وللفحص حدود يجب فهمها: \*\*نتيجة طبيعية واحدة لا تنفي المرض إلى الأبد\*\*، وارتفاع عابر في الكرياتينين قد يكون سببه جفاف أو مجهود عنيف أو دواء ولا يعني بالضرورة مرضاً مزمناً؛ لذلك تُكرَّر الفحوص ويُفسَّر التغيّر عبر الزمن سريرياً. كما أن تحليل البول يتأثّر بالدورة الشهرية أو بمجهود رياضي شديد قبله. \*\*ولا يوجد فحص أو دواء أو خلطة عشبية «تُنظّف الكلى»، وكثير من هذه الخلطات يضرّها فعلاً\*\*.

القيمة الحقيقية للفحص أنه يكشف الخلل في مرحلة يمكن فيها إبطاء التدهور فعلاً، بضبط ضغط الدم والسكر وتعديل الأدوية الضارة، قبل أن يفقد المريض جزءاً كبيراً من وظيفة كليتيه — وما يتلف من نسيج الكلى لا يُستعاد. وتفسير النتائج وتحديد الحاجة إلى فحوص إضافية يتم بالفحص السريري لا بمقارنة الأرقام على الإنترنت.

## خطوات الإجراء

### 1. الاستشارة وتقييم عوامل الخطر

يسأل الطبيب عن السكري وضغط الدم والتاريخ العائلي والأدوية والمسكّنات والمكمّلات والأعشاب، وعن الحصى والالتهابات البولية، وعن أعراض مثل الرغوة في البول والتورّم وتغيّر كمية البول والتبوّل الليلي. هذه المعلومات تحدّد أي الفحوص تُطلب فعلاً بدل طلب قائمة عامة.

### 2. الفحص السريري وقياس ضغط الدم

يُقاس ضغط الدم بطريقة صحيحة بعد راحة قصيرة، ويُفحص التورّم في القدمين وحول العينين، ويُجسّ البطن والخصر، ويُستمع للقلب والرئتين. كثير من أمراض الكلى تظهر أول ما تظهر كارتفاع ضغط غير مفسّر، لذلك هذه الخطوة ليست شكلية.

### 3. تحليل البول وقياس الزلال

تُؤخذ عينة بول من منتصف التبول بعد تنظيف المنطقة، وتُفحص للبروتين والدم والسكر والالتهاب، ويُقاس الزلال لأنه يكشف التسرّب المبكر قبل أن يتغيّر أي رقم آخر. وقد يُطلب تكرار العينة في يوم آخر لتأكيد النتيجة، أو جمع بول 24 ساعة في حالات معيّنة.

### 4. فحوص الدم لوظائف الكلى والأملاح

تُسحب عيّنة دم لقياس الكرياتينين واليوريا وتقدير معدل الترشيح، مع الصوديوم والبوتاسيوم والكالسيوم والفوسفور وصورة الدم بحسب الحالة، وفحوص السكر عند مرضى السكري. تُقارَن النتائج مع فحوصك السابقة لأن اتجاه التغيّر أهمّ من رقم منفرد.

### 5. تفسير النتائج وخطة المتابعة

يشرح الطبيب ما تعنيه النتائج مجتمعة، ويقرّر إن كان يكفي تكرار الفحص بعد فترة، أم يلزم تصوير بالموجات الصوتية أو فحوص مناعية أو تحويل إلى طبيب كلى. كما تُراجَع أدويتك لإيقاف ما يضرّ الكلى، وتُحدَّد أهداف ضغط الدم والسكر وموعد الفحص القادم.

## Procedure steps

### 1. Consultation and risk-factor assessment

Your physician asks about diabetes, blood pressure, family history, medications, painkillers, supplements and herbal products, about stones and urinary infections, and about symptoms such as foamy urine, swelling, change in urine volume and night-time urination. This information decides which tests are actually needed rather than ordering a generic panel.

### 2. Physical examination and blood pressure measurement

Blood pressure is measured correctly after a short rest, the feet and the area around the eyes are examined for swelling, the abdomen and flanks are palpated, and the heart and lungs are auscultated. Many kidney diseases first present as unexplained high blood pressure, so this step is far from a formality.

### 3. Urine testing and albumin measurement

A midstream urine sample is collected after cleaning the area and tested for protein, blood, glucose and signs of infection, and albumin is measured because it reveals early leakage before any other number changes. The sample may be repeated on another day to confirm the result, or a 24-hour urine collection requested in certain cases.

### 4. Blood tests for kidney function and electrolytes

A blood sample is taken to measure creatinine and urea and to estimate the filtration rate, along with sodium, potassium, calcium, phosphate and a blood count as appropriate, plus glucose tests in people with diabetes. Results are compared with your earlier tests, because the direction of change matters more than a single value.

### 5. Interpreting results and planning follow-up

Your physician explains what the results mean together and decides whether repeating the test after an interval is enough, or whether ultrasound, immunological testing or referral to a nephrologist is needed. Your medications are also reviewed so that anything harmful to the kidneys is stopped, and blood pressure and glucose targets and the date of the next check are set.

## العناية بعد الإجراء

لا تُفسّر النتائج بنفسك ولا تقارنها بنتائج غيرك، فالأرقام تُقرأ مع عمرك وجسمك وأدويتك. إن كانت النتائج طبيعية وأنت من أصحاب عوامل الخطر، اسأل عن موعد الفحص القادم والتزم به، فالفحص الواحد لا يحمي إلى الأبد. وإن ظهر زلال أو ضعف في الوظيفة، اضبط ضغط الدم والسكر بجدّية، وقلّل الملح، وأوقف بإشراف الطبيب كل ما يضرّ الكلى، \*\*وتجنّب المسكّنات غير الستيرويدية المزمنة والأعشاب وخلطات «تفتيت الحصى»\*\*. أعد الفحوص في الموعد المحدّد ولو شعرت بأنك بحال جيدة. وراجع الطبيب فوراً عند: \*\*قلّة البول الحادة\*\*، \*\*تورّم مفاجئ في الوجه والقدمين\*\*، \*\*دم في البول\*\*، \*\*حرارة مع ألم في الخصر\*\*، أو ضيق نفس مع تورّم الساقين.

## About this service

A kidney check is a set of simple, painless tests that measure how well the kidneys are working, detect leakage of protein or blood into the urine, assess salts and blood pressure, and show the shape of the kidneys and urinary tract with imaging when needed. It is the most valuable tool available for early detection, because chronic kidney disease is silent until advanced stages and does not wait for the patient to complain.

The core check usually consists of a general urine test with measurement of urinary albumin, a blood creatinine measurement used to estimate the filtration rate, and, depending on the situation, urea, electrolytes, potassium, calcium and phosphate, together with blood pressure measurement and examination for swelling. Ultrasound of the kidneys and urinary tract may be added to look for stones, obstruction, cysts or a reduced kidney size.

Those who most need a periodic check include everyone with diabetes, everyone with high blood pressure, anyone with a family history of kidney disease or polycystic kidney disease, people who take painkillers frequently or long-term, those with recurrent stones or urinary infections, women who had high blood pressure or protein in the urine during pregnancy, and anyone who notices foam in the urine, swollen feet or a change in urine volume.

The test has limits that should be understood: a single normal result does not rule out disease forever, and a transient rise in creatinine may be caused by dehydration, vigorous exercise or a medicine rather than chronic disease. That is why tests are repeated and the trend over time is interpreted clinically. Urine results are also affected by menstruation or by hard exercise beforehand. No test, medicine or herbal mixture cleanses the kidneys, and many such mixtures actually harm them.

The real value of the check is that it identifies a problem at a stage when decline can genuinely be slowed, through blood pressure and glucose control and by stopping harmful medication, before a large share of kidney function has been lost, since kidney tissue that is damaged does not regenerate. Interpreting the results and deciding on further tests is done by clinical assessment, not by comparing numbers found online.

## Preparation

Ask in advance whether fasting is required, since some accompanying tests need it while kidney tests alone usually do not. Drink your usual amount of water, neither loading up nor abstaining, because both distort the result. Avoid vigorous exercise the day before, as it can transiently raise urinary protein and creatinine, and avoid heavy meat intake or protein supplements before the test. Tell the laboratory if you are menstruating, because blood will appear in the urine sample. Write out a list of your medicines, painkillers, supplements and herbal products and bring it with you, and mention any recent contrast imaging. Collect the urine sample midstream after cleaning the area, ideally the first morning sample, and deliver it to the laboratory promptly. Bring your older reports as well, because comparison matters more than a single reading.

## Aftercare

Do not interpret the results yourself and do not compare them with someone else's, because the numbers are read alongside your age, body and medications. If your results are normal but you carry risk factors, ask when the next check is due and keep to it, since one test does not protect you indefinitely. If albumin or reduced function is found, take blood pressure and glucose control seriously, cut down on salt, and stop anything harmful to the kidneys under medical supervision, while avoiding long-term non-steroidal painkillers, herbal products and herbal stone-dissolving mixtures. Repeat the tests when scheduled even if you feel well. Seek care promptly if you develop: \*\*a sharp drop in urine output\*\*, \*\*sudden swelling of the face and feet\*\*, \*\*blood in the urine\*\*, \*\*fever with flank pain\*\*, or breathlessness with leg swelling.
