## Procedure steps

### 1. Clinical diagnosis and exclusion of mimics

Diagnosis rests on neurological examination: bradykinesia with rest tremor or rigidity, and asymmetric distribution of symptoms. Medicines that can induce parkinsonism are reviewed, essential tremor, vascular disease and other disorders are excluded, and imaging is requested when atypical features are present.

### 2. Individualised drug plan with a timing schedule

Treatment is chosen according to age, functional disability and comorbidity, and a clear dose timing schedule is written down, because regularity of timing affects control more than most patients expect. Possible effects such as nausea, postural hypotension and drowsiness are explained, with strong emphasis on never stopping the medication abruptly.

### 3. Movement and speech rehabilitation programme

A structured programme includes gait training with external rhythm or counting, exercises for movement amplitude, balance, strength and flexibility, intensive voice training where speech has weakened, and occupational therapy to simplify home tasks and prevent falls. Consistency matters more than intensity, and gains are lost quickly once activity stops.

### 4. Managing non-motor symptoms

Sleep problems, constipation, orthostatic hypotension, urinary symptoms, pain, anxiety, depression and reduced smell are treated in parallel, and cognition is reviewed periodically. These symptoms are frequently neglected even though they are among the largest causes of reduced quality of life, and addressing them can help more than adjusting a motor drug dose.

### 5. Periodic review and advanced options

Review every few months allows dose timing to be refined and motor fluctuations or drug-induced involuntary movements to be addressed. In selected patients who still respond to medication but suffer disabling fluctuations, candidacy for deep brain stimulation or continuous delivery systems is assessed, with an explicit explanation of what these options do and do not achieve.

## About this service

Parkinson's disease is a degenerative neurological condition affecting brain cells that produce dopamine, which produces its characteristic motor features: slowness of movement, muscle rigidity, a tremor that typically appears at rest, and later difficulty with balance and walking. The disease is not only motor. Many patients experience, before or alongside the motor symptoms, reduced sense of smell, constipation, sleep behaviour disorder, blood pressure drops on standing, and changes in mood and anxiety. These non-motor symptoms often affect quality of life more than the tremor itself.

Diagnosis is primarily clinical, based on neurological examination, the pattern of symptoms and their response to treatment; imaging is used to exclude other causes rather than to confirm the disease. It is important to exclude parkinsonism caused by certain medicines or by vascular disease, and equally important to know that tremor is not always Parkinson's: essential tremor is more common and differs entirely in its features and its management.

The goal of treatment must be honest and explicit. No medicine available today cures Parkinson's disease or halts the degeneration of cells. The aim is to control symptoms, preserve as much movement, independence and daily function as possible, delay complications such as falls, and manage non-motor symptoms. This rests on three pillars: an individualised drug plan built on replacing dopamine or supporting its action, with careful attention to dose timing; a consistent movement rehabilitation programme; and parallel management of non-motor symptoms.

Rehabilitation is part of treatment, not an optional extra: gait training with external cueing, amplitude, balance and strength exercises, intensive voice training for those whose speech has weakened, and occupational therapy to simplify daily tasks. At certain stages, in selected patients who still respond well to medication but suffer troublesome fluctuations, deep brain stimulation or continuous drug delivery systems are discussed. Their limits are clear: deep brain stimulation does not cure the disease or stop its progression, and it generally does not improve symptoms that do not respond to dopamine, such as advanced balance impairment or cognitive decline. Dietary supplements do not change the course of the illness. Suitability for any option is determined by clinical assessment and specialist team review.

## Preparation

Bring a written list of all your medicines and supplements with the times you actually take them, not only the theoretical doses, because dose timing is fundamental to Parkinson's control. Before the appointment, keep a diary for a full day: when you feel the medication is working and when its effect wears off, when tremor or freezing while walking appears, and whether you fall or nearly fall. Record a short video while walking or during tremor if possible, as video shows what is hard to describe. Tell the doctor about dizziness on standing, hallucinations, vivid or violent dreams, movement during sleep, and any memory problems, and mention any drugs you have taken for nausea or psychiatric conditions, since some can produce similar symptoms. Wear comfortable shoes and bring someone with you if your walking is unsteady.

## Aftercare

Take your doses at exact clock times rather than approximately, use an alarm or a weekly pill organiser, and never stop Parkinson's medication abruptly on your own, since sudden withdrawal can cause severe motor deterioration and a dangerous condition requiring hospital care. Move every day and stay with your rehabilitation programme, drink enough water and eat fibre to prevent constipation, and stand up slowly in two stages to reduce dizziness. Remove loose rugs and fit bathroom grab rails to lower fall risk, keep regular sleep hours, and report any change in mood or thinking openly. \*\*Seek urgent care for fever with severe muscle rigidity or confusion after stopping or changing medication, a fall with head injury, new hallucinations or confusion, heavy drowsiness or sudden sleep attacks, blood pressure drops with fainting, difficulty swallowing or repeated choking, or acute motor deterioration over days.\*\*
