## Procedure steps

### 1. Establishing why the lip is enlarged

The doctor examines the lip and asks how long the fullness has been present, whether it is stable, and about any previous injections, recurrent swelling, allergy or trauma. If the cause is filler, inflammation or a medical condition, surgery is deferred or cancelled and the cause is treated first, rather than excising healthy tissue.

### 2. Planning and measurement with the patient upright

The proportion of upper to lower lip and their relationship to the teeth and chin are measured, and the amount of tissue to remove is calculated conservatively. The incision is drawn on the inner lip, away from the vermilion border, and it is explained that under-correcting is safer than over-correcting.

### 3. Anaesthesia and antisepsis

The mouth is rinsed with antiseptic and local anaesthetic is injected precisely, taking care not to distort lip shape before the final measurement. Full anaesthetic effect is awaited, and sterile instruments and good lighting are prepared, since clear vision is essential for a symmetrical incision on both sides.

### 4. Measured removal of excess tissue

A strip of mucosa and underlying tissue is excised in the planned shape and amount, preserving the minor salivary glands and sensory nerve as far as possible. Bleeding is controlled and the shape is reassessed with the patient semi-upright before closure, with any small residual excess trimmed cautiously.

### 5. Closure, follow-up and judging the result

The incision is closed with fine, usually absorbable sutures, and instructions on diet, oral hygiene and warning signs are given. A review is arranged at one week and again at one to three months, with a clear explanation that the final shape cannot be judged until the swelling has fully resolved.

## About this service

Lip reduction is a surgical procedure in which excess mucosal tissue is removed from the inner surface of the upper lip, the lower lip, or both, so that the lip projects less and its size sits in better proportion with the rest of the face. It is performed from inside the lip, leaving no visible skin scar, and is usually done under local anaesthesia in a clinic.

It is typically requested for a markedly prominent lip present from birth, enlargement after trauma, imbalance between the two lips, or bulk that causes functional problems such as repeatedly biting the lip or difficulty closing the mouth. The amount of tissue to be removed is planned carefully with the patient sitting upright, preserving the natural lip border and the lip's ability to move and express.

Suitability is determined by clinical examination and by first understanding why the lip is large. The surgeon assesses the proportion of the two lips to each other and to the teeth and chin, and distinguishes congenital fullness from inflammatory swelling, from previously injected filler, or from a medical condition causing recurrent swelling. \*\*If the volume comes from filler, excision is not the answer\*\*: the correct route is to wait for absorption or, for products that allow it, to dissolve them enzymatically. Removing healthy tissue in that situation loses it for nothing.

The limits are explicit. Lip reduction reduces tissue bulk; it does not treat lip pigmentation, dryness or cracking, does not change tooth position, bite or jaw projection, and does not alter the philtrum or the distance between lip and nose. Perfect symmetry is not realistic, and swelling masks the outcome for weeks, so the result cannot be judged early. Most importantly, removed tissue cannot be replaced: over-reduction is a hard mistake to repair and produces a tight or unnatural lip on smiling.

Verify that the facility is licensed and the surgeon is certified in plastic surgery or oral and maxillofacial surgery through the official Jordanian authorities. The lip is a functional organ for speech, eating and expression, not merely an aesthetic detail, and the decision to remove tissue from it does not belong in an unlicensed setting or in unqualified hands.

## Preparation

Tell your surgeon about all medicines and supplements you use, especially blood thinners and anti-inflammatory drugs, and stop a long-term medicine only on medical advice. Disclose any clotting disorder, diabetes, autoimmune disease or allergy, and any recurrent lip swelling or previous diagnosis of a condition that causes it. Declare everything ever injected into your lip, what it was and when, and do not conceal it, because dissolving the product may be the right treatment instead of surgery. Mention any history of cold sores, since preventive treatment may be advised beforehand. See a dentist and treat gum inflammation or decay first. Stop smoking before and after the procedure, as it delays mucosal healing. Avoid alcohol beforehand and eat a light meal. Prepare soft, cool food and the prescribed mouth rinse, and do not schedule an event or public appearance within three to four weeks.

## Aftercare

Expect marked lip swelling that peaks in the first two days and temporarily exaggerates the shape, with most of it settling over two to four weeks, though mild swelling and firmness can last months. Cool the lip gently on the first day, sleep with your head raised, and use the prescribed rinse after every meal. Eat soft, cool food and avoid spicy or acidic items, hot drinks, straws and smoking. Do not open your mouth widely, stretch the lip or touch the sutures, and postpone long dental appointments. \*\*Seek care immediately for fever, purulent discharge, a foul smell, increasing pain, or wound separation.\*\* \*\*Go to emergency care immediately for bleeding that will not stop or rapid swelling that interferes with swallowing, breathing or speech.\*\* Report numbness of the lip that persists for weeks, obvious asymmetry, or a tight feeling that prevents you closing your mouth or smiling normally.
