## Procedure steps

### 1. Assessment before starting and choosing the modality

The paediatric kidney team assesses the cause of failure, remaining kidney function, weight and growth, the condition of vessels and abdomen, and the family situation including distance to the centre. Options are explained and the modality chosen is the one that fits the child's life, not the one easiest to arrange.

### 2. Creating the access

Haemodialysis needs a vascular access, either a catheter or an arteriovenous fistula, while peritoneal dialysis needs a catheter placed in the abdomen through a short operation. Healing time is allowed before use whenever possible, and parents are taught how to care for and watch the access.

### 3. The session or the overnight exchange

In haemodialysis the child sits connected to the machine for a few hours per session under nursing supervision, with weight and blood pressure monitored. In peritoneal dialysis sterile fluid is instilled into the abdomen and drained in cycles, usually by a machine running overnight during sleep.

### 4. Family training and home monitoring

The family is trained in sterile technique, connecting the machine, recording weight, fluid in and out and blood pressure, spotting redness, pain or discharge around the catheter, and checking the appearance of the drained fluid. This training is the foundation of safe home dialysis.

### 5. Follow-up, nutrition, growth and transplant preparation

Blood tests and dialysis adequacy are reviewed regularly, nutrition is adjusted with a dietitian, and height, weight, bone health and anaemia are monitored. Transplant assessment is begun at the appropriate time, and psychological and social support for the child and family is delivered as part of the plan.

## About this service

Dialysis in children takes over part of the work of the kidneys when they can no longer do it: clearing waste from the blood, controlling water and salts, and correcting the acid balance. It is used temporarily in acute kidney failure from which the kidneys may recover, or continuously in advanced chronic kidney failure until the child becomes a candidate for a kidney transplant.

There are two forms in paediatric practice. Haemodialysis is usually done in an equipped centre about three times a week through a vascular access, with blood passing through a filter outside the body and returning. Peritoneal dialysis uses the child's own abdominal membrane as a natural filter through a catheter placed in the abdomen, and is most often carried out at home by a machine that runs overnight while the child sleeps, leaving the day free for school and play. Choosing between them is not simply a matter of preference: it depends on the child's age and weight, the cause of kidney failure, the state of the blood vessels and abdomen, the family's ability to run a home programme, and the distance to the centre.

It must be clear that \*\*dialysis does not cure the kidneys or restore their previous function\*\*, and it does not replace the rest of the treatment: a measured plan for fluid, salt, potassium and phosphate, medicines for blood pressure, anaemia and bone health, and close monitoring of growth. Equally, \*\*dialysis is not a permanent alternative to transplantation\*\*. For most children the aim is that it serves as a safe bridge to a kidney transplant, which offers better growth and a life closer to normal. Starting dialysis also does not mean the end of school, movement or light sport; it means organising them around a plan.

The part most often neglected is growth, nutrition and emotional support. Chronic kidney failure slows height gain and reduces appetite, so a nutrition plan is built with a dietitian and growth indicators are checked at every visit. The child and the family need genuine psychological and social support: a mother's anxiety, a child's separation from friends, and the pressure of a treatment timetable are part of the condition, not an optional extra to be addressed if time allows.

Every one of these decisions is made by the paediatric kidney team after full assessment. Which form of dialysis suits a child, and when to begin, are determined by clinical examination and laboratory results, not by reading a page or by another family's experience.

## Preparation

Bring to every session or visit the daily weight log, blood pressure readings, the amount of fluid the child drank and urine output if any, and a complete medication list including vitamins and phosphate binders. Give blood pressure medicines at the times the doctor specified relative to the session, since some are deliberately delayed on dialysis days and only on medical instruction. Bring permitted food and drink plus a toy or book to make the time pass, and dress the child so the access can be reached without embarrassment. Keep the catheter or fistula site clean, and never allow blood pressure measurement or blood sampling from the fistula arm. Tell the team before the session about any fever, diarrhoea, vomiting, sudden weight gain or breathlessness, and about any missed dose. If dialysis is done at home, check your stock of solutions and consumables before it runs low and keep the designated room clean.

## Aftercare

After a session a child may feel tired, dizzy or crampy and needs rest with fluids kept within the allowed limit. Weigh the child after dialysis and record the figure, keep the dressing over the needle site clean and dry as you were taught, and do not remove it before the stated time. Stick to the plan for fluid, salt, potassium and phosphate; exceeding it between sessions is the commonest reason for swelling, breathlessness and high blood pressure. Give medicines on time, encourage school attendance and suitable activity, and talk with the child about how they feel using words that match their age. \*\*Seek urgent care immediately for\*\* \*\*fever or shivering, redness, pain or discharge around the catheter, or cloudy peritoneal fluid\*\*, which signal serious infection, \*\*breathlessness, rapid swelling or inability to lie flat\*\*, \*\*drainage that stops or severe abdominal pain\*\*, bleeding from the needle site that will not stop with pressure, vomiting that prevents drinking or medication, seizures, reduced consciousness or a pounding heartbeat, or a sudden loss of the thrill in the fistula.
