## Procedure steps

### 1. Assessment and scheduling the delayed visits

The team reviews your doctor's question, your history, and your medicines — particularly antibiotics and steroids — and explains that the study needs delayed visits one, two, and sometimes three days later. The appointments are given to you in writing, and keeping them is a condition of a valid study, because a single image at the wrong time cannot be interpreted.

### 2. Intravenous injection of the tracer

The tracer is injected into a vein in the hand or arm within a few minutes. Most protocols require no fasting, and nothing is expected beyond the sensation of the needle. Images are usually not taken at this visit, so you leave the centre and return at the appointed time, going about your day normally without special restrictions unless you have been told otherwise.

### 3. Bowel preparation when required

Part of the tracer is cleared through the bowel, so abdominal activity can appear and confuse interpretation. Some centres therefore give a laxative or dietary instructions before the delayed images to clear the bowel. Follow exactly what you are given in writing, do not take a laxative on your own initiative, and drink enough fluid with it.

### 4. Delayed whole-body imaging

At the delayed appointment you lie on a couch while the camera passes slowly over the body from head to feet, and tomographic images of a particular region may be added. The session usually lasts from about half an hour to an hour, and staying still is essential for image sharpness. Tell the technologist if lying flat is painful so that you can be supported before scanning starts.

### 5. Reading the result in the context of the illness

The nuclear medicine physician compares the sites of accumulation, their intensity, and their distribution, and issues a descriptive report. The result is then read alongside your clinical examination, inflammatory markers, cultures, and CT or MRI before any treatment decision, because the scan points to the location and leaves the cause to be established by clinical assessment and, where needed, a sample or culture.

## About this service

A gallium scan is a nuclear medicine study that searches the whole body for sites of hidden inflammation or infection, and for some tumours. A tracer carrying gallium is injected into a vein; over time it accumulates in tissue where inflammatory activity is present, and images are taken hours and days later rather than minutes later, because the accumulation itself takes time. By its nature, therefore, this is a study spread across more than one visit.

It is usually requested when there is a clear question that ordinary imaging has not answered: recurrent fever with no identified source, suspicion of deep infection in the spine, in bone, or around a prosthesis, or assessment of how active and how widespread certain inflammatory and granulomatous lung diseases are. It may also be used to follow how a known inflammatory process responds to treatment. As with nuclear medicine generally — including nuclear cardiac studies — the scan is ordered to answer a specific clinical question, never as a routine check-up or a general exploratory survey in someone without symptoms.

The dose is low, diagnostic, and justified by the question being asked; it leaves the body gradually in urine and stool. The injection itself causes nothing beyond the needle prick, no sedation is involved, and nothing else enters the body. Even so, \*\*tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant before the injection\*\* — the study may be cancelled, postponed, or replaced with another approach. Mention as well if you are taking an antibiotic or a steroid, since both affect how the images are interpreted.

The limits of the scan are fundamental. It shows where inflammatory activity is, not what is causing it: it cannot reliably separate infection from tumour or from non-microbial inflammation, it cannot identify which organism is responsible — cultures and laboratory tests do that — and it does not give the anatomical detail of CT or MRI. A negative scan does not entirely rule disease out. The findings must be read together with your clinical examination, laboratory results, and other imaging, never in isolation and never self-interpreted, and whether you need the scan is decided by your doctor's examination.

## Preparation

Keep the delayed appointments exactly as they were given to you in writing: the study runs over several days and is not valid from a single image. Tell your doctor about all your medicines, especially antibiotics and steroids, and do not stop anything yourself — any hold is made on the doctor's instruction. Mention recent contrast imaging or oral contrast agents, any surgery, prosthesis, or catheter, and any cultures recently taken. Most protocols need no fasting, but if you are given a laxative or dietary instructions before the delayed images, follow them precisely and drink enough fluid. Wear comfortable, metal-free clothing and remove jewellery. \*\*Tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant\*\* before the injection, as the study may be cancelled or replaced. Tell the technologist if lying flat for a long period is painful for you.

## Aftercare

Drink more fluid than usual across the days the study spans and pass urine whenever you need to, which speeds clearance and improves image quality, and wash your hands after using the toilet. If you were given a laxative, expect loose stool or mild cramping for some hours; this is expected and short-lived. In ordinary diagnostic use no special restrictions at home or at work are needed, and if you are breastfeeding, follow the centre's instructions about feeding and the interval it specifies. Do not restart a medicine that was held before the scan without your doctor's agreement. \*\*Seek care immediately if you develop fever with shivering, low blood pressure, or confusion; new severe back pain with weakness or numbness in the legs; or a rash, itching, or breathlessness after the injection.\*\* Do not interpret the report yourself: it is read together with your doctor's examination, blood tests, and imaging.
