Parathyroid Scan
- **Procedure duration:** The injection takes a few minutes, the early images usually fifteen to thirty minutes, and then there is a waiting interval set by the protocol before the delayed images, which take about the same time. The full visit therefore commonly runs from one to three hours at the centre, a little longer if tomographic images fused with X-ray or views of the upper chest are added.

## Procedure steps

### 1. Reviewing tests and medicines before the appointment

The team reviews your calcium, parathyroid hormone, and vitamin D results and your ultrasound report, and asks about any thyroid disease or thyroid medication and about recent imaging with iodinated contrast. Any medication hold is made on the doctor's instruction, never on your own, and you will be asked about previous neck surgery, since it changes where the gland is expected to be.

### 2. Intravenous injection of the tracer

The tracer is injected into a vein in the hand or arm within a few minutes. No sedation is needed and nothing is expected beyond the needle prick. Jewellery and any metal around the neck are removed, some protocols ask you to take out removable dentures, and the length of time you will need to stay still is explained before scanning begins.

### 3. Early neck imaging

You lie down with the neck slightly extended and the camera close to the front of the neck, and early images are taken showing the thyroid and neighbouring tissue. Staying still is essential, because any movement blurs small structures that lie close together. Tell the technologist if extending your neck is painful so it can be supported with a pad before scanning starts.

### 4. Delayed images and possible tomography

You return after an interval set by the protocol for delayed imaging, because the difference between the early and delayed pictures is what reveals the overactive gland. Tomographic images fused with X-ray may be added to establish depth and position, and the field may include the upper chest if the gland is in an unusual site.

### 5. The report and reading it with the surgeon

The nuclear medicine physician describes the site of any persisting focus, its depth, and its relationship to the thyroid. The report is then read with your calcium and parathyroid hormone levels, ultrasound, and clinical examination, and the surgeon and endocrinologist decide the plan with you. The image alone does not determine surgery; the decision rests on the whole picture.

## About this service

A parathyroid scan is a nuclear medicine study that locates an overactive parathyroid gland before surgery. The parathyroids are four small glands beside the thyroid that regulate blood calcium, and one of them can become overactive and push the calcium level up. A tracer is injected into a vein and taken up by tissue in the neck; early and delayed images are then acquired. Because the thyroid clears the tracer faster, the site of the overactive gland remains visible on the delayed pictures.

The scan is requested after laboratory tests have already established the problem — calcium and parathyroid hormone — and the question is purely practical: exactly where is the overactive gland so the surgeon can plan for it, and does it sit in the usual place or somewhere unexpected, behind the thyroid or slipped down towards the upper chest? It is equally useful when the problem persists or returns after previous surgery. Tomographic images fused with X-ray may be added to pin the site down more precisely, and the scan is normally read alongside neck ultrasound.

The dose is low, diagnostic, and justified by the localisation question. Nothing is expected from the injection beyond the needle prick, no sedation is used, and nothing enters the body. Even so, \*\*tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant before the injection\*\* — the study may be cancelled, postponed, or replaced with an alternative that uses no radioactive material. Mention any thyroid disease and any thyroid medication, and any recent imaging with iodinated contrast, since all of these affect the pictures; and any medication hold is made on the doctor's instruction, never on your own.

The limits matter. This is a localisation study, not a diagnostic one: the diagnosis rests on blood tests, not on the image, and a negative scan does not rule out an overactive gland, because small glands and overactivity of all four are frequently not seen. It does not measure calcium, it does not assess bone thinning or kidney stones — separate tests do that — and it cannot reliably distinguish a benign growth from the rare malignant ones. The result must be read together with your clinical examination, laboratory tests, and ultrasound, never alone and never self-interpreted.

## Preparation

Bring your calcium, parathyroid hormone, and vitamin D results, your neck ultrasound report, and any previous operation report, because this scan is read alongside them rather than on its own. Tell your doctor about all your medicines, especially thyroid medication and calcium or vitamin D supplements, and about any recent imaging with iodinated contrast; any medication hold is made on the doctor's instruction and at the time the doctor sets, not by your decision. Ask the centre about fasting, since some protocols require it and others do not. Remove jewellery, chains, and any metal around the neck, and wear comfortable clothing without metal zips. Drink water well unless you have been told otherwise. Ask when the delayed images are due so you can plan your day. \*\*Tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant\*\* before the injection, as the study may be cancelled or replaced.

## Aftercare

Drink more fluid than usual for the rest of the day and pass urine whenever you need to, which speeds clearance, and wash your hands after using the toilet. Return to your usual food, work, and activity; no special restrictions at home are needed in ordinary diagnostic use, and if you are breastfeeding, follow the centre's instructions about feeding and the interval it specifies. Do not restart a medicine that was held before the scan without your doctor's agreement, and do not start or stop a calcium or vitamin D supplement on the basis of an image. \*\*Seek care immediately if you develop severe nausea and vomiting that prevents you drinking, confusion or unusual drowsiness, marked muscle weakness, or severe flank or abdominal pain\*\* — these can indicate a high calcium level that needs urgent assessment. The result is read with your blood tests and your doctor's examination before any surgical decision.
