\*\*تقنية القياس الخاطئة تُنتج قرارات خاطئة\*\*، وهذا أشهر خطأ يرتكبه المريض في بيته. أهم الأخطاء: كفّ أصغر أو أكبر من محيط الذراع، لفّ الكفّ فوق كمّ سميك، الجلوس بلا إسناد للظهر، تعليق الذراع بلا سند أو رفعها أعلى من مستوى القلب أو خفضها كثيراً، تقاطع الساقين، الكلام أو النظر في الهاتف أثناء القياس، القياس بعد قهوة أو تدخين أو مجهود أو مع مثانة ممتلئة، القياس بلا راحة خمس دقائق، والاعتماد على قراءة واحدة بلا تكرار. القياس الصحيح: راحة خمس دقائق، جلوس مسند، القدمان على الأرض، الذراع مسندة على طاولة بمستوى القلب، الكفّ على الجلد مباشرة، صمت تام، وقراءتان بفاصل دقيقة تُسجَّل كلتاهما.

المراقبة تكشف أنماطاً لا تظهر في العيادة: ارتفاع القراءة في العيادة فقط بسبب التوتر وهو ما يُعرف بضغط المعطف الأبيض، وضغط مقنّع يكون طبيعياً في العيادة ومرتفعاً في البيت وهو الأخطر لأنه يُهمَل، وقراءات مرتفعة صباحاً فقط، وقراءات منخفضة مع دوخة عند الوقوف تستدعي قياس الضغط واقفاً بعد الجلوس. وتغطّي المراقبة أيضاً الجانب المنخفض: هبوط الضغط الذي يسبّب دوخة أو إغماء أو تعباً أو غثياناً، خصوصاً عند كبار السن ومن يتناولون أدوية عدّة، وقياسه واقفاً هو ما يُظهره غالباً.

وحدود المراقبة يجب أن تكون واضحة: الجهاز يعطي أرقاماً ولا يشخّص وحده ولا يضع خطة ولا يعدّل جرعة؛ وأجهزة المعصم والإصبع أقل دقّة من جهاز أعلى الذراع؛ والقياس المتكرّر بقلق عشرات المرات في اليوم يرفع القراءة نفسها ويزيد التوتّر بلا فائدة. الأصل أن تُسجَّل قراءات منظّمة في أوقات محدّدة وتُعرض على الطبيب، فهو من يفسّرها في سياق عمرك وأمراضك وأدويتك. وأي قرار ببدء علاج أو تعديله أو إيقافه يُحدَّد بالفحص السريري والتقييم المباشر لا بقراءة صفحة.

## خطوات الإجراء

### 1. اختيار الجهاز والكفّ المناسب

يُفضّل جهاز رقمي أوتوماتيكي يُلبس على أعلى الذراع من نوع موثوق، مع كفّ يطابق محيط ذراعك؛ فالكفّ الصغير يعطي قراءة أعلى من الحقيقة والكبير أدنى منها. أجهزة المعصم والإصبع أقل دقّة وتصلح للاستثناءات فقط. أحضر جهازك إلى العيادة ليُقارَن بقياس الطبيب مرة واحدة على الأقل، وأعِد فحصه دورياً.

### 2. التحضير قبل كل قياس

امتنع عن القهوة والشاي والتدخين والمجهود قبل القياس بنصف ساعة، وأفرغ مثانتك، واجلس هادئاً خمس دقائق كاملة. لا تقِس في لحظة غضب أو ألم أو بعد صعود الدرج، لأن القراءة حينها لا تمثّل ضغطك المعتاد. اخلع الكمّ السميك ولا تلفّ الكفّ فوق الملابس.

### 3. وضعية الجسم الصحيحة

اجلس على كرسي بظهر مسند، والقدمان مستويتان على الأرض بلا تقاطع، والذراع مسندة على طاولة بحيث يكون منتصف الكفّ بمستوى القلب. ضع الكفّ على الجلد مباشرة فوق المرفق بإصبعين تقريباً وبإحكام يسمح بإدخال إصبع تحته. لا تتكلّم ولا تنظر في الهاتف أثناء القياس، لأن الكلام وحده يرفع القراءة.

### 4. جدول قياس منظّم وتسجيل أمين

خُذ قراءتين بفاصل دقيقة صباحاً قبل الدواء والطعام، وقراءتين مساءً، لمدة من ثلاثة إلى سبعة أيام قبل المراجعة. سجّل كل القراءات كما ظهرت بلا انتقاء ولا حذف للقراءة المرتفعة، مع التاريخ والوقت. إن شعرت بدوخة عند الوقوف، قِس جالساً ثم واقفاً بعد دقيقة وثلاث دقائق وسجّل الفرق.

### 5. المراقبة المحمولة وتفسير النتائج

عند الشكّ في ضغط المعطف الأبيض أو الضغط المقنّع أو تفاوت القراءات، يُركّب جهاز محمول يقيس تلقائياً 24 ساعة بما فيها النوم، فتُعطي صورة أدقّ من أي قياس منفرد. يفسّر الطبيب السجل مع عمرك وكليتيك وسكرك وأدويتك، ويقرّر البدء أو التعديل أو الانتظار. الجهاز أداة قياس لا أداة قرار.

## Procedure steps

### 1. Choosing the right device and cuff

A validated automatic upper-arm digital monitor is preferred, with a cuff matching your arm circumference: too small a cuff reads higher than the truth and too large reads lower. Wrist and finger devices are less accurate and suit exceptions only. Bring your device to the clinic to be compared against the doctor's measurement at least once, and have it rechecked periodically.

### 2. Preparation before each reading

Avoid coffee, tea, smoking and exertion for half an hour before measuring, empty your bladder, and sit quietly for a full five minutes. Do not measure in a moment of anger or pain or after climbing stairs, because the reading then does not represent your usual pressure. Remove a thick sleeve and never wrap the cuff over clothing.

### 3. Correct body position

Sit on a chair with your back supported, feet flat on the floor and uncrossed, and the arm supported on a table so that the middle of the cuff is at heart level. Place the cuff directly on the skin about two finger-widths above the elbow, snug enough to slip one finger underneath. Do not talk or look at your phone while measuring, because talking alone raises the reading.

### 4. A structured schedule and honest recording

Take two readings a minute apart in the morning before medicine and food, and two in the evening, for three to seven days before your review. Record every reading exactly as it appeared, without selecting or deleting the high ones, together with date and time. If you feel dizzy on standing, measure seated then standing at one and three minutes and record the difference.

### 5. Ambulatory monitoring and interpreting results

When white-coat or masked hypertension is suspected, or readings vary widely, an ambulatory monitor is fitted to record automatically over 24 hours including sleep, giving a more accurate picture than any single measurement. The doctor interprets the record alongside your age, kidney function, diabetes and medicines, and decides whether to start, adjust or wait. The device is a measuring tool, not a decision-making tool.

اشترِ جهازاً موثوقاً يُلبس على أعلى الذراع، وقِس محيط ذراعك قبل الشراء لتختار مقاس الكفّ الصحيح، واحتفظ بدليل الاستخدام. تأكّد من البطاريات وتاريخ الجهاز ووقته حتى تُسجَّل القراءات صحيحة. جهّز دفتراً أو تطبيقاً لتسجيل التاريخ والوقت وكل قراءة كما ظهرت. قبل كل قياس امتنع عن القهوة والتدخين والمجهود نصف ساعة، وأفرغ مثانتك، واجلس هادئاً خمس دقائق. أخبر طبيبك بكل أدويتك ومنها المسكّنات ومزيلات احتقان الأنف وحبوب منع الحمل والمكمّلات لأنها تؤثّر في القراءة. وإن كنت ستلبس جهاز الأربع والعشرين ساعة، فارتدِ قميصاً واسع الأكمام، وواصل يومك الطبيعي، ولا تُبلّل الجهاز، وسجّل أوقات النوم والأعراض.

## العناية بعد الإجراء

أحضر سجل قراءاتك كاملاً وجهازك نفسه إلى كل مراجعة، فالسجل الأمين أهم من أي قراءة منفردة. لا تعدّل دواءك ولا توقفه بناءً على قراءة أعجبتك أو أقلقتك، وناقش النمط مع طبيبك فهو من يفسّره. لا تُكرّر القياس عشرات المرات يومياً لأن ذلك يرفع القراءة ويزيد القلق؛ التزم بالجدول المتفق عليه. أعِد فحص دقّة الجهاز دورياً وقارنه بقياس العيادة. \*\*واطلب الطوارئ فوراً عند قراءة عالية جداً مصحوبة بصداع شديد أو ألم في الصدر أو ضيق نفس أو تغيّر في الرؤية أو ضعف أو تنميل في أحد شقّي الجسم أو تلعثم أو تشوّش وعي، وكذلك عند قراءة منخفضة جداً مع إغماء أو دوار شديد أو برودة وتعرّق أو تسرّع نبض.\*\* وراجع طبيبك سريعاً عند تكرار القراءات المرتفعة أو المنخفضة أو الدوخة المتكرّرة عند الوقوف.

## About this service

Blood pressure monitoring means measuring it in a structured, repeated way outside the moment of the clinic visit, either with a home device over several days or with an ambulatory monitor worn for twenty-four hours that records automatically during the day and during sleep. It serves three purposes: confirming or excluding the diagnosis, showing whether treatment truly controls the pressure across the whole day, and following the effect of any change to the plan. Blood pressure is not a fixed number; it shifts with the hour, the situation and activity, which is why no decision is built on a single reading.

\*\*Faulty measurement technique produces faulty decisions\*\*, and this is the commonest mistake made at home. The main errors are: a cuff smaller or larger than the arm circumference, wrapping the cuff over a thick sleeve, sitting without back support, letting the arm hang unsupported or holding it above or well below heart level, crossing the legs, talking or looking at a phone during the reading, measuring after coffee, smoking, exertion or with a full bladder, measuring without five minutes of rest, and relying on one reading without repeating it. Correct technique is: five minutes of rest, seated with the back supported, feet flat on the floor, arm supported on a table at heart level, cuff on bare skin, complete silence, and two readings a minute apart with both recorded.

Monitoring reveals patterns invisible in clinic: readings that rise only in the clinic because of stress, known as white-coat hypertension; masked hypertension that looks normal in clinic and is high at home, which is the more dangerous because it goes untreated; readings that are high only in the morning; and low readings with dizziness on standing, which call for a measurement taken standing after sitting. Monitoring also covers the low side: falls in blood pressure causing dizziness, fainting, fatigue or nausea, particularly in older people and those on several medicines, and a standing measurement is usually what exposes them.

The limits should be stated clearly. The device gives numbers; it does not diagnose on its own, does not build a plan, and does not adjust a dose. Wrist and finger devices are less accurate than an upper-arm monitor. Anxious repeated measuring dozens of times a day raises the reading itself and adds stress without benefit. The proper approach is structured readings at set times, brought to the doctor, who interprets them in the context of your age, other conditions and medicines. Any decision to start, adjust or stop treatment is made by clinical examination and direct assessment, not by reading a page.

## Preparation

Buy a validated upper-arm monitor, measure your arm circumference before purchase so you choose the right cuff size, and keep the instruction leaflet. Check the batteries and that the device date and time are correct so readings are logged properly. Prepare a notebook or app to record the date, time and every reading exactly as displayed. Before each measurement avoid coffee, smoking and exertion for half an hour, empty your bladder, and sit quietly for five minutes. Tell your doctor about all your medicines, including painkillers, nasal decongestants, contraceptive pills and supplements, because these affect the reading. If you are to wear a 24-hour monitor, choose a loose-sleeved shirt, carry on with your normal day, keep the device dry, and note your sleep times and any symptoms.

## Aftercare

Bring your complete record and the device itself to every review, since an honest log matters more than any single reading. Do not adjust or stop your medicine because of a reading that pleased or alarmed you; discuss the pattern with your doctor, who interprets it. Do not measure dozens of times a day, as this raises the reading and increases anxiety; stay with the agreed schedule. Have the device accuracy rechecked periodically against a clinic measurement. \*\*Call emergency services immediately for a very high reading with severe headache, chest pain, breathlessness, change in vision, weakness or numbness on one side of the body, slurred speech or confusion, and equally for a very low reading with fainting, severe dizziness, cold sweating or a racing pulse.\*\* See your doctor promptly for repeatedly high or low readings, or repeated dizziness on standing.
