Hepatobiliary Scan ( HIDA )
- **Procedure duration:** The dynamic imaging usually takes about an hour on the couch, and with arrival, preparation, and the injection, plan on one to two hours at the centre. If a medicine is given to make the gallbladder contract, or delayed images are needed, the visit may stretch to around three hours. Infant protocols may need preparation starting days earlier.

## Procedure steps

### 1. Fasting for the interval the centre specifies

Fasting is a condition of a valid study: a gallbladder already full of bile after a meal will not take up the tracer, producing a misleading result. Excessively long fasting also distorts the picture, so the centre specifies the interval precisely and asks when you last ate. Tell the team if you have diabetes or are on intravenous feeding, because the protocol changes.

### 2. Reviewing painkillers and medicines

Strong painkillers tighten the outlet of the bile duct and change how bile moves, so you are usually asked to avoid them for a period before the scan. That avoidance is set by your doctor and for an interval the doctor specifies: do not stop a painkiller you genuinely need on your own, but ask about a suitable alternative. Tell the team all your medicines and about any previous gallbladder or bile duct surgery.

### 3. Injection and the start of dynamic imaging

You lie on the couch with the camera over the upper abdomen, the tracer is injected into a vein, and recording begins at the same moment. Sequential images are usually acquired for about an hour, and you need to stay still because movement blurs the liver, ducts, and gallbladder together. Apart from the needle prick the study is painless, and you can breathe and talk normally.

### 4. Stimulation or delayed images when needed

If the question is whether the gallbladder can empty, a medicine that makes it contract is given and imaging continues so that the amount emptied can be measured. If the gallbladder has not appeared within the first hour, delayed images may be taken some hours later, or an adjunct medicine may be given on the doctor's order. Mild nausea or passing cramps are expected and not a cause for concern.

### 5. Reading the result with the rest of the workup

The report describes when the liver, ducts, gallbladder, and bowel appeared, any leak or delay, and the emptying fraction if it was measured. The result is then read with your clinical examination, liver and biliary blood tests, and ultrasound before any surgical decision. Neither a number nor an image alone decides on removing a gallbladder: the decision rests on the whole picture.

## About this service

A hepatobiliary scan, commonly called a HIDA scan, is a nuclear medicine study that follows the path of bile moment by moment. A tracer is injected into a vein, taken up by liver cells, and excreted into the bile, so the camera can watch it leave the liver, pass into the bile ducts, fill the gallbladder, and drain into the bowel. What you get is effectively a film of bile flow and gallbladder function, not a still anatomical picture.

The scan answers specific questions. Is the gallbladder acutely inflamed when ultrasound has not settled the matter — since failure of the gallbladder to fill with tracer is an important sign? Is bile leaking after gallbladder surgery or injury? Is the bile pathway obstructed? And does the gallbladder contract and empty as it should in someone with recurrent pain in the upper right abdomen? It is also used in infants with persistent jaundice to assess whether bile reaches the bowel, under a special protocol set by the doctor.

The dose is low, diagnostic, and justified by the question asked, and it leaves the body in bile and urine. Midway through the study a medicine that makes the gallbladder contract may be given; you may feel mild nausea, cramping, or an urge to open your bowels for a few minutes, which is expected and short-lived rather than a complication. \*\*Tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant before the injection\*\* — the study may be cancelled, postponed, or replaced with an alternative that uses no radioactive material. Say too if you use strong painkillers, since they alter bile flow and can spoil the result.

The limits should be clear from the outset: \*\*this scan does not show gallstones\*\* — seeing stones is ultrasound's job — it does not measure liver function the way blood tests do, it is not a way of cleansing the liver or gallbladder as is sometimes wrongly claimed, and it does not diagnose stomach ulcer, reflux, or irritable bowel. A normal scan does not rule out other causes of upper abdominal pain. The findings must be read together with your clinical examination, blood tests, and ultrasound, never alone and never self-interpreted, and whether you need the scan is decided by your doctor's examination.

## Preparation

Keep to the fasting interval the centre specifies exactly — neither shorter nor much longer — because eating beforehand or fasting far too long will spoil the result, and tell the team when you last ate. Avoid strong painkillers for the period your doctor specifies; do not stop a painkiller you genuinely need on your own, but ask about an alternative. Tell your doctor all your medicines, any previous gallbladder or bile duct surgery or endoscopy, and whether you have diabetes or are on intravenous feeding. Bring your ultrasound reports and liver blood tests. Drink water according to the centre's instructions and remove metal from the abdominal area. \*\*Tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant\*\* before the injection. If the study is for an infant, ask about the special preparation that may begin days earlier on the doctor's order.

## Aftercare

Return to your usual food and drink after the scan unless the centre has advised otherwise, starting with a light meal if the fast was long. Drink more fluid than usual for the rest of the day, pass urine whenever you need to, and wash your hands after using the toilet. If you were given a medicine to make the gallbladder contract, mild cramping, nausea, or loose stool may persist for an hour or two; this is expected and settles by itself. If you are breastfeeding, follow the centre's instructions about feeding and how long to wait. \*\*Seek care immediately if you develop severe or worsening pain in the upper right abdomen, fever with shivering, yellowing of the eyes or skin, persistent vomiting that prevents you drinking, or dark urine with pale stool\*\* — these need urgent assessment rather than waiting for an appointment. Do not interpret the report yourself.
