## Procedure steps

### 1. Clinical assessment and X-ray imaging

The limb is examined to define the position of the extra digit and what it contains — bone, joint or tendon — along with the function, stability and axis of the neighbouring digits, and an X-ray is requested, sometimes repeated as the bone matures, to plan the repair precisely.

### 2. Excluding syndromes and pre-anaesthetic assessment

The child is examined for associated anomalies of the heart, kidneys, spine or face, genetics input is requested if syndromic features or a family history are present, and a paediatric anaesthetist assesses weight, growth, breathing and any recent infection.

### 3. Surgical planning and timing with the team

The type of repair and whether stages are required is chosen, timing is set according to the child's age, growth, anaesthetic safety and school entry, and the family is told what to expect in appearance, function, risks and rehabilitation time before informed consent.

### 4. Surgery and reconstruction

Surgery is performed under general anaesthesia: the extra digit is removed together with its bone and cartilage, the collateral ligament, tendon and axis of the retained digit are reconstructed, skin and nail are reshaped, and the limb is held temporarily with a cast or a wire removed at the planned time.

### 5. Aftercare, rehabilitation and growth follow-up

The cast is protected and the wound monitored, then occupational therapy begins with range-of-motion, grip and play-based hand use, and the child is followed for years because growth may reveal deviation, stiffness or the need for a later corrective stage.

## About this service

Surgical treatment of polydactyly corrects an extra digit on the hand or foot and is one of the most common congenital limb differences. The presentations vary widely: the extra part may be a soft-tissue nubbin attached by a narrow stalk with no bone, a fully formed digit with its own bone, joint, tendon, nerves and vessels, or a partial duplication that shares bone or tendon with the neighbouring digit. Its position is classified as being on the little-finger side, on the thumb side, or as a central pattern between the digits, which is the most demanding to plan.

The aim of surgery is functional as well as cosmetic: a hand that can grip and pinch on a straight axis, or a foot that fits a shoe without pain or rubbing. \*\*The result is therefore not judged simply by the number of digits afterwards.\*\* In thumb duplication, for example, removing the extra part is not enough; the collateral ligament must be reconstructed, tendon balance restored, axis deviation corrected, the skin reshaped and sometimes the bone realigned. Without this, deviation, an unstable joint or a weak grip can appear later.

\*\*The timing of surgery follows the child's growth and anaesthetic safety rather than the family's wish to remove the appearance quickly, and it is decided within a multidisciplinary team\*\* that includes paediatric or hand and plastic surgery, paediatric anaesthesia, occupational and physical therapy, and sometimes genetics, cardiology or nephrology input if the difference is part of a syndrome or accompanied by other features. Repair is often preferred within the first one to two years, before a fixed pattern of hand use develops and before school entry. \*\*More than one surgical stage may be needed\*\*, particularly in central patterns, thumb duplication, or when webbing between the digits is also present, with rehabilitation between stages.

\*\*One dangerous practice must be named plainly: tying the extra tissue with a thread at home or cutting it without sterile technique. This causes infection and bleeding and often leaves a bony remnant or a painful nerve nodule that requires more difficult surgery later.\*\* Even a thin nubbin should be dealt with medically. The limits of the result should also be clear: surgery improves both appearance and function, but it does not produce a hand identical to the other one, and a small difference in size, movement, a scar or an incomplete nail may remain. Whether your child is a candidate, and which repair at which age, is decided by clinical examination and X-ray imaging, not by reading or by comparing your child with another.

## Preparation

Bring pregnancy and birth records, the immunisation book and growth chart, any previous X-rays or reports, and clear photographs of the hand or foot in several positions if the appearance changes with movement. Tell the surgeon about any similar condition in the family, about any cardiac, kidney, hearing or spinal problem in your child, about medicines and allergies, and about any recent infection, cold, fever or cough, since these may postpone anaesthesia. Follow the fasting instructions before anaesthesia exactly according to your child's age and what they take, whether milk or solids, and ask how long is allowed for each. Remove any ring, thread or plaster placed on the extra digit and never attempt to tie or cut it at home. Prepare loose clothing that fits over a cast, bring a familiar toy to settle your child, arrange for two adults to accompany you home if possible, and ask about the length of stay and when the cast or wire will be removed.

## Aftercare

Keep your child's hand or foot elevated on a pillow in the first days to reduce swelling, and keep the cast completely clean and dry using a waterproof cover for bathing. Give the prescribed pain relief on schedule, keep your child occupied with quiet activities, and avoid running, jumping and play in sand, water or soil until it is allowed. Do not lift the cast or dressing or move the wire, and do not apply ointments, herbs or dyes to the wound. Check the exposed fingertips or toes for colour, warmth and movement. Once the cast is removed, attend occupational therapy and do the range-of-motion and grip exercises, and encourage using the hand in play, because neglecting this leaves stiffness; keep annual follow-up as your child grows. \*\*Go to emergency immediately for fever or shivering, increasing swelling, redness, pus or a foul smell from the wound, bleeding that soaks the dressing, cold, pale or bluish fingertips, numbness or inability to move the digits, escalating pain not relieved by analgesia, persistent inconsolable crying, or breathlessness or repeated vomiting after anaesthesia.\*\*
