## Procedure steps

### 1. Preparation and hydration before the appointment

The request, the clinical question, and your medicines are reviewed, and you are usually asked to drink a measured amount of water beforehand, because dehydration distorts the drainage curve. If the protocol includes a diuretic or a drug acting on blood pressure, you may be asked to hold certain medicines temporarily or to fast for a set period — on the doctor's instruction, never on your own.

### 2. Intravenous injection and imaging position

A thin intravenous line is placed in the hand or arm, you empty your bladder before lying down, and you then lie on the couch in a comfortable position with the camera behind your back or over the abdomen according to protocol. The tracer is injected and recording starts at the same instant, so there is no room to move or adjust position once the study has begun.

### 3. Sequential dynamic imaging

The camera records a continuous series of images over roughly twenty to forty minutes, so the team sees blood reaching the kidneys and then the tracer being filtered, transiting, and draining away. Staying still matters, because movement blurs the phases of the curve. Apart from the needle prick the study is painless, and you can breathe and talk normally throughout.

### 4. Diuretic when needed and delayed images

If obstruction remains in question, a diuretic is given intravenously on the doctor's order and imaging continues to see whether drainage improves under a higher flow. You may feel your bladder filling and an urge to pass urine, which is expected and short-lived. You may be asked to empty your bladder and have a brief further image taken, or delayed images in selected cases.

### 5. Calculating relative function and reading the report

The nuclear medicine physician calculates each kidney's share of function, plots the drainage curve, and describes the time to peak and the response to the diuretic. Those figures are then read alongside your clinical examination, kidney blood tests, and ultrasound or CT before any surgical or medical decision. The numbers are never interpreted on their own or by the patient.

## About this service

A MAG3 renal scan is a dynamic, functional study of the kidneys. A tracer that the kidneys clear is injected into a vein, and a gamma camera records its arrival in each kidney, its passage through the kidney tissue, and its drainage down the ureter into the bladder. What the study produces is a time-activity curve and a set of numbers describing function and drainage, rather than a still anatomical picture.

The scan is requested to answer a defined question, not as a routine check: is urine drainage from the kidney genuinely obstructed, or is the renal pelvis simply dilated without obstruction? What share of total function does each kidney contribute before a surgical decision? How is a transplanted kidney working, and is there a hold-up at its outlet? It is particularly useful in children with a dilated renal pelvis and in adults with recurrent flank pain that anatomical imaging does not explain. A diuretic, or a drug that acts on the kidney's blood pressure pathway, may be added — on the doctor's order and according to protocol — to sharpen the answer.

The dose used is low, diagnostic, and justified by the question being asked; it is cleared quickly in the urine. The tracer is not iodinated X-ray contrast and does not carry contrast-related risk to the kidneys, which is one reason this study is sometimes chosen when kidney function is reduced. Even so, \*\*tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant before the injection\*\* — the scan may be cancelled, postponed, or replaced with an alternative that uses no radioactive material. Mention all your medicines and any previous allergic reaction as well.

The limits of the study should be clear from the start: it assesses function and drainage, not anatomy. It does not show stones or the detail of a mass the way ultrasound and CT do, it does not measure creatinine, it cannot say whether a renal mass is benign or malignant, and it does not diagnose a urinary tract infection. The curves and figures must be read together with your clinical examination, laboratory results, and other imaging — never alone and never self-interpreted — and whether you need the scan at all is decided by your doctor's examination.

## Preparation

Drink water as the centre instructs before your appointment: dehydration is one of the commonest reasons this study gives a misleading result. Tell your doctor about all your medicines, especially diuretics and blood pressure drugs, because some protocols require holding them temporarily or fasting for a set period — and any such hold is made on the doctor's instruction, not on your own decision. Mention recent contrast imaging, any previous allergic reaction, and past urinary surgery or catheterisation. Bring your earlier reports, imaging, and kidney blood tests. Empty your bladder before lying down and wear comfortable, metal-free clothing. \*\*Tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant\*\* before the injection, as the scan may be cancelled or replaced. If the study is for a child, ask in advance whether you can stay with them and how to help them keep still.

## Aftercare

Drink more fluid than usual for the rest of the day and pass urine whenever you need to, which speeds the tracer's clearance, and wash your hands after using the toilet. If you were given a diuretic, expect frequent urination for some hours and possibly mild light-headedness on standing: get up slowly, drink well, and do not drive while you feel dizzy. Return to your usual food and medicines, but do not restart a drug that was held before the scan without your doctor's agreement. If you are breastfeeding, follow the centre's instructions about feeding and the interval it specifies. \*\*Seek care immediately if you pass no urine, develop severe flank pain with fever or vomiting, faint, or develop a rash, itching, or breathlessness after the injection.\*\* The result is read together with your doctor's examination, blood tests, and imaging, so do not act on a curve or number you have read alone.
