المتلازمة لا تحدث إلا في الحمل بتوأم بمشيمة واحدة، ولذلك يُتابع هذا النوع من الحمل بسونار كل أسبوعين تقريباً من نحو الأسبوع السادس عشر. والتشخيص سونارِيّ لا تحليلي: كثرة سائل حول أحد التوأمين وقلّته الشديدة حول الآخر حتى يبدو ملتصقاً بالجدار، مع فروق في حجم المثانة ونتائج الدوبلر، ومن ذلك تُحدَّد المرحلة التي يُبنى عليها القرار. وتوجد حالات قريبة لكنها مختلفة، منها تفاوت النمو بين التوأمين وفقر الدم-كثرة الحمر، ولكل منها تدبير خاصّ.

الخيارات تُطرح بحسب المرحلة وعمر الحمل: مراقبة لصيقة في الحالات المبكّرة الخفيفة جداً، أو تخثير الأوعية بالليزر وهو العلاج الأمثل للحالات المتقدّمة في نافذة زمنية محدّدة من الحمل، أو تفريغ السائل الزائد كإجراء مساعد أو حين يتعذّر الليزر، وفي حالات قصوى تُطرح خيارات انتقائية صعبة تُشرح باستشارة مفصّلة. والإجراء يحتاج مركزاً ذا خبرة في طبّ الأجنّة، والتحويل المبكر جزء من العلاج لا تفصيل إداري.

وحدود العلاج يجب أن تُقال بوضوح: تخثير الأوعية يوقف سبب المتلازمة لكنه \*\*لا يُصلح ما حدث من أذى قبله\*\*، ولا يمنع خطر الولادة المبكّرة أو تمزّق الأغشية، ولا يضمن نجاة التوأمين معاً، وقد تبقى وصلة صغيرة تحتاج إعادة تقييم. وليس كل تفاوت في حجم التوأمين متلازمة نقل دم، ولا كل زيادة سائل تعني حاجة لتداخل. والترشيح لكل خيار يُحدَّد بالفحص والسونار المتخصّص لا بالقراءة، ويُعاد النظر فيه أسبوعياً لأن الحالة تتغيّر بسرعة.

## خطوات الإجراء

### 1. تأكيد التشخيص وتحديد المرحلة

يُثبَت أوّلاً أن المشيمة واحدة (من سونار الثلث الأول إن توفّر)، ثم يُقاس السائل في كل كيس، وتُفحص مثانة كل جنين، ويُجرى دوبلر الشريان السري والقناة الوريدية والشريان الدماغي، ويُبحث عن استسقاء أو خلل في قلب المتلقّي. من مجموع ذلك تُحدَّد المرحلة.

### 2. استشارة الخيارات والتحويل

تُشرح الخيارات ونتائجها المحتملة: مراقبة لصيقة، تخثير الأوعية بالليزر، تفريغ السائل، والخيارات الانتقائية في الحالات القصوى. ويُنسَّق التحويل إلى مركز طبّ أجنّة قادر على التداخل، مع خطّة نقل طارئة إن تدهورت الحالة قبل الموعد.

### 3. التخدير وإدخال المنظار

يُجرى الإجراء بتخدير موضعي مع تسكين وريدي عادة، وأحياناً بتخدير ناحي بحسب الحالة. يُختار موضع الدخول على البطن بالسونار، ثم يُدخل منظار رقيق جداً إلى كيس التوأم الذي حوله سائل زائد.

### 4. تخثير الأوعية الواصلة بالليزر

تُمسح سطح المشيمة بالمنظار وتُحدَّد الأوعية الواصلة على خط الفصل بين الدورتين، ثم تُخثَّر بالليزر بأسلوب يفصل الدورتين فصلاً كاملاً قدر الإمكان، وبعدها يُفرَّغ السائل الزائد لتخفيف تمدّد الرحم.

### 5. متابعة لصيقة بعد الإجراء

يُعاد السونار والدوبلر خلال أيام ثم كل أسبوع إلى أسبوعين، لمراقبة السائل والمثانة والنمو وظهور فقر دم عند أحد التوأمين أو بقاء وصلة. وتُراقب الأغشية وعنق الرحم لخطر الولادة المبكّرة، ويُخطَّط مكان الولادة، ويُقيَّم دماغ الناجين بعد الولادة.

## Procedure steps

### 1. Confirming the diagnosis and staging

Monochorionicity is confirmed first, ideally from a first-trimester scan. Fluid volume is then measured in each sac, each twin's bladder is examined, Doppler studies of the umbilical artery, ductus venosus and cerebral artery are performed, and hydrops or recipient cardiac strain is looked for. Together these define the stage.

### 2. Counselling and referral

The options and their possible outcomes are explained: close surveillance, fetoscopic laser coagulation, amnioreduction, and selective options in extreme cases. Referral to a fetal medicine centre capable of intervening is arranged, with an emergency transfer plan if the situation deteriorates before the appointment.

### 3. Anaesthesia and fetoscope insertion

The procedure is usually performed under local anaesthesia with intravenous sedation, and sometimes with regional anaesthesia depending on the case. The entry site on the abdomen is chosen with ultrasound, and a very fine fetoscope is introduced into the sac of the twin surrounded by excess fluid.

### 4. Laser coagulation of the connections

The placental surface is surveyed endoscopically, the communicating vessels along the dividing line between the circulations are identified, and they are coagulated with the laser in a pattern that separates the circulations as completely as possible. Excess fluid is then drained to relieve uterine distension.

### 5. Intensive follow-up

Ultrasound and Doppler are repeated within days and then every one to two weeks to monitor fluid, bladders, growth, the appearance of anaemia in either twin and any residual connection. Membranes and cervix are watched for preterm birth risk, the place of delivery is planned, and survivors undergo neurological assessment after birth.

## العناية بعد الإجراء

توقّعي مغصاً وشدّاً في البطن ووجعاً بسيطاً في موضع الدخول لأيام، والتزمي بالراحة النسبية وبالأدوية الموصوفة فقط، وتجنّبي حمل الأثقال والمجهود والجماع للمدّة التي يحدّدها الفريق. راقبي حرارتك ودوّني أي إفرازات أو تسرّب، ولا تفوّتي سونار المتابعة القريب لأن الأسابيع الأولى بعد الإجراء هي الأهمّ. \*\*راجعي الطوارئ فوراً عند تسرّب سائل من المهبل، أو نزف مهبلي، أو تقلّصات منتظمة أو شدّ متكرّر يشبه المخاض، أو حرارة ثمان وثلاثين درجة أو أكثر أو رجفة، أو إفراز كريه الرائحة، أو نقص واضح في حركة الأجنّة، أو صداع مع تشوّش رؤية أو ألم أعلى البطن، أو ألم شديد لا يهدأ بالمسكّن.\*\*

## About this service

Twin-to-twin transfusion syndrome is an imbalance in blood distribution between twins who share a single placenta and are connected by communicating vessels on its surface: one twin receives more blood and fluid than it needs (the recipient) while the other remains underperfused (the donor). In advanced cases the treatment is laser coagulation of those communicating vessels through a fine fetoscope introduced into the amniotic sac under ultrasound guidance, which separates the two circulations and stops the unbalanced transfer.

The syndrome occurs only in monochorionic twin pregnancies, which is why this type of pregnancy is followed with ultrasound roughly every two weeks from around the sixteenth week. Diagnosis is sonographic rather than laboratory-based: excess fluid around one twin and severe deficiency around the other until it appears stuck against the uterine wall, together with differences in bladder size and Doppler findings. From these features a stage is assigned, and the stage guides the decision. Related but distinct conditions exist, including selective growth discordance and the anaemia-polycythaemia sequence, each with its own management.

Options depend on stage and gestational age: close surveillance in very early mild cases; fetoscopic laser coagulation, which is the preferred treatment for advanced disease within a defined window of pregnancy; amnioreduction as an adjunct or when laser is not feasible; and, in extreme situations, difficult selective options that are explained in detailed counselling. The procedure requires a centre experienced in fetal medicine, and early referral is part of the treatment rather than an administrative detail.

The limits deserve plain statement. Laser coagulation removes the cause but \*\*cannot undo damage that occurred before it\*\*, does not eliminate the risk of preterm birth or membrane rupture, cannot guarantee the survival of both twins, and occasionally leaves a small residual connection that requires reassessment. Not every size difference between twins is transfusion syndrome, and not every increase in fluid means an intervention is needed. Suitability for each option is determined by specialist examination and ultrasound rather than by reading, and it is reviewed weekly because the situation changes quickly.

## Preparation

Bring all previous scan reports, especially the first-trimester scan that establishes chorionicity, and a list of your medicines and supplements. Follow fasting instructions if sedation is planned, and tell the team about any drug or antiseptic allergy, any blood thinners or aspirin, your blood group and rhesus status, and any active infection or previous uterine or abdominal surgery. Ask about the expected length of stay, the emergency plan and the direct contact number after discharge, and if you live in another governorate arrange an escort and somewhere to rest near the centre. Ask for the possible outcomes and the follow-up plan to be given to you in writing before the procedure, not after it.

## Aftercare

Expect cramping, abdominal tightness and mild soreness at the entry site for some days. Rest relatively, take only the prescribed medication, and avoid heavy lifting, exertion and intercourse for as long as the team advises. Monitor your temperature and note any discharge or leakage, and never miss the early follow-up scan, because the first weeks after the procedure matter most. \*\*Seek emergency care immediately if fluid leaks from the vagina, if you have vaginal bleeding, regular contractions or repeated labour-like tightening, a temperature of 38C or above or shivering, foul-smelling discharge, a clear reduction in fetal movements, headache with blurred vision or upper abdominal pain, or severe pain not relieved by your painkiller.\*\*
