## Procedure steps

### 1. Correct first aid at the scene

Move the person away from the heat source safely, cool the area under running cool water for ten to twenty minutes, and remove jewellery and non-adherent clothing. No ice, toothpaste, oil or traditional pastes. For an electrical burn cut the power before touching the casualty; for a chemical burn irrigate for longer and remove contaminated clothes, then cover the wound with clean cloth.

### 2. Assessing depth, extent and risk

The team estimates the burned surface as a percentage of total body area and judges depth from skin colour, moisture, blistering and pain sensation, classifying it as superficial, partial thickness or full thickness. They ask about mechanism, contact time and possible smoke inhalation, review chronic illnesses and tetanus status, then decide between outpatient care and referral to a burns unit.

### 3. Wound cleansing, blister care and dressings

The wound is cleansed with a gentle solution, loose devitalised tissue is removed, and blisters are managed according to size and site: small ones are usually left intact, while large or easily ruptured blisters may be drained with sterile instruments in the clinic, never at home. A suitable ointment or specialised dressing is then applied to keep moisture balanced, with a scheduled change, infection checks and pain relief beforehand.

### 4. Surgical excision and skin grafting

For deep burns that will not close on their own, dead tissue is excised in theatre under anaesthesia and the surface is covered with a thin sheet of skin taken from a healthy donor site, or with a temporary substitute dressing until the bed is ready for grafting. Where a circumferential burn compresses a limb or the chest, an urgent releasing incision may be needed to protect circulation and breathing.

### 5. Rehabilitation, scar care and preventing contracture

Once the wound is closed a longer phase begins: range-of-motion exercises to stop scar tissue tightening across joints, splints or pressure garments when indicated, silicone sheeting and moisturising massage for the scar, and strict sun protection for months because new skin pigments easily. Follow-up visits assess function and whether later reconstructive surgery is needed for a scar that limits movement.

## About this service

Burn care and burn surgery form a single pathway that begins with assessing the depth, surface area, site and mechanism of the burn, then covers wound cleansing and dressing, infection surveillance, pain control and fluid replacement, and may extend to surgery such as removal of dead tissue and skin grafting for deep burns. The order of priorities never changes: protect life and limb first, close the wound as early as possible, then limit scarring and the contractures that tighten skin and restrict joint movement.

What happens in the first minutes shapes everything that follows. Remove the heat source, then run \*\*cool (not iced) running water over the area for ten to twenty minutes\*\*. Do not use ice or iced water: over-cooling constricts blood vessels and can deepen the injury. Do not apply toothpaste, oil, ghee, coffee, salt, egg or any traditional paste. These are widespread myths that contaminate the wound, trap heat inside it and hide the true depth from the treating doctor, delaying the right decision and raising the risk of infection. Take off rings, watches and any clothing that is not stuck to the skin, leave adherent material alone, cover the area with a clean non-adherent dressing or clean cloth, and seek assessment.

Go to the emergency department immediately if the burn involves the face, neck, hands, feet, joints or genital area, if it encircles a limb or the chest, if the skin looks white, dry brown or charred, or has lost pain sensation, or if it is wider than the injured person's own palm. The same applies to electrical and chemical burns, to smoke inhalation with hoarseness, sooty sputum or breathing difficulty, and to burns in children, pregnant women, older adults and people with diabetes or reduced immunity, as well as when tetanus vaccination is not up to date. Chemical burns need prolonged irrigation with running water and removal of contaminated clothing; never try to neutralise them with another substance.

The limits of treatment matter as much as its benefits. Surgery closes the wound; it does not restore the skin to its original state, so differences in colour and texture usually remain and scars may need months of pressure therapy, silicone sheeting and sun protection. A skin graft requires a healthy donor site that also heals with a mark of its own, and topical antibiotic cream alone neither prevents infection nor replaces regular cleansing and dressing changes. Burn depth can evolve over the first forty-eight hours, so it is reassessed rather than judged once. Suitability for any surgical step is decided by clinical examination of depth and extent, not by reading about it.

## Preparation

Tell the doctor exactly how and when the burn happened: steam, hot liquid, flame, electricity or a chemical, and name the substance if you know it. Mention chronic illnesses, especially diabetes, heart disease and any condition that weakens immunity, list all your medicines including blood thinners and steroids, and report drug allergies or allergy to adhesive dressings, along with the date of your last tetanus vaccination. Do not pop blisters and do not apply coloured ointments or household remedies before assessment, because they mask the true depth of the burn. If surgery is a possibility, follow the anaesthetist's fasting instructions precisely, bring previous reports and any photograph you took of the wound on day one, and arrange someone to drive you home rather than driving yourself.

## Aftercare

Keep every dressing appointment and do not postpone them, raise the injured limb above heart level to reduce swelling, and move the nearby joints through the prescribed exercises even if slightly uncomfortable, because immobility lets scar tissue tighten. Keep the dressing clean and dry, take the pain reliever before dressing changes as instructed, and eat protein-rich food with plenty of fluids, since healing consumes a great deal of energy. Keep new skin out of the sun and use sunscreen only once your doctor approves.

Seek care immediately with \*\*fever or chills\*\*, \*\*increasing pain, spreading redness around the wound, cloudy discharge or a bad smell\*\*, \*\*numbness, coolness, pallor or severe swelling in the limb below the burn\*\*, breathlessness, a hoarse voice or worsening cough, an enlarging burn or one turning white or brown, or inability to eat and drink.
