## Procedure steps

### 1. Diagnosis and differentiating the lump

The ophthalmologist examines the lid on the slit lamp and everts it to see the inner surface, distinguishing a chalazion from a stye, an epidermal cyst and eyelid tumours, and assesses any effect on the cornea and on vision. A lump that recurs in the same site in an adult is sent for histology.

### 2. Conservative treatment first

Management starts with warm compresses several times a day, gentle massage towards the lid margin, and lid-margin cleaning, together with treatment of any blepharitis or meibomian gland dysfunction. This stage is given enough time before any incision, because many lumps resolve with it alone.

### 3. Local anaesthesia and lid stabilisation

The lid is cleaned and local anaesthetic is injected into the area, then a special clamp everts and stabilises the lid while limiting bleeding. The procedure is done while you are awake in a clinic or minor-operations room; in children a short general anaesthetic is preferred to guarantee stillness.

### 4. Incision and curettage

A small incision is usually made from the inner surface of the lid so no external mark shows, or through the skin when the lump lies superficially or a specimen is needed. The fatty content and inflammatory tissue are curetted out, and a skin incision may be closed with a fine suture, while an internal one usually needs none.

### 5. Aftercare and preventing recurrence

Gentle pressure and ointment are applied for a short period, and most people return to normal activity the same or the next day. Continuing warm compresses and lid-margin hygiene after healing is what reduces the chance of a new lump, because the procedure does not treat the tendency of the glands to block.

## About this service

The lump people call a fatty cyst of the eyelid is usually a chalazion: one of the oil glands along the lid margin becomes blocked, its secretion is trapped inside the tissue, and a closed, non-infectious inflammatory reaction forms. It appears as a firm round lump within the body of the lid, typically painless or only mildly tender at the start. It is different from a stye, which is a red, painful, pus-forming swelling on the lid margin itself caused by infection at the base of an eyelash.

Treatment does not begin with surgery. Most of these lumps settle with repeated warm compresses, lid massage, lid-margin hygiene, and treatment of any associated condition such as blepharitis, meibomian gland dysfunction or rosacea, and resolution can take several weeks. Incision and curettage is considered when the lump persists after an adequate period of conservative care, when it is large enough to distort the lid, when it presses on the cornea and blurs vision, or when it keeps coming back in the same spot.

The limits of the operation are clear. It removes this particular lump but does not treat the underlying reason the glands block, so a new lump can appear elsewhere if daily lid-margin care is not maintained. It is not a treatment for an acute stye, it does not prevent future ones, and it does not correct a droopy lid or change the shape of the eye. A small scar, a slight notch in the lid margin, or a change in lash growth at the site remain possible.

Do not squeeze the lump or open it with a needle at home; squeezing spreads inflammation through the lid tissue and makes things worse. One important caution: a lump that recurs in exactly the same place in an adult, causes loss of lashes, or hardens and ulcerates the lid margin needs a biopsy to exclude a sebaceous gland tumour. Suitability for surgery and the type of incision are decided by clinical examination, not by guesswork.

## Preparation

Tell your doctor about every medicine you take, particularly blood thinners, anticoagulants and herbal supplements, and do not stop any of them yourself; only the prescribing doctor decides the timing. Say whether this lump has appeared in the same spot before, how many times, and what was used for it, because recurrence in one location changes the plan and calls for histology. Mention any allergy to local anaesthetic or dressings, and any diabetes or clotting disorder. Do not squeeze the lump or try to open it. Remove make-up and contact lenses before the appointment, bring an escort if the patient is a child or if general anaesthesia is planned, and follow the fasting time set by the anaesthetic team.

## Aftercare

Use the drops or ointment exactly as prescribed, and start warm compresses once your doctor allows, to help the remaining glands drain. Do not rub the eye or squeeze the lid, and postpone make-up, contact lenses and swimming until the area has been reviewed. Expect swelling, bruising, a gritty feeling under the lid and watering that improve over days to about two weeks, with mild firmness at the site sometimes lasting longer. \*\*Seek care immediately if you develop severe worsening pain, any drop in vision, swelling and redness spreading into the eye socket or cheek, fever, bleeding that does not stop with gentle pressure, or heavy pus discharge.\*\* Also return if the lump comes back in the same place, lashes are lost there, or the lid margin ulcerates, because a biopsy is then needed to exclude a tumour.
