يبدأ التقييم بما لا يحتاج جهازاً: قياس ضغط الدم بطريقة صحيحة وأكثر من مرة — فارتفاع الضغط هو أهمّ عامل خطر قابل للتعديل، وهو صامت لا يسبّب صداعاً في الغالب — ثم فحص النبض بحثاً عن عدم انتظام قد يعني رجفاناً أذينياً يُضاعف الخطر ويُعالج بأدوية مانعة للتخثّر. تُضاف فحوص السكر والدهون ووظائف الكلى، وحساب مؤشّر الكتلة والمحيط، وسؤال دقيق عن التدخين والنارجيلة والنشاط البدني والشخير وانقطاع النفس أثناء النوم، وعن حبوب منع الحمل مع التدخين والشقيقة المصحوبة بأعراض بصرية، وعن التاريخ العائلي.

بحسب النتائج تُطلب فحوص موجّهة لا شاملة: تخطيط قلب أو مراقبة نظم لعدة أيام عند الشكّ في الرجفان، صدى للقلب، دوبلر لشرايين الرقبة عند وجود دواعٍ سريرية، وتصوير للدماغ والأوعية في حالات محدّدة. ومن كانت له نوبة عابرة — ضعف أو تلعثم أو فقدان رؤية استمرّ دقائق ثم زال — فهذه ليست حالة «انتهت» بل إنذار صريح: خطر السكتة الكاملة يكون أعلى ما يمكن في الأيام التالية، والتقييم يجب أن يكون عاجلاً خلال ساعات إلى أيام، لا موعداً بعد شهر.

حدود الفحص بصراحة: الفحص بذاته لا يمنع السكتة؛ ما يمنعها هو ضبط الضغط والسكر والدهون، والإقلاع عن التدخين، والالتزام بالدواء الموصوف. ولا يوجد فحص واحد «يستبعد» السكتة مستقبلاً، ونتيجة طبيعية اليوم لا تعني أماناً دائماً. كما لا يُنصح بإجراء دوبلر رقبة أو رنين دماغ لكل شخص بلا دواعٍ، لأنه يكشف أحياناً أشياء عارضة تجرّ إلى تدخّلات لا تنفع وقد تضرّ. ومضادات الصفيحات ومانعات التخثّر ليست وقاية للجميع: بدؤها من تلقاء النفس يحمل خطر نزف قد يزيد على المنفعة، والقرار طبي بحسب حالتك.

وأهم ما يخرج به المريض من هذه الزيارة ليس ورقة نتائج، بل معرفة علامات السكتة والتصرّف الصحيح: ميلان في الوجه، ضعف في ذراع، اضطراب في الكلام أو الفهم، فقدان رؤية أو توازن مفاجئ. اتّصل بالإسعاف فوراً ولا تنتظر أن تزول ولا تذهب بسيارتك الخاصة، لأن العلاجات المذيبة للجلطة والقسطرة مرتبطة بنافذة زمنية ضيّقة. والترشيح لأي فحص أو دواء وقائي يُحدَّد بالفحص السريري لا بطلب المريض.

## خطوات الإجراء

### 1. حصر عوامل الخطر والتاريخ

سؤال منهجي عن ضغط الدم والسكري والدهون والتدخين والنارجيلة والوزن والنشاط، وعن الشخير وانقطاع النفس النومي، والشقيقة المصحوبة بأعراض بصرية، وحبوب منع الحمل، وأمراض المناعة، والتاريخ العائلي للسكتة والجلطات المبكرة، وعن أي أعراض عابرة سابقة حتى لو استمرّت دقائق فقط.

### 2. الفحص السريري وقياس الضغط والنبض بدقّة

يُقاس ضغط الدم في الذراعين بعد راحة وبأكثر من قراءة، ويُفحص النبض لكشف عدم الانتظام، ويُسمع القلب والشرايين، ويُجرى فحص عصبي أساسي وفحص لقاع العين. تُقارن القياسات مع سجلّ منزلي إن توفّر، لأن قراءة واحدة في العيادة لا تكفي لتشخيص ارتفاع الضغط أو نفيه.

### 3. الفحوص المخبرية والقلبية المستهدفة

تُطلب فحوص السكر التراكمي والدهون ووظائف الكلى وصورة الدم، وتخطيط للقلب. عند الشكّ في رجفان أذيني تُستخدم مراقبة نظم القلب لعدة أيام، وقد يُطلب صدى للقلب للبحث عن مصدر تخثّر أو خلل صمّامي. في الحالات المبكّرة العمر أو غير المفسّرة تُضاف فحوص تخثّر ومناعة.

### 4. تصوير الأوعية عند الدواعي فقط

يُطلب دوبلر شرايين الرقبة أو تصوير للأوعية عند وجود نوبة عابرة، أو أعراض عصبية غير مفسّرة، أو نفخة شريانية، أو خطر مرتفع مجتمعاً. لا يُنصح بهذه الصور كفحص روتيني لمن لا أعراض له ولا عوامل خطر، لأن الكشف العارض قد يؤدّي إلى قلق وإجراءات لا تحسّن النتيجة.

### 5. خطة خفض الخطر المكتوبة والمتابعة

تُكتب أهداف رقمية للضغط والسكر والدهون، وخطة إقلاع عن التدخين، ونشاط بدني متدرّج، وتقليل الملح، وعلاج انقطاع النفس النومي، وأدوية وقائية إن كانت لها دواعٍ واضحة. يُدرَّب المريض على قياس الضغط منزلياً وعلى علامات السكتة، وتُحدَّد إعادة تقييم سنوية أو أبكر عند تغيّر الحالة.

## Procedure steps

### 1. Mapping risk factors and history

Systematic questioning about blood pressure, diabetes, lipids, smoking and waterpipe use, weight and activity, snoring and sleep apnoea, migraine with visual aura, contraceptive pills, autoimmune disease, and family history of stroke or early clots, plus any previous transient symptoms even if they lasted only minutes.

### 2. Clinical examination with accurate pressure and pulse

Blood pressure is measured in both arms after rest and on more than one reading, the pulse is checked for irregularity, the heart and arteries are auscultated, and a basic neurological and fundus examination is performed. Readings are compared with a home log where available, because a single clinic reading neither confirms nor excludes hypertension.

### 3. Targeted laboratory and cardiac tests

HbA1c, lipids, kidney function and a blood count are requested along with an ECG. Where atrial fibrillation is suspected, several days of rhythm monitoring are used, and echocardiography may be added to look for a source of clot or valve disease. In young or unexplained cases, clotting and autoimmune tests are included.

### 4. Vessel imaging only when indicated

Carotid Doppler or vessel imaging is requested when there has been a transient attack, unexplained neurological symptoms, an arterial bruit, or a combination of high-risk factors. Such imaging is not advised as a routine screen in people without symptoms or risk factors, because incidental findings can generate anxiety and procedures that do not improve outcomes.

### 5. A written risk-reduction plan and follow-up

Numerical targets are written for blood pressure, glucose and lipids, along with a smoking cessation plan, graded physical activity, salt reduction, treatment of sleep apnoea, and preventive medication where clearly indicated. The patient is trained in home blood pressure measurement and in recognising stroke signs, with reassessment yearly or sooner if circumstances change.

## التحضير قبل الإجراء

قِس ضغط دمك في المنزل مرتين يومياً لأسبوع قبل الموعد وسجّل القراءات بتاريخها، فهذا أنفع من أي فحص آخر تحمله. أحضر قائمة كاملة بأدويتك ومكمّلاتك بما فيها المسكّنات وحبوب منع الحمل وأدوية الأنف، وأحضر آخر فحوص دم وتخطيط قلب وأي تصوير سابق للرقبة أو الدماغ على قرص. اكتب وصفاً دقيقاً لأي عرض عابر مررت به: ماذا شعرت، وأيّ جهة، وكم دقيقة استمرّ، ومتى بالضبط. أحضر تاريخ العائلة: من أُصيب بسكتة أو جلطة أو رجفان وفي أي عمر. التزم بالصيام إن طُلبت فحوص دهون، ولا تشرب قهوة أو تدخّن قبل قياس الضغط بنصف ساعة، ولا توقف أي دواء ضغط قبل الزيارة.

## About this service

Stroke prevention screening is a structured assessment of risk factors and of the brain and heart circulation in someone who has not yet had a stroke, or who had transient symptoms that resolved on their own. The purpose is not a reassuring brain scan; it is to estimate the probability of an event in the coming years and then reduce it through specific, written steps.

The assessment begins with what needs no machine: measuring blood pressure correctly and more than once, because hypertension is the most important modifiable risk factor and is silent, rarely causing headache. The pulse is then checked for irregularity that may indicate atrial fibrillation, which multiplies risk and is treated with anticoagulation. Glucose, lipids and kidney function are tested, body mass index and waist measured, and careful questions asked about smoking and waterpipe use, physical activity, snoring and sleep apnoea, the combination of contraceptive pills with smoking, migraine with visual aura, and family history.

Depending on the findings, targeted rather than blanket tests are ordered: an ECG or several days of rhythm monitoring when fibrillation is suspected, echocardiography, carotid Doppler where clinically indicated, and brain or vessel imaging in selected cases. Anyone who has had a transient attack — weakness, slurred speech or visual loss lasting minutes and then clearing — has not had a closed episode but an explicit warning: the risk of a completed stroke is at its highest in the following days, and assessment must be urgent, within hours to days, not an appointment next month.

The honest limits: screening by itself prevents nothing. What prevents stroke is controlling blood pressure, glucose and lipids, stopping smoking, and taking prescribed medication consistently. No single test can rule out a future stroke, and a normal result today does not mean permanent safety. Carotid Doppler or brain MRI for everyone without an indication is not advised, because it sometimes reveals incidental findings that lead to interventions which do not help and may harm. Antiplatelet and anticoagulant medicines are not prevention for everyone: starting one yourself carries a bleeding risk that can outweigh the benefit, and the decision is a medical one based on your own profile.

The most valuable thing to take away from this visit is not a results sheet but knowing the signs of stroke and the correct response: facial droop, arm weakness, disturbed speech or comprehension, and sudden loss of vision or balance. Call emergency services immediately, do not wait for symptoms to pass, and do not drive yourself, because clot-dissolving treatment and catheter removal of clot depend on a narrow time window. Eligibility for any test or preventive medicine is decided by clinical assessment.

## Preparation

Measure your blood pressure at home twice daily for a week before the appointment and record the readings with their dates; this is more useful than any test you can carry with you. Bring a complete list of your medicines and supplements, including painkillers, contraceptive pills and nasal preparations, along with recent blood tests, ECGs and any previous neck or brain imaging on a disc. Write a precise description of any transient symptom you experienced: what you felt, which side, how many minutes it lasted and exactly when. Bring your family history: who had a stroke, clot or atrial fibrillation and at what age. Fast if lipid tests are requested, avoid coffee and smoking for half an hour before pressure measurement, and do not stop any blood pressure medicine before the visit.

## Aftercare

Work through the written plan item by item: measure and log blood pressure at home, take medicines on time without interruption even once readings look normal — their silent return is the danger — reduce salt, walk regularly, stop smoking and waterpipe use, and treat sleep apnoea if confirmed. Do not start an antiplatelet or any blood thinner on your own, and do not stop one before surgery or a dental extraction without your doctor's advice. Make sure you and your household know the signs of stroke. \*\*Call emergency services immediately if\*\* there is facial droop, sudden weakness or numbness of an arm or leg, slurred speech or inability to understand, loss of vision in one eye or to one side, sudden vertigo and loss of balance, or an unusually severe headache — do not wait for symptoms to pass, do not sleep on them and do not drive yourself. \*\*Seek prompt review for\*\* an irregular pulse, repeatedly high pressure readings, or unexplained bleeding while on a blood thinner.
