## Procedure steps

### 1. Pre-procedure assessment and informed consent

The indication, neurological examination, blood-thinning medicines, clotting disorders and allergies are reviewed, coagulation and platelet tests are requested when needed, and brain imaging is obtained before the puncture in specific situations such as focal deficit, altered consciousness or suspected mass lesion. The steps and possible complications are explained and consent is taken.

### 2. Positioning, landmarks and sterile preparation

The patient lies on one side with the back curled and knees drawn towards the chest, or sits leaning forward. A safe lumbar level below the end of the spinal cord is identified using anatomical landmarks, with ultrasound assistance when landmarks are difficult. The skin is disinfected and covered with a sterile drape.

### 3. Local anaesthesia, needle insertion and pressure measurement

The skin and deeper tissues are anaesthetised, then a fine needle is advanced between the vertebrae into the space where the fluid flows. Patients usually feel pressure or pushing rather than sharp pain, and should report immediately any transient tingling down a leg. Opening pressure is measured when indicated, before the sample is collected.

### 4. Collecting and sending the samples

Fluid is collected into several numbered tubes sent for different analyses: cell count and differential, protein and glucose, stains and bacterial culture, molecular tests for viruses, and immune banding or specific markers according to the clinical suspicion. The fluid's appearance is documented, then the needle is withdrawn and the site dressed.

### 5. Short observation, instructions and result follow-up

The patient rests briefly with fluids, the features of post-puncture headache and how it is managed are explained, and written discharge instructions are given. Urgent results follow within hours, specialised assays take days to two weeks, and treatment decisions are then based on the full set of results alongside the clinical picture.

## About this service

A lumbar puncture, also called a spinal fluid test, is a diagnostic procedure in which a small sample of cerebrospinal fluid is withdrawn through a fine needle inserted between the vertebrae of the lower back under local anaesthetic. This fluid surrounds the brain and spinal cord, and analysing it provides information that neither blood tests nor imaging can give: evidence of infection or inflammation, abnormal cells, immune markers, or old blood, along with a direct measurement of the fluid pressure itself.

The test is requested in specific situations: suspected meningitis or encephalitis; assessment of multiple sclerosis and other neuroimmune diseases; suspected subarachnoid haemorrhage when imaging is negative but the history is convincing; idiopathic intracranial hypertension, where measuring the opening pressure is part of the diagnosis; certain neuropathies and radiculopathies; and suspected spread of cancer cells to the meninges. The same route is sometimes used to deliver medication into the spinal fluid or for spinal anaesthesia.

The most common and most harmful myth is the belief that the needle is pushed into the spinal cord and can cause paralysis. In reality the puncture is performed at a level below where the spinal cord ends, so there is no spinal cord at the needle site to injure. The procedure uses clear anatomical landmarks, with ultrasound or fluoroscopic guidance when needed. The realistic complication is post-puncture headache, a pain that worsens on standing and eases on lying flat; it usually improves with fluids, rest and simple analgesia, and can be treated with a straightforward procedure if it persists.

The limits should be stated. Lumbar puncture is a diagnostic tool rather than a treatment, except in specific situations such as removing fluid to relieve pressure or delivering intrathecal medication. A normal result does not exclude every disease, and some specialised assays take days to two weeks. There are also situations where the procedure is contraindicated or postponed: suspected mass lesion or raised intracranial pressure with neurological deficit or altered consciousness before imaging, infection of the skin at the puncture site, significant coagulopathy or low platelets, and treatment with anticoagulants, whose interruption must be arranged by the treating doctor. Whether and when the test is appropriate is decided by clinical assessment alongside imaging and blood results.

## Preparation

Tell your doctor in advance about all your medicines, especially blood thinners and antiplatelet drugs, and do not stop them yourself; follow your doctor's instructions on whether and how long to interrupt them. Also report any clotting disorder or low platelets, infection or inflammation of the skin over the lower back, previous spinal surgery, allergy to local anaesthetic, dressings or antiseptics, and pregnancy. Drink fluids well before the appointment unless you have been asked to fast for another reason, wear comfortable clothing that is easy to remove, and bring previous reports and scans. Empty your bladder before the procedure, arrange for someone to take you home and do not plan to drive afterwards, and keep the rest of the day free for rest. If you are frightened of needles or have fainted during procedures before, tell the team so they can take precautions.

## Aftercare

Rest lying down for a period after the procedure as the team advises, drink plenty of fluids for the first day or two, and avoid heavy exertion, lifting and vigorous sport on the first day. Keep the puncture site clean and dry, and avoid baths and swimming pools until you are told it is safe. If a headache appears that worsens on standing and eases on lying flat, lie down, increase fluids and take simple analgesia with your doctor's approval, and report it if it has not improved within two to three days, since a straightforward procedure usually resolves it. Restart any blood thinner only at the time your doctor specified. \*\*Seek care urgently for fever or chills, neck stiffness with severe headache, a headache that does not ease when lying flat with fluids and analgesia, clear fluid or blood leaking from the puncture site, increasing redness, swelling or pain at the site, weakness or numbness in the legs, difficulty or inability to pass urine, or loss of bowel control.\*\*
