## Procedure steps

### 1. Reviewing the indication and previous imaging

The team reviews your symptoms, neurological examination and previous CT or MR angiography, and defines the exact question the catheter study must answer. The purpose, alternatives and possible complications are explained and informed consent is signed before anything else.

### 2. Preparation, local anaesthetic and arterial access

After fasting and hydration, the puncture area in the groin or wrist is cleaned, local anaesthetic is given and a thin sheath is placed in the artery. Monitoring of blood pressure, pulse and oxygen is attached, and general anaesthesia is used for children or patients unable to cooperate when clinically decided.

### 3. Navigating the catheter to the neck arteries

The catheter is passed through the aorta into the carotid and vertebral arteries under X-ray guidance. You are usually asked to keep completely still without moving your head, and to tell the team at once about any sudden headache, numbness, or change in vision or speech.

### 4. Contrast injection and multi-projection imaging

Measured amounts of contrast are injected with image runs for each vessel in different projections, with three-dimensional rotational imaging when needed. A brief sensation of warmth in the head or a flash in the vision may be felt; this is expected and explained beforehand.

### 5. Catheter removal, haemostasis and neurological observation

The equipment is withdrawn, the puncture site is closed by compression or a closure device, and you are observed for several hours with repeated neurological checks and pulse assessment in the limb. The report is discussed with your treating doctor, since treatment decisions rest on the whole clinical and imaging picture.

## About this service

Diagnostic cerebral angiography, often called a brain catheter study, is an imaging test that examines the arteries of the neck and brain from the inside. Under local anaesthetic a fine catheter is introduced through an artery in the upper thigh or the wrist and guided into the neck arteries; contrast is then injected while sequences of X-ray images are taken in several projections. The result is an extremely detailed map of blood flow to the brain that no other test reproduces with the same resolution.

The study is usually requested when non-invasive tests such as CT or MR angiography are not sufficient: to characterise a cerebral aneurysm or an arteriovenous malformation, to investigate subarachnoid haemorrhage, to assess arterial narrowing, to look for suspected vasculitis, or to plan treatment before surgery or an endovascular intervention. A diagnostic session may be followed immediately by treatment through the same catheter when this has been planned and consented to in advance.

Its limits are clear. This catheter study is diagnostic in nature and does not treat anything by itself, and it images blood vessels rather than brain tissue, so it does not replace MRI or CT when the question concerns a stroke, a tumour or inflammation within the brain substance. Because it is an invasive test that carries a small but real neurological risk, it is performed only when a clinical question cannot be answered by simpler tests, and the decision is made by clinical examination and the neurovascular team's assessment rather than on request.

A widespread myth is that a brain catheter study is open surgery in which the skull is opened. In fact it is done through a skin puncture with no incision in the head; patients are usually awake and are asked to stay still and hold their breath briefly during imaging runs. Ask your doctor why this particular test has been chosen for you, what the alternatives are and what complications are possible before you consent.

## Preparation

Tell your doctor about every medicine and supplement you take, especially blood thinners, antiplatelet drugs and certain diabetes medicines that may be paused around contrast administration, and never stop or adjust anything yourself. Report any previous reaction to contrast or medicines, any kidney disease, and any possibility of pregnancy. Blood tests for kidney function, clotting and a blood count are usually arranged. Fasting is normally six to eight hours, with sips of water and approved tablets allowed, and good hydration before the session is advised as instructed. Bring discs or reports of your previous imaging. Remove jewellery and removable dentures, tell the team if you find it hard to lie still, and arrange for someone to take you home.

## Aftercare

Stay in bed in the position the team specifies for a few hours; with a groin puncture keep the leg straight and avoid bending the hip, and with a wrist puncture keep the compression band on and carry nothing with that hand. Drink enough fluids to help clear the contrast if your doctor allows. Keep the dressing clean and dry for the first day and avoid driving, heavy lifting and sport for about 24 to 48 hours. A small bruise and mild soreness at the puncture site are expected. \*\*Go to the emergency department immediately if you develop weakness or numbness on one side of the body, slurred or difficult speech, facial drooping, double vision or loss of vision, a sudden severe headache, a seizure or confusion, since these are stroke warning signs that must be assessed within minutes rather than hours, and also if there is continued bleeding or a rapidly expanding swelling at the puncture site, a cold, pale or numb limb, or a high fever.\*\*
