## Procedure steps

### 1. A confidential follow-up visit and full assessment

The visit begins with a review of general health: weight, temperature and blood pressure, any new symptoms, sleep, mood and energy, smoking and nutrition. \*\*You are reassured that the file is confidential and that what is said in the clinic stays there\*\*, and you are asked about any obstacle to taking your treatment, without judgement or reproach.

### 2. Routine monitoring tests

Viral load and CD4 immune-cell counts are requested to assess the response to treatment, along with a blood count, kidney and liver function, lipids and glucose to monitor medication effects and general health, plus hepatitis and sexually transmitted infection screening as needed. \*\*Results are read together with you; they are not self-interpreted or compared with another patient's numbers.\*\*

### 3. Reviewing adherence and removing obstacles

Daily medication is reviewed along with any missed dose and its reason, openly and without blame: forgetfulness, travel, shift work, side effects, difficulty collecting the prescription, or a wish for privacy at home. Practical solutions are built: reminders, tying the dose to a daily routine, arranging longer dispensing, and reviewing interactions with your other medicines and supplements.

### 4. Prevention and preventive screening

Tuberculosis risk is assessed and preventive treatment offered where appropriate, recommended vaccinations are updated, and screening for sexually transmitted infections and for cancers is arranged according to age and sex. A partner's protection is discussed confidentially with preventive options, along with safe pregnancy and breastfeeding, mental-health support and stopping smoking.

### 5. A plan until the next visit

The plan is documented in writing: test dates, the next appointment, a supply of medication sufficient until then, and how to make contact in between. The warning signs that require attention before the appointment are explained, along with arrangements for travel with medication and a route to psychological or social support if you would like it.

## About this service

An HIV review is a structured, regular follow-up visit for a person living with the virus. Its purpose is to assess the body's response to antiretroviral treatment, monitor general health and organ function, prevent associated infections, and discuss whatever concerns the patient in an atmosphere of respect. \*\*Your health information is confidential and is not shared with anyone — family, employer or anyone else — without your explicit permission\*\*, and this is the foundation of the clinic, with no exceptions.

The medical fact that changed everything is that this is now a chronic condition managed with regular care much like diabetes or high blood pressure, and life expectancy for someone who keeps up with treatment is close to that of anyone else. Daily adherence to medication lowers the amount of virus in the blood to an undetectable level, and \*\*a person who achieves and maintains an undetectable viral load does not transmit the virus to a sexual partner\*\* — an established finding that deserves to be stated plainly, because it releases many people from years of anxiety. Preventive options for a partner are also available and are discussed in the clinic in complete confidence.

Testing is a step of strength, not fear: an early diagnosis means starting treatment early with far better outcomes, and there is nothing that justifies delaying it. \*\*There is no place for stigma or judgement in this care\*\* — the medical team is here for treatment and follow-up only, and any question about marriage, pregnancy, breastfeeding, work or travel is entirely legitimate and has a practical answer. Family planning and safe pregnancy, mental-health support, stopping smoking, recommended vaccinations, and age-appropriate preventive screening are all part of the same conversation.

This care has explicit limits worth knowing. \*\*Treatment suppresses the virus and protects the immune system, but it does not remove the virus from the body, and there is no complete cure today.\*\* Stopping the medicines or missing doses therefore allows the virus to become active again and breeds resistance that narrows future treatment options. \*\*An undetectable viral load does not protect against other sexually transmitted infections and does not prevent pregnancy\*\*, so condoms and periodic screening remain important. Your test numbers should not be interpreted on your own or compared with anyone else's; they are read with your doctor in the context of your whole situation. Clinical assessment remains what determines any change to your treatment plan.

## Preparation

Bring your current medication packs or photographs of them, along with a list of any other medicine, supplement or herbal product you take, because drug interactions matter a great deal in this treatment. Before the visit, note honestly any missed doses, when they happened and why — this information is used to help you, not to hold you to account. Write down any new symptom and how long it has lasted: fever, cough, diarrhoea, weight loss, rash, numbness, headache, or changes in mood or sleep. Bring previous test results if they were done at another laboratory. Write your questions in advance, including those about marriage, pregnancy, breastfeeding, travel and work — all of them are legitimate. Ask ahead whether fasting is needed for glucose and lipid tests, and tell the clinic if you prefer a particular way of being contacted that protects your privacy.

## Aftercare

Take your medication daily at a fixed time and never stop it even if you feel completely well — regularity is what protects your immune system and prevents resistance. Tie the dose to a daily habit and use an alarm, and do not wait for the pack to run out before collecting the next one. Complete your tests on schedule, keep the results, and read them with your doctor rather than on your own. Remember that an undetectable viral load does not protect against other sexually transmitted infections and does not prevent pregnancy. Look after sleep, nutrition and physical activity, and ask for psychological support without hesitation — it is part of the treatment. Contact your doctor promptly if any of the following appears: \*\*persistent fever, night sweats or unexplained weight loss\*\* · \*\*a cough lasting more than two weeks or blood in the sputum — to screen for tuberculosis\*\* · \*\*prolonged diarrhoea, difficulty swallowing or mouth ulcers\*\* · \*\*a rash after starting a new medicine\*\* · \*\*any change in vision\*\*. Go to the emergency department immediately for: \*\*a severe headache with a stiff neck, light sensitivity or confusion\*\* · \*\*a seizure or weakness in a limb\*\* · \*\*rapid breathing with fever\*\* · \*\*a rash that does not fade under pressure\*\*.
