القاعدة الحاكمة لكل هذه الأدوات أن نتيجتها لا تُقرأ وحدها. الاختبار يُظهر أن الجهاز المناعي «يعرف» المادة، أما كون هذه المعرفة مرضاً فيقرّره ظهور أعراض حقيقية عند التعرّض لها. ولهذا فإن نتيجة موجبة عند شخص يأكل الطعام أو يخالط الحيوان بلا أي أعراض ليست حساسية بل تحسّس مخبري، والقرار الصحيح فيها هو عدم تغيير أي شيء.

ومن أخطر الممارسات أن تُطلب قائمة اختبار طويلة لأطعمة كثيرة دون سؤال سريري، ثم يُحذف كل ما ظهر فيها موجباً. هذا الحذف الواسع يُفقد الطفل بروتينات وكالسيوم وحديداً في سنوات النمو، ويُدخل الأسرة في قلق دائم من الطعام، وقد يُضعف تحمّل الجسم لتلك الأطعمة فيزيد خطر تفاعل حقيقي عند العودة إليها لاحقاً. كما أن التحاليل التي تُسوَّق لقياس «عدم تحمّل الطعام» بأجسام مضادة من نوع آخر لا تُشخّص حساسية ولا تُبنى عليها حميات.

وللفحص حدود من الجهة الأخرى أيضاً: النتيجة السلبية لا تنفي كل احتمال، وخصوصاً في حساسية الدواء وفي بعض تفاعلات الطعام، وحجم بقعة الجلد لا يعني شدّة التفاعل المتوقّع، وبعض الأدوية تُضعف الاستجابة فتُظهر نتيجة سالبة كاذبة. الترشيح لأي اختبار وقراءة نتيجته يُحدَّدان بالفحص السريري وتاريخك المرضي، لا بطلب المريض ولا بقائمة جاهزة.

## خطوات الإجراء

### 1. المقابلة السريرية تحدّد ما يُختبر

يسألك الطبيب عن شكل الأعراض وسرعة ظهورها بعد التعرّض ومدّتها، وعن الأطعمة أو الأدوية أو المواد المُشتبهة، وعن الموسم والمكان والمهنة. من هذا الحوار تُبنى قائمة قصيرة ومحدّدة للاختبار، لأن اختبار ما لا تشتبه به سريرياً يخلق نتائج مُحيِّرة لا قرارات.

### 2. التحضير الدوائي وفحص سلامة الجلد

تُراجع أدويتك، وقد يُطلب إيقاف مضادات الهيستامين وبعض أدوية أخرى قبل الاختبار بفترة يحدّدها الطبيب لأنها تُخفي الاستجابة. ويُفحص الجلد نفسه: الإكزيما الواسعة أو الطفح النشط في موضع الاختبار قد تؤخّره وتُوجّه الطبيب إلى تحليل الدم بدلاً منه.

### 3. اختبار وخز الجلد وقراءته

تُوضع نقاط المستخلصات على الساعد أو الظهر بمسافات محدّدة مع نقطة ضابطة موجبة وأخرى سالبة للمقارنة، ثم يُوخز الجلد وخزاً سطحياً غير مؤلم عادةً. تظهر الاستجابة كحكّة وانتفاخ صغير خلال دقائق، وتُقاس وتُقارن بالضابطين، وليست الحكّة وحدها دليلاً.

### 4. تحليل الدم أو اختبار الرقعة عند الحاجة

إن لم يكن اختبار الجلد ممكناً أو كانت النتيجة غير حاسمة، يُطلب تحليل دم لأجسام مضادة نوعية. وإن كانت الشكوى طفحاً متأخراً بعد ملامسة معدن أو عطر أو مادة في العمل، يُلصق اختبار الرقعة على الظهر ويُقرأ على مراحل خلال أيام، ويجب حينها تجنّب البلل والتعرّق الشديد.

### 5. قراءة النتيجة مع الأعراض وبناء الخطة

يجمع الطبيب النتيجة مع قصتك ليقرّر: هل هذه حساسية تستوجب تجنّباً وخطة علاج، أم تحسّس مخبري يُترك بلا تغيير، أم حالة تحتاج تحدّياً مُراقَباً للحكم. وتُكتب لك خطة واضحة تشمل ما يُتجنّب فعلاً وما يبقى في غذائك، وعلامات الخطر ومتى تطلب الإسعاف.

## Procedure steps

### 1. The clinical interview decides what gets tested

Your doctor asks what the symptoms looked like, how quickly they came on after exposure and how long they lasted, which foods, medicines or materials are suspected, and the season, setting and occupation involved. From that conversation a short, targeted list is built, because testing what is not clinically suspected produces confusing results rather than decisions.

### 2. Medication review and checking the skin is testable

Your medicines are reviewed, and antihistamines and some other drugs may need to be stopped for a period your doctor specifies because they mask the response. The skin itself is checked too: widespread eczema or an active rash at the test site may delay testing and steer your doctor towards a blood test instead.

### 3. Skin prick testing and its reading

Drops of extract are placed on the forearm or back at set distances, alongside a positive and a negative control for comparison, then the skin is pricked superficially, which is usually not painful. A response appears as itching and a small raised wheal within minutes; it is measured and compared with both controls, and itching by itself is not evidence.

### 4. Blood testing or patch testing when needed

If skin testing is not possible or the result is inconclusive, a blood test for specific antibodies is requested. If the complaint is a delayed rash after contact with a metal, a fragrance or a workplace material, patch tests are applied to the back and read in stages over several days, during which you must keep the area dry and avoid heavy sweating.

### 5. Reading results against symptoms and building the plan

Your doctor combines the result with your story to decide whether this is an allergy that warrants avoidance and treatment, sensitisation to be left untouched, or a case needing a supervised challenge to settle. You leave with a written plan covering what genuinely must be avoided, what stays in your diet, and the warning signs that mean calling for emergency help.

## التحضير قبل الإجراء

اسأل العيادة قبل الموعد عن الأدوية التي يجب إيقافها ومتى، ولا توقف أي دواء من نفسك، وخصوصاً أدوية الربو والقلب. اكتب قائمة بكل ما تشتبه به وكيف كان التفاعل: ماذا أكلت أو لمست، بعد كم وقت ظهرت الأعراض، شكلها ومدّتها، وما فعلته حينها، وهل تكرّر. صوّر الطفح إن ظهر مرة أخرى قبل الموعد، فالصور أقوى من الوصف. الْبس ملابس تسهّل كشف الساعدين والظهر. لا تضع مرطّبات أو كريمات على منطقة الاختبار في يوم الفحص. أخبر الطبيب إن كنتِ حاملاً، وإن كان في تاريخك تفاعل شديد سابق أو إغماء بعد تعرّض. وإن كان الفحص لطفل، أحضر معه ما يُشغله بهدوء ولا تُخوّفه من «الإبرة»، فالوخز سطحي وسريع.

## العناية بعد الإجراء

الحكّة والانتفاخ في مواضع الوخز يهدآن عادةً خلال ساعات، ولا تخدش المنطقة، والتزم بما يوصي به الطبيب لتهدئتها. احتفظ بورقة النتائج وخطة الطبيب معاً، فالورقة بلا خطة تُقرأ خطأً. \*\*لا تحذف أي طعام من غذائك أو غذاء طفلك اعتماداً على النتائج وحدها ودون قرار طبيبك: الحذف على قائمة اختبار يضرّ التغذية والنمو وقد يزيد خطر تفاعل حقيقي عند العودة للطعام لاحقاً.\*\* وإن كان اختبار الرقعة، فحافظ على جفاف الظهر وتجنّب الرياضة والاستحمام حتى موعد القراءة النهائي. \*\*راجع الإسعاف فوراً عند ضيق النفس أو صفير الصدر أو تورّم الشفة أو اللسان أو الحلق أو بحّة الصوت أو طفح منتشر أو دوخة وإغماء بعد الاختبار أو بعد أي تعرّض: هذا تفاعل تحسّسي شديد ولا يحتمل الانتظار.\*\*

## About this service

Allergy testing is not a single test but a group of tools your doctor selects from according to your complaint: skin prick testing, in which tiny drops of suspected extracts are placed on the forearm and read within minutes; a blood test for antibodies specific to one substance; patch testing, applied to the back for several days to investigate contact allergy; and a supervised challenge test inside a properly equipped centre when a definitive answer is needed about a food or a medicine.

The rule governing all of these is that a result is never read on its own. A test shows that the immune system recognises a substance; whether that recognition amounts to disease is decided by real symptoms appearing on exposure. A positive result in someone who eats the food or handles the animal with no symptoms at all is therefore not an allergy but laboratory sensitisation, and the correct response to it is to change nothing.

One of the most damaging practices is ordering a long panel covering many foods with no clinical questioning, then removing everything that came back positive. Broad exclusion of that kind strips a child of protein, calcium and iron during growth years, leaves the family in permanent anxiety around food, and can reduce the body's tolerance so that the risk of a genuine reaction rises when those foods are reintroduced later. Tests marketed as measuring food intolerance through a different class of antibody do not diagnose allergy and must not be used to build diets.

Testing also has limits in the other direction. A negative result does not rule out every possibility, particularly in drug allergy and some food reactions. The size of a skin reaction does not predict how severe a future reaction will be, and certain medicines blunt the response and produce false negatives. Which test is appropriate, and how its result should be read, is decided by clinical examination and your history rather than by patient request or a ready-made panel.

## Preparation

Ask the clinic before your appointment which medicines must be stopped and when, and never stop any medicine on your own, especially asthma or heart medication. Write a list of everything you suspect and how the reaction went: what you ate or touched, how long afterwards symptoms appeared, what they looked like, how long they lasted, what you did, and whether it happened again. Photograph any rash that recurs before the visit, since pictures beat description. Wear clothes that give easy access to the forearms and back. Do not apply moisturisers or creams to the test area on the day. Tell the doctor if you are pregnant, and if you have ever had a severe reaction or fainted after an exposure. If the test is for a child, bring something quiet to occupy them and avoid frightening them about needles: the prick is shallow and quick.

## Aftercare

Itching and swelling at the prick sites usually settle within hours; do not scratch, and follow your doctor's advice for relieving them. Keep the results sheet and the doctor's plan together, because results read without the plan are read wrongly. \*\*Do not remove any food from your own or your child's diet on the basis of results alone and without your doctor's decision: exclusion driven by a test panel harms nutrition and growth and can increase the risk of a genuine reaction when the food is reintroduced later.\*\* If you had patch tests, keep your back dry and avoid sport and showering until the final reading appointment. \*\*Get emergency care immediately for shortness of breath or wheeze, swelling of the lip, tongue or throat, a hoarse voice, a widespread rash, or dizziness and fainting after the test or after any exposure: this is a severe allergic reaction and it cannot wait.\*\*
