## Procedure steps

### 1. Medication review and a short fast

The team reviews your medicines, since many allergy, mood, bladder, and blood pressure drugs dry the mouth and reduce secretion; some may be held temporarily — on the doctor's instruction, never on your own. A short fast is usually required, along with no smoking, no chewing gum, and no sour foods beforehand, because stimulating the glands before the study empties them and distorts the result.

### 2. Positioning under the camera and the injection

Your head is placed in a fixed position in front of or under the camera, jewellery and removable dentures are taken out if requested, and the tracer is then injected into a vein with recording starting at the same instant. There is no sedation and nothing enters the mouth at this stage, and it will be explained to you that moving or talking a great deal distorts the curve.

### 3. Dynamic imaging of the uptake phase

The camera records a series of images showing the tracer gradually accumulating in the glands under the jaw and in front of the ear on both sides, so the glands can be compared with each other. This phase usually takes about twenty minutes, and you are asked to stay still, to avoid swallowing repeatedly as far as you can, and to breathe normally.

### 4. The sour stimulus and the secretion phase

Midway through the study a sour stimulus is given by mouth — lemon juice or something similar — so the glands release what they have accumulated, and the equipment records how quickly and how much each gland secretes. You will notice a sharp taste and more saliva for a few minutes, which is intended and temporary. Tell the technologist at once if a painful sore in your mouth would make the sour stimulus intolerable.

### 5. Reading the curves in context

The nuclear medicine physician compares each gland's uptake and secretion and describes glands that are weak, delayed, or unresponsive. The result is then read with your clinical examination, immune blood tests, ultrasound or MRI, and dental or eye assessment where relevant. A curve on its own establishes no diagnosis and is not for self-interpretation.

## About this service

A salivary gland scan is a nuclear medicine study that measures how the major salivary glands work rather than what they look like. A tracer is injected into a vein, taken up by salivary gland tissue, and secreted with saliva into the mouth, and the camera records that whole path live: how much each gland takes up, when it secretes, and how it responds to a sour stimulus given midway through the study. The result is a functional curve for four glands at once.

The scan is requested for defined questions. Is a dry mouth actually caused by reduced gland secretion or not? How much working tissue remains in someone whose glands have been affected by chronic immune inflammation, by radiotherapy, or after radioiodine treatment? Is there a drainage hold-up in a duct that would explain recurrent swelling with meals? And how well does the opposite gland work before a surgical decision on one side? It is a functional study ordered for a specific question, never as a routine check in someone without symptoms.

The dose is low, diagnostic, and justified by the question asked. Nothing is expected from the injection beyond the needle prick, and the sour stimulus is given by mouth, so you will notice a sharp taste and a rush of saliva for a few minutes — which is exactly its purpose. \*\*Tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant before the injection\*\*, as the study may be cancelled, postponed, or replaced with an alternative that uses no radioactive material. Mention all your medicines too, because many common drugs dry the mouth and change the result, and any hold is made on the doctor's instruction rather than on your own.

The limits are clear: this study measures function, not structure. It does not show a stone or a narrowing in a duct — ultrasound, duct imaging, CT, or MRI do that — it cannot say whether a lump in a gland is benign or malignant, and it does not diagnose chronic immune inflammation on its own, which needs blood tests, eye assessment, and sometimes a small biopsy. A dry mouth is also not an inevitable consequence of ageing, as is often assumed: it has causes that can be assessed and managed, medication being the commonest. The findings must be read together with your clinical examination, laboratory tests, and other imaging, never alone and never self-interpreted.

## Preparation

Keep to the short fast the centre specifies before your appointment, and avoid smoking, chewing gum, and sour foods or sweets beforehand, because stimulating the glands in advance empties them and makes the result invalid. Tell your doctor about all your medicines and supplements, since many allergy, mood, bladder, and blood pressure drugs dry the mouth; any hold is made on the doctor's instruction and at the time the doctor sets, not by your decision. Mention any radiotherapy to the head and neck or previous radioiodine treatment, any recent contrast imaging, and any gland surgery or recurrent gland inflammation. Remove jewellery and removable dentures if asked. \*\*Tell your doctor if you are pregnant, breastfeeding, or think you might be pregnant\*\* before the injection, as the study may be cancelled or replaced. And tell the technologist if you have a painful sore in your mouth.

## Aftercare

Drink water freely after the scan and pass urine whenever you need to, which speeds clearance, and wash your hands after using the toilet. A sharp taste or extra saliva may linger briefly and then settle. Return to your usual food and medicines, and do not restart a medicine that was held before the scan without your doctor's agreement. If dry mouth is your complaint, look after your teeth carefully — regular brushing and dental review — because reduced saliva raises the risk of decay, and frequent sips of water help. If you are breastfeeding, follow the centre's instructions about feeding and how long to wait. \*\*Seek care immediately if you develop rapid swelling of the face or under the jaw with fever, pus in the mouth, or difficulty opening your mouth, swallowing, or breathing\*\* — these are signs of acute gland infection that needs urgent treatment.
