## Procedure steps

### 1. A careful medication review before the appointment

This is the single most important step. Your doctor goes through every medicine you take — for blood pressure, mood, or the nose, including anything bought without a prescription — and decides what must be held and for how long before the scan. Any hold is made on the doctor's instruction and to a written schedule, never by your decision, and a drug may be temporarily swapped for another rather than stopped, to protect your blood pressure.

### 2. Protocol protection and the injection

A medicine that protects the thyroid may be given before the injection, depending on the protocol and the tracer used. The tracer is then injected slowly into a vein under observation, and some centres check blood pressure and pulse before and afterwards. Tell the team at once if you feel palpitations, a sudden headache, or sweating during the injection.

### 3. Waiting for the tracer to accumulate

Images are not taken straight away, because accumulation in the target cells takes time. You leave the centre and return at an appointment it sets — usually about a day later — and a second appointment the following day may be requested. Drink fluids during this period and pass urine whenever you need to, and keep the appointments exactly, since timing is part of the study.

### 4. Whole-body and tomographic imaging

You lie on the couch while the camera passes slowly from head to feet, and tomographic images of the abdomen, chest, or neck may be added to pin the location down more precisely. The session usually lasts from about half an hour to an hour and stillness is essential. Children may need special settling arrangements and a parent present.

### 5. Reading the result with tests and other imaging

Each area of uptake is described by site, intensity, and number, and the images are then read together with hormone tests, CT or MRI, the clinical examination, and genetic assessment where relevant. It is on that combined picture that a decision about surgery, monitoring, or targeted treatment is based, and it is explained to you by your treating team rather than by the report alone.

## About this service

An MIBG adrenal scan is a nuclear medicine study that localises tumours arising from tissue that produces adrenaline and related messengers. A tracer chemically similar to a natural nerve messenger is injected into a vein and is taken up by cells that store that messenger; whole-body images are then acquired about a day later and sometimes again the following day. The result is a map of where such activity sits, not an anatomical picture.

The scan is requested once laboratory tests point to a tumour of this type — an adrenal medullary tumour, a paraganglioma outside the adrenal gland, or neuroblastoma in children — and the questions are practical: exactly where is it, is there one lesion or several, has it spread beyond the adrenal, and does it take up this tracer at all, which matters before any tracer-based treatment is considered. It is therefore a study of location and extent, ordered for a specific question and never as a routine check or for someone without symptoms or suspicious laboratory results.

The dose is low, diagnostic, and justified by the question being asked, and it clears gradually in the urine. The most important part of preparation is that \*\*many common medicines block uptake of the tracer and can ruin the whole study\*\* — including some blood pressure drugs, certain antidepressants, and nasal decongestant sprays and drops. Your doctor reviews your medication list and decides what is held and when: \*\*any hold is made on the doctor's instruction, never on your own\*\*, because stopping a blood pressure medicine yourself can be genuinely dangerous in this particular condition. A thyroid-protecting medicine may also be given according to protocol. And \*\*tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant before the injection\*\* — the study may be cancelled, postponed, or replaced.

The limits are clear. The scan does not make the diagnosis: that rests on hormone tests. It cannot reliably separate a benign from a malignant tumour, it does not replace CT or MRI for size and relationship to neighbouring organs, and a negative scan does not exclude a tumour, because some lesions do not take up this tracer and need a different one. The findings must be read together with your clinical examination, laboratory results, and other imaging — never alone and never self-interpreted.

## Preparation

Bring a written list of all your medicines and supplements, including anything bought without a prescription such as nasal decongestants, because some of them block tracer uptake and ruin the study. Any hold or substitution is made on your doctor's instruction and to a schedule the doctor sets, and \*\*do not stop a blood pressure medicine yourself in this condition\*\*. Bring your hormone test results, CT or MRI images, and previous reports. Ask about fasting, since some centres require it and others do not. Drink water well before the appointment unless you have been told not to, remove jewellery and metal, and wear comfortable clothing. Keep the appointment for the delayed images a day or two later, because the study is not valid without them. \*\*Tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant\*\* before the injection. If the scan is for a child, ask about settling arrangements and whether you can stay.

## Aftercare

Drink more fluid than usual over the next two days and pass urine whenever you need to, which speeds clearance, and wash your hands after using the toilet. Do not restart any medicine that was held before the scan except at the time your doctor specifies, and monitor your blood pressure if your doctor has advised it. If you were given a thyroid-protecting medicine, complete it as prescribed. If you are breastfeeding, follow the centre's instructions about feeding and the interval it sets; and if the scan was for a child, follow any written instructions on temporary distancing for the period the centre determines, without alarm. \*\*Seek care immediately if you develop a sudden severe headache, violent palpitations, heavy sweating with pallor, a sharp rise in blood pressure, chest pain, or breathlessness\*\*, and also if a rash or itching follows the injection. Do not interpret the report yourself: it is read with your tests, imaging, and your doctor's examination.
