## Procedure steps

### 1. Describing the pattern and its triggers

When, where and for how long the child bites is established, whether nails alone or the surrounding skin too, and whether it rises with screens, homework, boredom or pressure. A simple log is kept at home and school, because impressions alone mislead.

### 2. Assessing stress, sleep and nail condition

Anxiety, school pressure, sleep and changes at home are assessed, and the effect on nails and surrounding skin is examined. Where there is infection, pain or clear deformity, medical review is advised alongside the behavioural work.

### 3. Habit awareness training

The child learns to notice the moment before biting: the position of the hand, the sensation in the fingers, the situation itself. Self-monitoring and a light agreed signal from the family are used, because awareness is an indispensable condition for any alternative.

### 4. A competing response and reinforcement

The child learns an action that occupies the hand and competes with biting for a few seconds when the urge appears: clenching the fist, a small ball, hand cream, or a nail file. This is backed by praise and simple rewards for successful days and situations rather than for the final outcome.

### 5. Follow-up, measurement and widening the assessment

Progress is measured by nail growth and the number of situations the child resisted rather than by impressions, and the plan is adjusted periodically. The assessment widens if hair pulling, skin picking, repeated bleeding, marked anxiety or other compulsive behaviour appears.

## About this service

Treatment for nail biting (onychophagia) is an assessment and behavioural training service delivered by a child mental health specialist when a child repeatedly bites their nails or the skin around their fingers to the point of damage, embarrassment, or resistance to every reminder. The work begins by describing the pattern: is the biting automatic and unnoticed — during screens, homework or car journeys — or does it rise in specific situations such as exams, conflict at home, or bedtime? That distinction decides which training is used.

Nail biting is a body-focused repetitive behaviour, very common in childhood and adolescence, and its function is usually to discharge tension, fill empty time, or simply an automatic habit that has settled in. It is therefore \*\*a message or a habit, not bad parenting and not bad manners\*\*, and for most of the time the child genuinely does not notice they are biting — which explains why repeated reminders fail. \*\*Seeing a specialist is not a stigma and not a parenting failure\*\*; it offers an alternative to a daily battle that turns fingernails into a war zone and adds shame, which in turn increases the biting.

The limits are clear. Biting in itself \*\*is not evidence of a psychiatric disorder\*\*, and most children taper off over time; the service does not stop it within days and does not rely on willpower alone. Bitter nail paint or gloves, without training in awareness and a competing response, rarely produce a lasting result. \*\*An important warning: never put pepper, caustic substances, or anything not intended for children on your child's fingers, do not bind their hands, and do not punish a behaviour they cannot even notice. That causes harm and shame and drives the biting underground.\*\* Nor is biting free of physical cost: it can deform the nail plate, cause painful infection around the nail, damage gums and teeth, and carry germs into the mouth.

The family is part of the treatment: often someone else at home bites too, and models are learned by watching; the household is asked to cut back reminders and teasing and to provide other outlets. Assessment draws on \*\*more than one source\*\* — parents, teacher, observation of the child and the state of the nails — never a single session, and it widens if hair pulling, skin picking or marked anxiety appear alongside. \*\*Be wary of any centre promising a fast cure or a "permanent end" within days, or a programme that does not publish the qualifications of whoever delivers it.\*\*

## Preparation

For about a week before the appointment, keep notes: in which situations your child bites, whether they notice it or not, whether it rises with boredom or with pressure, when exactly it began, and whether it coincided with an event at home or school. With their agreement, photograph the nails and fingertips to document the starting point, and note any pain, swelling or infection. Write down honestly everything you have already tried — reminders, bitter paint, gloves, punishment, teasing — and what each achieved. Notice too whether anyone else at home bites their nails, because modelling is part of the picture and nobody is being blamed for it. Most importantly: do not remind or scold your child in the days beforehand and do not describe the habit in front of others, and never present the session as a punishment; tell them you are visiting someone who helps children drop a habit that bothers them too.

## Aftercare

Stop the repeated reminders, the teasing and the slapping of hands; all of them raise tension, which raises biting and drives it out of sight. Use only the light signal you agreed on, praise immediately every situation the child resisted even once, and reward the attempt rather than "perfect nails". Keep a nail-care routine going: regular short trimming, cleaning and moisturiser, because a rough edge is itself an invitation to bite. Provide alternative outlets during high-risk times such as screen use and homework, and review academic pressure and sleep. Log progress weekly so you follow the trend rather than a single day, and expect setbacks around exams without treating them as a return to zero. \*\*Seek help immediately, not an appointment weeks away, if the child talks about hurting themselves or wanting to die, withdraws severely and stops eating and sleeping, shows a sudden collapse in functioning, or if biting is joined by hair pulling or skin picking until it bleeds.\*\* \*\*See a doctor without delay for swelling, redness, pus around the nail or severe pain, and any sign of abuse or violence towards the child requires immediate protective action.\*\*
