الفحوص المعتمدة تشمل تصوير الثدي الشعاعي (الماموغرام) للنساء في الفئات المحدّدة، ومسحة عنق الرحم مع فحص فيروس الورم الحلمي البشري، وفحص القولون والمستقيم بفحص الدم الخفي في البراز أو بتنظير القولون، والتصوير المقطعي منخفض الجرعة للرئة لمن لديه تاريخ تدخين كثيف ضمن شروط محدّدة. وتُضاف مسارات خاصة لأصحاب الخطورة المرتفعة: تاريخ عائلي قوي، أو متلازمة وراثية معروفة، أو مرض التهابي مزمن في الأمعاء، أو التهاب كبد مزمن، أو علاج إشعاعي سابق — وهؤلاء قد يبدأون أبكر أو بفواصل أقصر أو بوسائل مختلفة، وقد يُحوَّلون لاستشارة وراثية.

وحدود الفحص يجب أن تكون مفهومة قبل إجرائه: \*\*الفحص لا يشخّص السرطان؛ فالنتيجة غير الطبيعية تحتاج تأكيداً بتصوير موجّه أو خزعة، وكثير من النتائج غير الطبيعية تتبيّن سليمة\*\*. والنتيجة الطبيعية لا تعني أماناً دائماً ولا تُلغي ضرورة الاهتمام بأي عَرَض جديد بين موعدين. وللفحص أضرار حقيقية يجب موازنتها: نتائج موجبة كاذبة تجرّ قلقاً وإجراءات إضافية، ونتائج سالبة كاذبة تعطي طمأنينة زائفة، وفرط تشخيص لتغيّرات لم تكن ستؤذي، ومضاعفات نادرة للإجراءات، وتعرّض إشعاعي محدود. ولهذا يكون القرار مشتركاً بينك وبين طبيبك.

\*\*وتصحيح خرافتين يؤخّران التشخيص: الأولى أنّ «الخزعة تنشر السرطان» — وهذا غير صحيح، والخزعة هي الطريق لتشخيص دقيق وعلاج صحيح، وتأخيرها هو الضرر الحقيقي. والثانية أنّ «تحليل دلالات الأورام يكشف السرطان مبكّراً لدى الأصحّاء» — فهذه الدلالات ترتفع في حالات سليمة كثيرة وتبقى طبيعية في سرطانات موجودة، واستخدامها الأساسي متابعة حالات مشخّصة لا الكشف العام.\*\* وكذلك فإنّ حزم «الفحص الشامل للجسم» بلا مؤشّر ليست برنامج كشف مثبتاً، وقد تجرّ سلسلة إجراءات بلا فائدة. أمّا ما يناسبك بالضبط — وأي فحص، ومن أي عمر، وكل كم مدّة — فيُحدَّد بالفحص السريري وتقييم عوامل خطرك وتاريخك العائلي، لا بالقراءة.

## خطوات الإجراء

### 1. تقييم عوامل الخطر والتاريخ العائلي

يُسأل عن عمرك وجنسك والتدخين والوزن والكحول والنشاط، وعن أمراضك المزمنة والتهاب الكبد، وعن سرطانات العائلة ونوعها وأعمار الإصابة، وعن أي علاج إشعاعي سابق، لتُحدَّد فئة خطورتك قبل اختيار أي فحص.

### 2. اختيار الفحوص المناسبة وتوقيتها

يُبنى جدول شخصي: أي فحص يبدأ ومن أي عمر وكلّ كم مدّة، وأيّها لا ينطبق عليك؛ ويُقدَّم موعد البداية أو يُقصَّر الفاصل لأصحاب الخطورة المرتفعة، وقد يُحوَّل من له قصّة عائلية قوية لاستشارة وراثية.

### 3. الشرح المتوازن والموافقة المستنيرة

تُشرح فائدة كل فحص وحدوده معاً: احتمال نتيجة غير طبيعية تحتاج استكمالاً، واحتمال نتيجة سالبة كاذبة، وفرط التشخيص، وما ستفعله النتيجة فعلاً في قرارك؛ فالقرار مشترك ولا يُفرَض فحص بلا فهم.

### 4. إجراء الفحص وتفسير النتيجة

يُجرى الفحص بحسب نوعه — تصوير أو مسحة أو عيّنة براز أو تنظير — بتحضير خاص لكل منها، ثمّ تُفسَّر النتيجة في سياق خطرك وفحوصك السابقة، وتُصنَّف إلى طبيعية، أو تحتاج إعادة في مدّة أقصر، أو تحتاج استكمالاً فورياً.

### 5. مسار المتابعة أو الاستقصاء المؤكِّد

عند نتيجة طبيعية يُثبَّت موعد التكرار كتابةً؛ وعند نتيجة غير طبيعية يُرتَّب استقصاء مؤكّد بتصوير موجّه أو خزعة بلا تأخير، ويُحوَّل المريض إلى فريق الأورام إن ثبت التشخيص لوضع خطة كاملة.

## Procedure steps

### 1. Assessing risk factors and family history

You are asked about age and sex, smoking, weight, alcohol and activity, about chronic conditions and viral hepatitis, about cancers in the family with their type and the ages at diagnosis, and about previous radiotherapy, so your risk category is defined before any test is chosen.

### 2. Choosing the right tests and their timing

A personal schedule is built: which test starts, from what age and at what interval, and which does not apply to you. Start age is brought forward or intervals shortened for those at high risk, and anyone with a strong family history may be referred for genetic counselling.

### 3. Balanced explanation and informed consent

The benefit of each test and its limits are explained together: the chance of an abnormal result needing further work-up, the chance of a false negative, overdiagnosis, and what the result would actually change in your decisions; the choice is shared and no test is imposed without understanding.

### 4. Performing the test and interpreting the result

The test is performed according to its type — imaging, a smear, a stool sample or endoscopy — each with its own preparation, then the result is interpreted in the context of your risk and previous tests and classified as normal, needing a shorter-interval repeat, or needing immediate work-up.

### 5. Follow-up pathway or confirmatory work-up

With a normal result the repeat date is documented in writing; with an abnormal result confirmatory work-up by targeted imaging or biopsy is arranged without delay, and the patient is referred to an oncology team if the diagnosis is confirmed so a full plan can be made.

## التحضير قبل الإجراء

اكتب تاريخك العائلي بدقّة قبل الزيارة: من أُصيب بأي سرطان، وفي أي عمر، ومن أي جهة من العائلة، فهذه أهمّ معلومة تحدّد جدول فحوصك. أحضر نتائج فحوصك السابقة وتواريخها (ماموغرام، مسحة عنق رحم، تنظير قولون، تصوير صدر) وأي تقرير خزعة قديم، ولا تعتمد على الذاكرة. اذكر أمراضك المزمنة وأدويتك ومكمّلاتك، وخصوصاً مضادات التخثّر ومضادات الصفيحات وحبوب الحديد، وأي تحسّس. للنساء: اذكري تاريخ آخر دورة، وحملك أو إرضاعك، وأي كتلة أو إفراز من الثدي أو نزف غير معتاد، ولا تُجري الماموغرام مع مزيل عرق أو بودرة أو عطر على الصدر والإبط. لفحص القولون: اسأل مسبقاً عن تعليمات الطعام والتحضير وعن إيقاف أو تعديل أي مضاد تخثّر — ولا توقفه من نفسك — وعن الصيام والمرافق إن كان التنظيف بتهدئة. وإن كان لديك أي عَرَض الآن فاذكره صريحاً، لأنّ وجود عَرَض ينقل حالتك من مسار الكشف إلى مسار التشخيص العاجل.

## العناية بعد الإجراء

اطلب نتيجتك مكتوبة واحتفظ بها مع تاريخ الفحص وموعد التكرار المقترح، وسجّل الموعد في هاتفك، فمعظم فائدة الكشف المبكّر تأتي من الانتظام لا من فحص واحد. إن جاءت النتيجة طبيعية فاستمرّ في تقليل عوامل الخطر: الامتناع عن التدخين والأرجيلة، ووزن صحّي، ونشاط بدني، وتقليل الكحول، والتطعيم ضدّ فيروس الورم الحلمي والكبد الوبائي ب بحسب توصية طبيبك. وإن جاءت غير طبيعية فلا تفترض الأسوأ ولا تتجاهلها: أكمل الاستقصاء المطلوب في موعده، فكثير من النتائج تتبيّن سليمة. بعد التنظير توقّع انتفاخاً بسيطاً، ولا تقُد سيارة في يوم التهدئة. \*\*راجع فوراً — ولا تنتظر موعد الفحص القادم — عند كتلة جديدة في الثدي أو الرقبة أو الإبط أو الخصية، أو نزف غير مفسّر: من الشرج، أو بين الدورات أو بعد سنّ الإياس، أو دم في البول، أو نفث دم، أو تغيّر مستمرّ أكثر من أسبوعين في نمط التبرّز، أو صعوبة أو ألم في البلع، أو بحّة أو كحّة مستمرّة، أو فقدان وزن غير مقصود، أو تعرّق ليلي وحرارة مستمرّة، أو شامة تكبر أو تتغيّر أو تتقرّح، أو ألم عظمي ليلي مستمرّ.\*\* \*\*وبعد تنظير القولون راجع الطوارئ عند ألم بطن شديد أو انتفاخ متزايد أو حرارة أو نزف غزير.\*\*

## About this service

Cancer screening means performing tests in people who have \*\*no\*\* symptoms, in order to detect cancer or the changes that precede it at a stage when treatment is more effective and less extensive. It is fundamentally different from diagnostic testing, which is done because a symptom or an abnormal result already exists. There is no single cancer test: there are specific tests proven to help for particular cancers, each with its own age group, risk level and interval, which your clinician sets according to the guideline followed and your individual situation.

Established programmes include mammography for women in the defined age groups, cervical screening with a smear and human papillomavirus testing, bowel screening with faecal occult blood testing or colonoscopy, and low-dose CT of the chest for people with a heavy smoking history who meet defined criteria. Separate pathways are added for those at high risk: a strong family history, a known hereditary syndrome, chronic inflammatory bowel disease, chronic viral hepatitis, or previous radiotherapy. These individuals may start earlier, be screened at shorter intervals or with different methods, and may be referred for genetic counselling.

The limits must be understood before testing. \*\*Screening does not diagnose cancer: an abnormal result needs confirmation with targeted imaging or a biopsy, and many abnormal results turn out to be harmless.\*\* A normal result does not mean permanent safety and never cancels the need to take a new symptom seriously between appointments. Screening also carries real harms that must be weighed: false positives that bring anxiety and further procedures, false negatives that give false reassurance, overdiagnosis of changes that would never have caused harm, rare procedural complications, and limited radiation exposure. This is why the decision is shared between you and your clinician.

\*\*Two myths delay diagnosis and need correcting. The first is that a biopsy spreads cancer — it does not, and a biopsy is the route to an accurate diagnosis and the right treatment, while delaying it is the real harm. The second is that tumour marker blood tests detect cancer early in healthy people — these markers rise in many benign conditions and stay normal in cancers that are present, and their main use is monitoring already diagnosed disease rather than general screening.\*\* Likewise, commercial whole-body check-up packages without an indication are not a proven screening programme and can trigger a chain of procedures with no benefit. Exactly which tests suit you, from what age and at what interval, is decided by clinical assessment of your risk factors and family history, not by reading.

## Preparation

Write out your family history precisely before the visit: who had which cancer, at what age, and on which side of the family, because this is the single most important piece of information shaping your schedule. Bring your previous test results with their dates (mammogram, cervical smear, colonoscopy, chest imaging) and any old biopsy report rather than relying on memory. List your chronic conditions, medicines and supplements, particularly anticoagulants, antiplatelet drugs and iron tablets, and any allergies. For women: note the date of your last period, whether you are pregnant or breastfeeding, and any breast lump, nipple discharge or unusual bleeding, and do not attend for a mammogram wearing deodorant, powder or perfume on the chest and underarms. For bowel screening: ask in advance about dietary and bowel preparation instructions and about stopping or adjusting any anticoagulant — never stop it yourself — and about fasting and bringing an escort if sedation is used. If you have any symptom now, say so plainly, because a symptom moves you from the screening pathway to an urgent diagnostic one.

## Aftercare

Ask for your result in writing and keep it with the test date and the suggested repeat date, and put that date in your phone, because most of the benefit of screening comes from regularity rather than from a single test. If the result is normal, keep reducing your risk factors: no cigarettes or waterpipe, a healthy weight, physical activity, less alcohol, and vaccination against human papillomavirus and hepatitis B as your clinician advises. If the result is abnormal, neither assume the worst nor ignore it: complete the requested work-up on time, since many abnormal findings prove harmless. After endoscopy expect mild bloating, and do not drive on a day when sedation was used. \*\*Seek assessment immediately — without waiting for the next scheduled test — for a new lump in the breast, neck, armpit or testicle; unexplained bleeding from the rectum, between periods or after the menopause, blood in the urine, or coughing up blood; a change in bowel habit lasting more than two weeks; difficulty or pain on swallowing; a persistent hoarse voice or cough; unintended weight loss; night sweats with persistent fever; a mole that grows, changes or ulcerates; or persistent night-time bone pain.\*\* \*\*After colonoscopy, go to emergency for severe abdominal pain, increasing distension, fever or heavy bleeding.\*\*
