Middle ear tests
- **Procedure duration:** Middle ear testing usually takes about 5 to 15 minutes for both ears, with the measurement itself lasting only seconds per ear. The visit may extend to around half an hour if otoscopy and simple wax removal are added, or if a child needs time to settle. In cases of fluid, the measurement is typically repeated after a few weeks to track spontaneous improvement.

## Procedure steps

### 1. History and symptoms

The audiologist asks about ear pain and discharge, fullness or popping, recurrent infections, colds, allergy and snoring, and difficulty equalising pressure when flying or diving. In children, questions cover attention, speech delay and turning the television up, because middle ear fluid is often painless.

### 2. Visual examination of canal and eardrum

The canal is inspected with an otoscope to confirm it is free of impacted wax, foreign bodies or inflammation, and to view the eardrum: its colour, transparency, mobility, and whether there is a perforation or fluid behind it. This step must precede the measurements, since wax or a perforation change both the result and the method.

### 3. Tympanometry: compliance and pressure

A soft probe is placed at the entrance of the canal and produces a small pressure change with a low tone, while the equipment plots a curve showing how well the drum moves and what the middle ear pressure is. You feel mild pressure or a muffled sound for a few seconds, with no pain; you are simply asked not to swallow or talk during the measurement.

### 4. Acoustic reflex measurement

Relatively louder sounds are presented to measure the automatic contraction of the small muscle inside the middle ear. The presence or absence of this reflex helps localise the problem to the drum and ossicles or beyond them. The patient is told the sound will be loud for only brief moments, and intensity is reduced in people sensitive to sound.

### 5. Interpreting results with the hearing test

The curves are read together with the audiogram and speech testing to classify the loss as conductive, sensorineural or mixed. The result is explained to the patient or family, and a plan is set: monitoring with repeat measurement in a few weeks, medical treatment, or referral to the ear specialist to consider intervention if fluid persists.

## About this service

Middle ear tests are a group of measurements that assess the part of the ear between the eardrum and the cochlea: the movement of the drum itself, the air pressure behind it, the integrity of the three small bones, and the function of the Eustachian tube that equalises pressure. They usually include tympanometry, which measures compliance and pressure, and acoustic reflex testing, preceded by visual examination of the canal and drum. The tests are quick, painless and need little cooperation, so they suit both children and adults.

Their value lies in explaining hearing loss that can be treated medically without any device. Silent middle ear fluid in children is the classic example: the child does not complain and has no pain, and the effect shows instead as reduced attention, a louder television and delayed speech. These tests detect it accurately. They also reveal a perforated eardrum, negative pressure behind it, and stiffness or discontinuity of the ossicular chain.

The limits matter. \*\*These tests do not measure hearing level itself\*\* and do not replace an audiogram. They do not assess the hearing nerve or the cochlea, and they say nothing about your ability to understand speech. A normal result does not exclude sensorineural loss, and an abnormal one may be temporary after a cold. They are therefore always read together with pure tone audiometry, speech testing and clinical examination, never in isolation.

Two widespread myths need correcting here, because both cause real harm. First, \*\*cleaning the ear with cotton buds is a harmful practice\*\*: it pushes wax deeper and compacts it against the drum, scratches the skin of the canal and opens the way to infection, and can even perforate the eardrum. Wax migrates out by itself and protects the canal; it \*\*does not need routine removal without a reason\*\*, and removal belongs in the clinic and only when it causes symptoms. Second, \*\*warm olive oil does not cure an ear infection\*\*: it does not kill the organism and does not open the Eustachian tube, and it becomes genuinely dangerous if the eardrum is perforated, because any liquid then enters the middle ear.

What follows the test is decided by clinical examination and by the doctor and audiologist together: monitoring, medical treatment, or an intervention such as ventilation tubes when fluid persists and is affecting hearing and language development in a child.

## Preparation

The test needs no fasting and no special preparation. The most important thing is to put nothing into your ear before the visit: no cotton buds, keys or tools, and no olive oil, warm oils or over-the-counter drops, since any of these can push wax against the eardrum, leave liquid that blocks the measurement, or cause harm if the drum is perforated. If you have ear discharge, severe pain or fever, tell the clinic before your appointment. Bring reports of any previous ear surgery or ventilation tubes, a list of your medicines, and the results of any earlier hearing test. Tell the audiologist if you currently have a cold or active allergy, as repeating the measurement after it settles may be preferable. For children: bring the child rested and fed, explain that the test is painless, bring something to occupy him, and mention his history of ear infections and snoring.

## Aftercare

There are no restrictions afterwards and you can resume everything immediately, though you may feel mild fullness for a few minutes. Keep a copy of the result, and if it was abnormal after a cold, attend the repeat measurement in a few weeks, because many cases resolve on their own and need no intervention. If a child has persistent fluid, watch his attention, speech and classroom performance, and report any decline at the review visit. Do not put anything into the ear to clean it afterwards, do not use oils or drops without a prescription, and keep water out of the ear if you have a perforation or ventilation tubes. Seek care immediately if you develop: \*\*discharge, blood or a foul smell from the ear\*\*, \*\*severe pain with fever, or swelling and redness behind the ear\*\*, \*\*sudden hearing loss in one ear\*\*, \*\*severe dizziness with vomiting or imbalance\*\*, or weakness or numbness of the facial muscles.
