## Procedure steps

### 1. Heart team assessment and CT planning

The valve is assessed by echocardiography and a CT scan measures the annulus and maps the arterial route from the groin to the heart. The coronary arteries, kidney function and dental status are checked, then the team meets to choose between catheter treatment and surgery and discusses the decision with you and your family.

### 2. Preparation and anaesthesia

Your medicines are organised beforehand, and an intravenous line and full monitoring of pulse and blood pressure are established. Local anaesthesia with sedation is usual, though general anaesthesia is chosen in some cases. A temporary pacing wire is placed in case the heart rate slows while the valve is deployed.

### 3. Access and crossing the narrowed valve

A sheath is placed in an artery in the upper thigh after numbing the skin; if the leg arteries are unsuitable the team selects an alternative access route. A fine wire is passed through the narrow valve opening into the left ventricle, pressures are measured and the aortic root is imaged.

### 4. Deploying the new valve

The folded valve is guided with high precision under X-ray into the diseased valve position, then expanded either by a balloon or by its own self-expanding frame, depending on type. The team immediately confirms that the valve functions well, that leakage around it is minimal, and that the coronary arteries and electrical conduction are unaffected.

### 5. Completion and rhythm monitoring

The equipment is withdrawn and the arterial puncture is closed with a closure device or a suture. You are monitored on continuous ECG in the cardiac unit for several days, because conduction disturbance can appear late and may call for a permanent pacemaker; an echocardiogram is done and supervised early mobilisation begins.

## About this service

Transcatheter aortic valve replacement (TAVR) treats a narrowed aortic valve by delivering a tissue valve, folded onto a catheter and usually introduced through an artery in the upper thigh, into the diseased valve position. The new valve is expanded there, pushing the old leaflets aside and taking over their function — without opening the chest and without stopping the heart on a bypass machine. In many cases it is performed under local anaesthesia with light sedation while the patient is awake.

The main indication is severe aortic stenosis with symptoms: breathlessness on exertion, dizziness or fainting, chest pressure, or a marked fall in exercise tolerance. The decision is not made by one doctor but by a heart team that includes the interventional cardiologist, cardiac surgeon, anaesthetist and imaging specialist, after echocardiography and a CT scan that measure the valve annulus and map the arteries the device must travel through, together with assessment of the coronary arteries and of the teeth and any other source of infection. Choosing between catheter treatment and surgery depends on age, anatomy, surgical risk and other illnesses — not simply on a wish to avoid surgery.

The limits must be stated plainly: \*\*this procedure treats valve narrowing only.\*\* It does not treat coronary artery disease, which may need separate treatment; it does not repair heart muscle already weakened or damaged; and it does not relieve breathlessness caused by lung disease, anaemia or obesity. A tissue valve has a finite lifespan and may deteriorate after years, requiring a further intervention, which is why age and anatomy weigh heavily in the decision. Not every anatomy is suitable either: certain annulus sizes, diseased peripheral arteries, or bicuspid valves may make surgery or a different access route more appropriate.

Recognised complications include disturbance of the heart's electrical conduction that may require a permanent pacemaker, stroke, injury to the access artery, mild leakage around the valve, and contrast effects on the kidneys. \*\*After the procedure, call an ambulance immediately for fainting or a very slow pulse; for pain or pressure in the centre of the chest spreading to the arm or jaw with sweating, nausea or breathlessness; or for sudden weakness of the face or a limb or slurred speech — do not wait for it to pass and do not drive yourself to hospital.\*\* \*\*Note that symptoms can be atypical in older adults, women and people with diabetes: severe fatigue, nausea or breathlessness with no clear chest pain.\*\* Suitability is determined by clinical assessment and imaging, not by reading.

## Preparation

Tell your doctor your kidney function if you have a recent result, and any previous reaction to X-ray contrast or iodine, since both the CT scan and the procedure rely on contrast. Report every medicine you take, especially anticoagulants, antiplatelet drugs and diabetes medicines, some of which are adjusted or withheld temporarily on your doctor's instruction alone — never stop a medicine yourself. Raise any active infection before the date: gum infection, a dental abscess, or a urinary or skin infection, because implanting a valve while an infection is present is hazardous and is better treated first; a dental review beforehand is usually advised. Fast for the period specified, arrange a companion and for a stay of several days, and bring your medicine list, reports and the names of the doctors following you.

## Aftercare

Protect the groin access site: avoid lifting, deep bending and driving in the first days, and inspect the site daily. Drink enough fluid within the limits your heart condition allows. \*\*Never stop or change the dose of the anticoagulant or antiplatelet medicine prescribed after your valve on your own — not even for a tooth extraction or minor surgery. Only the doctor who performed the procedure may decide, and every dentist or surgeon must know you have a prosthetic valve.\*\* Keep your teeth clean and your dental appointments, and carry your valve card at all times. \*\*Seek emergency care immediately for: fainting, severe dizziness or a noticeably slow pulse; increasing breathlessness or swollen legs; increasing swelling, bleeding or severe pain at the groin access site; a cold, blue or numb leg; sudden weakness of the face or a limb or slurred speech; or fever with chills or night sweats — which may mean valve infection, so do not start an antibiotic yourself before being examined. For chest pain or pressure spreading to the arm or jaw with sweating or nausea, call an ambulance and do not drive yourself.\*\*
