## Procedure steps

### 1. Interview and history

It begins with a detailed interview about the complaint, its onset and course, together with your medical, neurological and psychiatric history, medication, sleep, education, occupation and first language. A relative who knows the details of the change is ideally present, because a person's awareness of their own performance may differ from what those around them observe.

### 2. Selecting the test battery

Tests are selected according to the referral question: memory, attention, language, executive function, or a comprehensive profile. Your language, education, and sensory and motor status are taken into account, and mood and anxiety measures are added along with validity checks of effort, so that the results genuinely represent your true ability.

### 3. Conducting the session

Tests are administered in one or more sessions lasting several hours with rest breaks, using paper and computerised tools: recall of words, stories and figures, concentration and speed tasks, naming and verbal fluency, drawing and copying, and sequencing and planning. There are no needles, no radiation and no pain; the expected fatigue is mental rather than physical.

### 4. Scoring and normative comparison

Scores are calculated and compared with norms corrected for age, education and language, then the pattern of strengths and weaknesses across functions is examined rather than any single score. Mood, sleep, pain and medication effects are weighed before any conclusion, and results are compared with a previous assessment where available to establish the direction of change.

### 5. Report, recommendations and rehabilitation

Results are explained to you in plain language at a feedback session, and a report is provided to the referring physician setting out the affected functions, their functional impact and the suggested differential diagnosis. A practical plan follows: compensation strategies, study or work adjustments, safety and driving recommendations, cognitive rehabilitation, and a date for reassessment.

## About this service

Clinical neuropsychology is the specialty that measures mental functions with standardised, objective instruments in order to relate your actual performance to the state and function of your brain. Assessment covers short- and long-term memory, attention and sustained concentration, information-processing speed, language and naming, visual and spatial perception, executive functions such as planning, organisation and impulse control, and mood and behaviour.

It is requested after head injury and concussion, after stroke, in epilepsy, after brain tumours or their surgery, in neurodegenerative conditions such as Alzheimer's disease and other dementias, Parkinson's disease and multiple sclerosis, in the assessment of attention deficit and learning difficulties, in unexplained complaints of poor memory or concentration, and to define functional impact before epilepsy surgery or when judging fitness to drive or to return to work or study. It is also the tool used to measure response to cognitive rehabilitation programmes.

Its greatest value lies in differentiation. Many complaints of memory and concentration are caused by depression, anxiety, insufficient sleep, sleep apnoea, chronic pain, thyroid dysfunction, vitamin deficiency or the effect of prescribed medication — not by brain damage. The pattern that emerges across tests helps separate these treatable causes from early degenerative disease, and that distinction changes the whole direction of management.

The limits must be stated honestly. The assessment does not replace brain imaging or the neurological examination and it does not deliver a biological cause on its own; a single result never makes a diagnosis but is read together with history, examination, imaging and laboratory findings. Its scores are not a label of intelligence and not a verdict on someone's future. Performance is affected by poor sleep, fatigue, pain, anxiety and prescribed sedating medication, which is why testing is postponed if you are unwell or have not slept, and repeated later to measure change. It is also not a treatment in itself: it is the step on which a plan is built, and no test strengthens memory simply by being taken.

What the patient receives is a detailed report comparing performance with norms corrected for age, education and language, identifying intact and affected functions, and translating this into practical recommendations: compensation strategies, adjustments at work or in study, safety decisions, a cognitive rehabilitation programme, and any referrals needed to a treating physician.

## Preparation

Sleep properly the night before, since insufficient sleep alone lowers performance and distorts the result. Have your usual breakfast and keep your caffeine at its normal level — neither more nor abrupt withdrawal. Bring your glasses and hearing aid if you use them, because the assessment relies on vision and hearing. Bring a complete list of your medication with doses as prescribed, your medical reports, MRI or CT images, and any previous psychological or neurological assessment. Do not stop any medication on your own before the session, and tell the examiner if you take anything that causes drowsiness. Inform them in advance of your first language, level of education and type of work so that appropriate instruments are chosen. Bring a relative who knows the details of the change in your memory or behaviour. Allow enough time, as the session may run for hours, and avoid scheduling anything pressing afterwards. If you are ill, feverish, sleep-deprived or in severe pain on the day, ask to postpone — testing under poor conditions gives an inaccurate picture of you.

## Aftercare

\*\*If you ever have thoughts of harming yourself or ending your life, this needs urgent help immediately: do not wait for the report appointment — contact your clinician, go to the nearest emergency department, or ask someone you trust to stay with you.\*\* Seek emergency care too for sudden weakness on one side, slurred speech, facial drooping or sudden loss of vision (suspected stroke); a sudden severe and unfamiliar headache; confusion with fever; a new seizure; or rapid deterioration in awareness and behaviour over days. After the session: mental fatigue and even frustration are expected, because the tests are designed to reach the limit of your ability — rest, and do not judge yourself by your performance on a single task. Do not rely on your own impression of how you did; wait for the report and the feedback session and ask about anything you did not understand. Apply the practical recommendations: a regular sleep routine, physical activity, compensation strategies such as a diary, reminders and a fixed place for keys and medication, and less multitasking. Treat what is treatable with your doctor — sleep, mood, thyroid, pain — never adjust prescribed medication yourself, and keep the reassessment appointment, because the direction of change matters more than a single measurement.
