا تنزع ما عليه من نسيج، وإن كان ملوّثاً فاشطفه لثوانٍ بالحليب أو بمحلول ملحي فقط، ثم أعِده إلى مكانه في التجويف فوراً وعُضّ على منديل نظيف — وإن تعذّر ذلك فاحفظه في \*\*حليب\*\* واذهب إلى طوارئ الأسنان في الحال.\*\* أمّا سنّ الطفل اللبني المخلوع فلا يُعاد إلى مكانه، لأن إعادته قد تُضرّ ببرعم السنّ الدائم تحته، ويُكتفى بمراجعة الطبيب لتقييم اللثة والعظم.

\*\*وإذا ظهر تورّم في الوجه أو الرقبة مع صعوبة في البلع أو في فتح الفم أو مع حرارة، فهذه حالة طوارئ فورية لا تنتظر موعداً.\*\* العلاج بعد ذلك يُختار بحسب الحالة: تلميع وترميم بسيط، أو حشوة مركبة بلون السنّ، أو إعادة لصق القطعة المكسورة إن كانت محفوظة سليمة، أو تلبيسة، أو تغطية لبّية، أو معالجة جذور إذا تأذّى العصب، مع تثبيت السنّ المتحرّك بجبيرة عند وجود إصابة في الأنسجة الداعمة.

ومن حدود العلاج التي يجب قولها بصراحة: الترميم يُعيد الشكل والوظيفة لكنه لا يجعل السنّ بقوّته الأصلية تماماً، ويحتاج حماية من العضّ القاسي وواقياً ليلياً إن كان السبب صريف الأسنان. والأهمّ: زوال الألم لا يعني أن العصب سلِم — قد يموت اللبّ بعد أسابيع أو أشهر بهدوء ويظهر تغيّر لون السنّ أو خرّاج، ولذلك المتابعة بالأشعة ضرورية حتى مع الشعور بالتحسّن. والشقوق الشعرية الدقيقة قد لا تظهر على الأشعة، والترشيح لكل خيار يُحدَّد بالفحص السريري والأشعة لا بالوصف عبر الهاتف.

## خطوات الإجراء

### 1. تقييم عاجل وتحديد نوع الكسر

يفحص الطبيب عمق الكسر وحركة السنّ والإطباق وحيوية اللبّ، ويطلب أشعة لاستبعاد كسر في الجذر أو في عظم الفك، ويبحث عن شذرات سنّية داخل الشفة أو اللسان. تُوثّق الإصابة وتُقيَّم الأسنان المجاورة أيضاً لأن الصدمة قد تصيب أكثر من سنّ.

### 2. السيطرة على الألم والنزف وتثبيت السنّ

يُخدَّر الموضع عند الحاجة، ويُوقف النزف وتُنظَّف الجروح، ويُغطّى العاج المكشوف بمادة عازلة لتخفيف الحساسية. إذا كان السنّ متحرّكاً أو أُعيد إلى مكانه فتُثبَّت جبيرة رقيقة على الأسنان المجاورة لمدة يحدّدها الطبيب.

### 3. الترميم بحسب عمق الكسر

الكسر السطحي يُلمَّع أو يُرمَّم بمادة مركبة، والكسر الأوسع يحتاج ترميماً أكبر أو تلبيسة، ويمكن إعادة لصق القطعة المكسورة إن حُفظت رطبة وسليمة. يُراعى الشكل واللون وخطّ الإطباق حتى لا يتحمّل السنّ ضغطاً زائداً يكسره مرة أخرى.

### 4. علاج اللبّ عند تعرّضه

إذا كان اللبّ مكشوفاً حديثاً وفي سنّ ما زال ينمو، قد يُختار علاج محافظ يحفظ حيويته بتغطية لبّية أو بتر جزئي. وإذا كان اللبّ ملتهباً بلا رجعة أو ميتاً، تُجرى معالجة الجذور على جلسة أو أكثر ثم يُدعَّم السنّ بترميم مناسب.

### 5. المتابعة وحماية السنّ من كسر جديد

تُحدَّد مراجعات دورية على أسابيع وأشهر لفحص حيوية اللبّ ولون السنّ والأشعة، لأن العصب قد يموت متأخراً بلا ألم. ويُعالَج السبب: واقٍ ليلي لصريف الأسنان، واقٍ رياضي لمن يمارس رياضات التلامس، وتعديل الإطباق، واستبدال الحشوات الكبيرة المُضعِّفة.

## Procedure steps

### 1. Urgent assessment and fracture classification

The dentist examines fracture depth, tooth mobility, the bite and pulp vitality, orders radiographs to exclude a root or jaw fracture, and looks for tooth fragments embedded in the lip or tongue. The injury is documented and neighbouring teeth are assessed too, since trauma often affects more than one tooth.

### 2. Pain and bleeding control, and splinting

The area is anaesthetised if needed, bleeding is controlled, wounds are cleaned, and exposed dentine is sealed with a protective material to reduce sensitivity. If the tooth is loose or has been repositioned, a thin splint is bonded to the neighbouring teeth for a period the dentist decides.

### 3. Restoration matched to fracture depth

A superficial chip is polished or restored with composite; a larger fracture needs a bigger restoration or a crown; and the broken fragment can sometimes be re-bonded if it was kept moist and intact. Shape, colour and occlusal contacts are adjusted so the tooth does not carry excess load and fracture again.

### 4. Managing an exposed or injured pulp

If the pulp is freshly exposed in a still-developing tooth, a conservative vital pulp therapy such as pulp capping or partial pulpotomy may be chosen. If the pulp is irreversibly inflamed or necrotic, root canal treatment is completed over one or more visits and the tooth is then reinforced with a suitable restoration.

### 5. Follow-up and preventing another fracture

Scheduled reviews over weeks and months check pulp vitality, tooth colour and radiographs, because the nerve can die late and painlessly. The cause is treated as well: a night guard for grinding, a sports mouthguard for contact sports, occlusal adjustment, and replacement of large weakening fillings.

## التحضير قبل الإجراء

لا تنتظر زوال الألم لتراجع الطبيب؛ كسور الأسنان يتحسّن مآلها كلما كان التقييم أسرع. إن وجدت قطعة السنّ المكسورة فأحضرها في وعاء صغير فيه حليب أو محلول ملحي ولا تدعها تجفّ، ولا تحاول لصقها بنفسك بأي مادة. لا تعُضّ على الجهة المصابة، وتجنّب الأطعمة والمشروبات الساخنة أو الباردة جداً. أخبر الطبيب بكل دواء تتناوله وخاصة مسيلات الدم ومضادات الصفائح والبايفوسفونات وأدوية المناعة، وبكل مكمّل عشبي، وبالحساسية لأي مخدر أو مضاد حيوي أو للاتكس. أخبره أيضاً بأمراض القلب أو الصمام الصناعي أو ضعف المناعة أو السكري، وبموعد آخر جرعة لقاح الكزاز إن كان الجرح ملوّثاً بتراب أو حدث في الخارج. إن كانت الإصابة بسبب سقوط أو إغماء أو تشنّج فاذكر ذلك، فالسقوط غير المُفسَّر يحتاج تقييماً طبياً بذاته لا يقلّ أهمية عن السنّ. أحضر أي أشعة أو تقارير سابقة، واحضر مع مرافق إن كان الطفل هو المصاب.

## العناية بعد الإجراء

التزم بالأطعمة الطرية أول أيام ولا تعُضّ على الجهة المصابة، وقطّع الطعام إلى قطع صغيرة، وامتنع عن الأطعمة القاسية واللزجة وعن قضم الثلج والأقلام. نظّف بفرشاة ناعمة حول موضع الإصابة ولا تهمل التنظيف، واستخدم غرغرة ماء وملح دافئة بعد أربع وعشرين ساعة أو الغسول الذي يوصفه الطبيب. كمادات باردة على الوجه في اليوم الأول تخفّف التورّم. إن كانت هناك جبيرة فلا تحرّكها ولا تنظّف تحتها بعنف، وراجع في الموعد المحدّد لإزالتها. راجع فوراً عند: \*\*تورّم في الوجه أو الرقبة مع صعوبة بلع أو فتح فم أو حرارة\*\*، أو \*\*ألم نابض متزايد أو ألم يوقظك من النوم\*\*، أو \*\*تغيّر لون السنّ إلى الرمادي أو الداكن\*\*، أو نزف مستمر لا يتوقف بالضغط، أو تنميل في الشفة أو الذقن، أو خروج قيح أو طعم كريه، أو عدم انطباق الأسنان على بعضها، أو حركة متزايدة في السنّ. وحتى مع اختفاء الأعراض، لا تُلغِ مراجعات المتابعة بالأشعة.

## About this service

Treatment of dental fractures is both an emergency and a restorative pathway aimed at saving the broken tooth, protecting its pulp (nerve) and restoring shape and function. It starts by defining the depth of the fracture: a superficial enamel chip; a fracture reaching dentine, which makes the tooth sensitive to cold and air; a fracture exposing the pulp, with bleeding or a red point and sharp pain; or a fracture extending below the gum line or into the root, which is the hardest to save. The cause matters too: a direct blow, biting something hard, tooth grinding, or a tooth already weakened by a large filling or old root canal treatment.

\*\*If a permanent tooth is knocked out completely, minutes decide whether it can be saved. Hold it by the crown, never the root; do not brush it, do not scrub the root and do not remove the tissue attached to it. If it is dirty, rinse it for a few seconds in milk or saline only, then put it back into its socket immediately and bite gently on a clean tissue. If replanting is not possible, store it in \*\*milk\*\* and go to dental emergency care straight away.\*\* A knocked-out baby tooth is not replanted, because replanting may damage the developing permanent tooth underneath; the child is still examined to assess the gum and bone.

\*\*Facial or neck swelling with difficulty swallowing, difficulty opening the mouth, or fever is an immediate emergency and must not wait for an appointment.\*\* Once the situation is stable, treatment is chosen to match the injury: smoothing and minor reshaping, a tooth-coloured composite restoration, re-bonding the broken fragment if it was stored properly, a crown, pulp capping, or root canal treatment if the nerve is damaged, together with splinting a loosened tooth when the supporting tissues are injured.

The limits deserve plain language. A restoration returns shape and function but never quite restores the original strength, so the tooth needs protection from hard biting and a night guard if grinding caused the fracture. More importantly, pain relief does not prove the nerve survived: the pulp can die quietly weeks or months later and show up as tooth discolouration or an abscess, which is why radiographic follow-up remains necessary even when you feel better. Fine hairline cracks may not appear on X-rays at all, and suitability for each option is decided by clinical examination and imaging, not by a description over the phone.

## Preparation

Do not wait for the pain to settle before being seen: outcomes for fractured teeth improve the sooner they are assessed. If you find the broken fragment, bring it in a small container of milk or saline and never let it dry out, and do not try to glue it back yourself with any material. Avoid biting on the injured side and avoid very hot or very cold food and drinks. Tell the dentist about every medication you take, especially blood thinners, antiplatelet drugs, bisphosphonates and immune-suppressing drugs, about every herbal supplement, and about allergies to local anaesthetics, antibiotics or latex. Mention heart disease, a prosthetic valve, immune deficiency or diabetes, and the date of your last tetanus vaccination if the wound was contaminated with soil or happened outdoors. If the injury followed a fall, a faint or a seizure, say so: an unexplained fall needs medical assessment in its own right, no less important than the tooth. Bring any previous radiographs or reports, and bring an accompanying adult if the patient is a child.

## Aftercare

Stick to soft food for the first days, do not bite on the injured side, cut food into small pieces, and avoid hard or sticky items, ice and pen chewing. Clean gently around the injury with a soft brush rather than avoiding cleaning altogether, and use warm salt-water rinses after twenty-four hours or the mouthwash your dentist prescribes. Cold compresses on the face during the first day reduce swelling. If a splint is in place, do not move it or clean under it forcefully, and attend the appointment set for its removal. Seek care immediately for: \*\*facial or neck swelling with difficulty swallowing, difficulty opening the mouth, or fever\*\*, \*\*throbbing pain that increases or wakes you at night\*\*, \*\*a tooth turning grey or dark\*\*, bleeding that does not stop with pressure, numbness of the lip or chin, pus or a foul taste, teeth that no longer meet properly, or increasing looseness of the tooth. Even if symptoms disappear, do not cancel the radiographic follow-up visits.
