## Procedure steps

### 1. Pre-test counselling

What the test does and does not examine is explained, along with the meaning of screening versus diagnosis and the possibility of an inconclusive result, so the decision rests on understanding rather than a mistaken expectation.

### 2. Confirming gestational age

Gestational age and the number of fetuses are confirmed by ultrasound, since testing before the appropriate week increases the chance of an inconclusive result.

### 3. Maternal blood draw

A routine blood draw from the mother’s arm with no intervention in the uterus and no risk to the pregnancy; the sample is sent to a specialised laboratory.

### 4. Interpreting the result with a specialist

The result is interpreted as a probability in the context of maternal age, ultrasound findings and family history, not read as a standalone number.

### 5. Diagnostic testing after a high-risk result

Amniocentesis or chorionic villus sampling is offered for confirmation before any decision, with the risk of each explained precisely — \*\*no decision rests on screening alone\*\*.

## About this service

Non-invasive prenatal testing (NIPT) is performed on a blood sample from the pregnant mother, examining fragments of fetal DNA that have crossed into her blood to estimate the likelihood of an abnormality in the number of certain chromosomes — most notably trisomy 21 (Down syndrome), 18 and 13, and it may include sex chromosomes.

Its advantage is that it is \*\*completely safe for the pregnancy\*\*, because it is only a maternal blood draw — no needle enters the uterus — and its accuracy in estimating risk is far higher than traditional screening tests. It is usually performed from around the tenth week.

But here is \*\*the most important thing every pregnant woman must understand, and it is often explained imprecisely\*\*: this is a \*\*screening\*\* test, not a diagnostic one. Its result is a probability, not a verdict:
• A “low risk” result \*\*does not exclude\*\* every condition and does not mean a completely healthy fetus — it does not test for all genetic diseases or structural anomalies.
• A “high risk” result \*\*is not a confirmed diagnosis\*\* — it requires confirmation with a diagnostic test (amniocentesis or chorionic villus sampling) before \*\*any\*\* decision.

This distinction is not academic: life-changing decisions have been taken on unconfirmed screening results. False positives occur for known reasons including a chromosomal difference confined to the placenta, a twin pregnancy where one twin stopped developing, or conditions in the mother herself.

The result may also come back \*\*inconclusive\*\* if the fetal DNA fraction is low — commoner with higher weight or very early testing — and is then repeated. The test should therefore be accompanied by \*\*genetic counselling\*\* explaining its meaning before the draw and after the result, and it is optional, decided by the mother after understanding it.

## Preparation

Fasting is usually not required, but ask the laboratory. Bring the ultrasound report confirming gestational age and the number of fetuses. \*\*Tell your doctor if the pregnancy is twin, if it followed IVF with a donor egg, or if you have had a blood transfusion, bone marrow transplant or cancer — all of these affect interpretation and may make the result unreliable.\*\* Mention a high body weight, as it lowers the fetal DNA fraction. \*\*Ask explicitly before paying: is this a screening or diagnostic test, exactly which conditions does it cover, what is the next step if the result is high risk, and is genetic counselling included.\*\*

## Aftercare

There are no restrictions after the blood draw — continue your day normally. \*\*Do not take any decision based on this test alone\*\*: a “high risk” result needs confirmation with a diagnostic test, and a “low risk” result does not replace routine pregnancy care and the detailed anomaly ultrasound, because this test does not see structural anomalies. Request a session to explain the result rather than relying on reading the report or on interpretation from a non-specialist — anxiety from misunderstanding is commoner than you might think. Continue folic acid, follow-up and other prescribed tests on schedule. \*\*Contact your obstetrician for any bleeding, pain or change in fetal movement — these are entirely separate from the test result and need assessment in their own right.\*\*
