Chronic Fatigue Syndrome treatment

## Procedure steps

### 1. Detailed clinical history

Assessment begins with a longer than usual consultation covering when the fatigue started and how it evolved, what worsens and eases it, how long the crash after exertion lasts, sleep pattern, concentration and memory, pain, dizziness on standing, and the effect of all this on work, study and family life. Past illnesses, operations, recent infections, travel and occupational exposure are reviewed too.

### 2. Full physical examination

The examination includes weight, blood pressure and pulse sitting and standing, assessment of the thyroid, lymph nodes, heart, lungs and abdomen, a check for pallor of the skin and mucous membranes, joint and muscle examination, and a brief neurological survey. The aim is to detect a physical sign pointing to a specific organic cause rather than relying on the verbal description of symptoms alone.

### 3. Targeted exclusion tests

Selected tests are ordered according to the history and examination, and may include a full blood count, inflammatory markers, thyroid, liver and kidney function, blood glucose, iron stores, vitamin D and B12, and urinalysis. Broad random test panels are avoided because they generate borderline results that worry patients without helping, and tests are repeated only when the clinical picture changes.

### 4. Sleep, mood and medication review

The doctor asks about snoring, witnessed pauses in breathing and leg movements during sleep, and may refer for a sleep study when apnoea is suspected. Mood, anxiety and life stress are assessed without blame or oversimplification, because depression can be a cause of fatigue, a consequence of it, or a companion to it. All medicines and supplements are reviewed, since several cause direct drowsiness.

### 5. Management plan and follow-up

A written plan is built covering how energy is distributed across the day and week, an activity ceiling not to be exceeded even on good days, a regular sleep schedule, treatment of pain or associated symptoms, and scheduled review to adjust the plan. Patients are told that improvement is usually gradual and fluctuating, and that any new symptom deserves fresh assessment rather than being attributed automatically to the syndrome.

## About this service

Chronic fatigue syndrome is a condition in which disabling exhaustion persists for six months or longer, is not relieved by rest or sleep, and worsens after even modest physical or mental effort in a flare that can last for days. It is usually accompanied by unrefreshing sleep, difficulty with concentration and memory, muscle and joint pain, headache, and lightheadedness or a racing pulse on standing. This exhaustion is different from ordinary tiredness: it is out of proportion to the effort spent and does not respond to a period of rest.

General internal medicine acts here as a diagnostic gateway that coordinates between organ systems. It is not a specialty for everything, but the discipline that assembles the whole picture before directing a case to a subspecialty when needed. No single blood test proves the syndrome, so the diagnosis rests on a careful history and on ruling out other causes of fatigue: anaemia, thyroid disease, diabetes, vitamin D or B12 deficiency, liver and kidney disease, chronic infection and autoimmune illness, obstructive sleep apnoea, depression and anxiety, and side effects of medicines the patient is already taking.

The limits of treatment must be stated plainly. No medicine cures the syndrome, vitamin infusions and supplements are not a treatment for it, and no scan or device shortcuts the process. What genuinely helps is a multi-track plan: pacing activity within the energy actually available rather than pushing into collapse, restoring sleep structure, treating pain and mood symptoms when present, controlling any coexisting illness, and reorganising daily priorities. Intensive graded exercise programmes can make symptoms worse in some patients, so movement is rebuilt slowly, under supervision, and scaled back at any sign of deterioration.

One harmful myth deserves direct correction: chronic fatigue is not weak willpower, laziness, or imagination, and blaming the patient delays diagnosis by years. At the same time, not every case of fatigue is this syndrome; some causes of fatigue are serious and fully treatable. For that reason, whether any diagnosis or plan applies to you is decided by clinical examination and direct assessment, never by reading a page online.

## Preparation

Before the visit, keep a two-week diary recording your sleep hours and wake time, your fatigue level in the morning and evening, what you did on the days you crashed afterwards, and how long that crash lasted. Bring a list of every medicine, supplement, herbal product and energy drink you take, together with all previous tests and reports, including old ones. Tell the doctor plainly about any unintentional weight loss, fever or night sweats, about snoring or witnessed pauses in breathing reported by whoever sleeps near you, and about ongoing stress, persistent low mood or distressing thoughts. Do not stop any medicine on your own before the visit, and do not start a severe diet or new supplements before the tests, because they can alter results and confuse the diagnosis. If possible bring someone who sees your daily condition.

## Aftercare

Stay within the agreed activity ceiling and resist using a good day to catch up on everything you postponed, because this is the commonest trigger of the crash that follows. Keep fixed sleep and wake times, reduce evening caffeine, drink enough fluid, and split tasks into short blocks with rest between them. Keep the diary going, since it is the best measure of improvement or decline. Do not adjust any prescribed medicine yourself, and do not buy intravenous vitamin drips or herbal mixtures marketed as a cure for chronic fatigue. \*\*Seek medical review immediately if you develop unintentional weight loss, persistent fever or heavy night sweats, enlarged lymph nodes, breathlessness or chest pain, weakness or numbness on one side of the body, confusion, unexplained bleeding, or a rapid decline that leaves you unable to care for yourself.\*\* These are not features of the syndrome and need urgent assessment.
