الفائدة هنا ليست محلّ جدال علمي: أمراض كانت تقتل الأطفال أو تُقعدهم بالشلل أو تُصمّهم أو تسبّب التهاب سحايا وتلفاً دماغياً صارت نادرة حيث انتظم التطعيم، وبعضها اختفى من العالم أو كاد. وحين تنخفض نسبة المطعَّمين في مجتمع ما تعود هذه الأمراض بالفعل، وأول من يدفع الثمن الأطفال الصغار جداً ومن لا يستطيعون التطعيم لأسباب مرضية، لأنهم يعتمدون على مناعة من حولهم.

نتفهّم قلق كثير من الأهل، ونجيب بصراحة عن أشهر خرافة: \*\*لا يوجد ارتباط بين التطعيمات والتوحد\*\*. الدراسة الصغيرة التي أطلقت هذه الفكرة سُحبت رسمياً من مجلّتها بعد ثبوت خلل جوهري في بياناتها، وتلاها عدد كبير من الأبحاث الواسعة في بلدان مختلفة لم تجد أي علاقة. سبب الالتباس أن علامات التوحد تظهر عادة في العمر نفسه الذي تُعطى فيه بعض المطاعيم، فالتزامن الزمني فُهم خطأً على أنه سبب. سؤالك عن سلامة ما يُعطى لطفلك حقّ، والإجابة الصادقة أن الدليل هنا واضح ومتراكم.

ومن الإنصاف ذكر الحدود: المطعوم لا يمنع كل مرض، ولا يمنع نزلات البرد العادية، ولا يُغني عن نظافة اليدين وسلامة الغذاء والماء، ولا يعالج مرضاً بعد أن يبدأ. وتأخير جرعة لا يعني البدء من الصفر؛ الجدول يُستكمل من حيث توقّف دون إعادة السلسلة كاملة. أخيراً: الحرارة الخفيفة أو انتفاخ موضع الإعطاء أو النعاس والفتور ليوم أو يومين استجابة مناعية متوقّعة وليست مضاعفة ولا دليلاً على خطأ. تحديد ما يستحقّه طفلك الآن، وأي حالة تستوجب تأجيلاً أو استثناءً، يُقرَّر بالفحص السريري ومراجعة سجلّه لا بالقراءة.

## خطوات الإجراء

### 1. مراجعة السجل وتحديد الجرعات المستحقّة

يُفتح سجل التطعيم أو البطاقة الصحية ويُقارَن بالجدول الوطني بحسب عمر الطفل، فتُحدَّد الجرعات المستحقّة اليوم والمتأخّرة، ويُوضَع جدول لحاق للمتأخّر يُكمل السلسلة من حيث توقّفت دون إعادتها من البداية.

### 2. الفحص السريري وأسئلة السلامة

يُفحص الطفل ويُقاس وزنه وحرارته، ويُسأل الأهل عن مرض حاد حالي، وعن نقص المناعة أو علاج مثبِّط للمناعة، وعن حساسية شديدة سابقة أو تفاعل غير معتاد بعد جرعة ماضية، وعن نوبات أو أمراض دم أو مسيّلات.

### 3. شرح الفائدة والأعراض المتوقّعة

يُشرح للأهل ما يقي منه كل مطعوم، وما هو متوقّع بعده مثل الحرارة الخفيفة وانتفاخ الموضع والفتور، وما هو غير متوقّع ويستوجب مراجعة فورية، وتُتاح لهم فرصة السؤال قبل الموافقة على الإعطاء.

### 4. الإعطاء بتقنية آمنة وتهدئة الطفل

يُعطى المطعوم في الموضع المناسب لعمر الطفل بأدوات معقّمة تُستخدم لمرة واحدة، مع تثبيت لطيف وتشتيت انتباه؛ ويساعد احتضان الطفل أو إرضاعه بعد الإعطاء مباشرة على تقصير البكاء وتقليل التوتّر.

### 5. المراقبة والتوثيق والموعد التالي

يُطلب البقاء في العيادة مدّة قصيرة للمراقبة تحسّباً لتفاعل تحسّسي فوري نادر، ثم يُوثَّق كل مطعوم في البطاقة مع تاريخه وموضعه ورقم دفعته، ويُحدَّد موعد الجرعة القادمة كتابةً.

## Procedure steps

### 1. Reviewing the record and identifying due doses

The immunisation card or health record is reviewed against the national schedule for the child's age, identifying which doses are due today and which are overdue. A catch-up plan resumes the series from where it stopped rather than restarting it.

### 2. Clinical check and safety questions

The child is examined and weight and temperature recorded. Parents are asked about any current acute illness, immune deficiency or immunosuppressive treatment, previous severe allergy or unusual reaction after an earlier dose, and any seizures, blood disorders or blood-thinning treatment.

### 3. Explaining benefit and expected reactions

Parents are told what each vaccine protects against, what is expected afterwards such as mild fever, local swelling and lethargy, and what is not expected and requires urgent review. They are given the chance to ask questions before consenting.

### 4. Safe administration and comforting the child

The vaccine is given at the site appropriate for the child's age using sterile single-use equipment, with gentle holding and distraction. Cuddling or breastfeeding immediately afterwards shortens crying and reduces distress.

### 5. Observation, documentation and next appointment

A short period of observation in the clinic is requested to cover the rare possibility of an immediate allergic reaction. Each vaccine is then documented on the card with its date, site and batch number, and the next appointment is written down.

## التحضير قبل الإجراء

أحضر بطاقة التطعيم أو السجل الصحي في كل زيارة، فهي أساس القرار ولا تُغني عنها الذاكرة. أخبر الطبيب إن كان الطفل يعاني حرارة الآن أو مرضاً حاداً، أو لديه نقص مناعة أو يتلقّى علاجاً مثبِّطاً للمناعة، أو حدث لديه تفاعل شديد أو حساسية معروفة بعد جرعة سابقة، أو يتناول مسيّلاً للدم. لا تؤجّل الموعد لأجل رشح خفيف بلا حرارة، فهذا ليس مانعاً. أطعِم الطفل أو أرضعيه قبل الموعد وألبسه ملابس تسهّل الوصول إلى الفخذ أو أعلى الذراع. لا تعطِ خافض حرارة على سبيل الوقاية قبل الإعطاء إلا بتوجيه من الطبيب. اصطحب معك لعبة مفضّلة، وتحدّث مع الطفل الأكبر بصدق وباختصار ولا تَعِده بأنه «لن يشعر بشيء».

## العناية بعد الإجراء

ابقَ في العيادة للمراقبة القصيرة قبل المغادرة. توقّع أعراضاً بسيطة خلال يوم أو يومين: حرارة خفيفة، احمرار وانتفاخ وألم في موضع الإعطاء، نعاس، أو قلّة شهيّة — كلّها استجابة مناعية متوقّعة وليست مضاعفة. أعطِ الطفل سوائل أكثر وأرضعيه على الطلب، وضع كمّادة باردة على الموضع، ولا تفرك المكان ولا تضع عليه معجوناً أو أعشاباً. حرّك الذراع أو الساق حركة طبيعية عادية. راجع فوراً عند: \*\*صعوبة تنفّس أو صفير، أو تورّم في الوجه والشفتين أو اللسان، أو طفح منتشر سريع خلال دقائق إلى ساعات\*\*، \*\*بكاء متواصل غير معتاد لساعات لا يهدأ\*\*، \*\*خمول شديد أو صعوبة في إيقاظ الطفل أو شحوب وبرودة\*\*، \*\*تشنّج أو رفرفة في الأطراف\*\*، \*\*حرارة عالية مستمرة أو أي حرارة عند طفل أقل من ثلاثة أشهر\*\*، \*\*احمرار متزايد وساخن في موضع الإعطاء مع إفرازات بعد اليوم الثاني\*\*. واحتفظ بالبطاقة موثّقة وحدّد موعد الجرعة القادمة قبل أن تخرج.

## About this service

Childhood immunisation is a preventive procedure given according to an organised national schedule. A vaccine introduces the immune system to a harmless part of a germ, or to a weakened version of it, so the body learns to recognise it and prepares a ready defence before meeting it in real life. The idea is simple and powerful: the child gains protection without going through the disease or its complications.

The benefit is not scientifically disputed. Diseases that used to kill children, paralyse them, leave them deaf, or cause meningitis and brain damage have become rare wherever immunisation is maintained, and some have disappeared from the world or nearly so. When the proportion of vaccinated children in a community falls, these diseases do return, and the first to pay are the very young and those who cannot be vaccinated for medical reasons, since they depend on the immunity of everyone around them.

Many parents worry, and that concern deserves a direct answer rather than dismissal. \*\*There is no link between vaccines and autism.\*\* The small study that launched this idea was formally retracted by its journal after fundamental problems with its data were established, and it was followed by very large studies in many countries that found no association whatsoever. The confusion arose because the signs of autism typically become noticeable at the same age some vaccines are given, so coincidence in timing was misread as cause. Asking whether what is given to your child is safe is entirely legitimate; the honest answer is that the evidence here is clear and consistent.

The limits should also be stated. A vaccine does not prevent every illness, does not prevent ordinary colds, does not replace hand hygiene and safe food and water, and does not treat a disease once it has started. A delayed dose does not mean starting again from zero: the schedule is resumed from where it stopped without repeating the whole series. Finally, mild fever, swelling at the injection site, or a day or two of sleepiness and reduced appetite are expected immune responses — not complications and not evidence that something went wrong. What your child is due now, and whether any condition justifies postponement or exemption, is decided by clinical examination and review of his record, not by reading online.

## Preparation

Bring the immunisation card or health record to every visit; it is the basis of the decision and memory is not a substitute. Tell the doctor if the child currently has a fever or an acute illness, has an immune deficiency or is receiving immunosuppressive treatment, has had a severe reaction or known allergy after a previous dose, or is taking a blood thinner. Do not postpone the appointment for a mild cold without fever, which is not a contraindication. Feed or breastfeed the child beforehand and dress him in clothes that give easy access to the thigh or upper arm. Do not give a fever-reducing medicine preventively before the vaccine unless the doctor advises it. Bring a favourite toy, and speak to an older child briefly and honestly rather than promising that he will feel nothing.

## Aftercare

Stay in the clinic for the short observation period before leaving. Expect minor effects over a day or two: mild fever, redness, swelling and tenderness at the injection site, sleepiness or reduced appetite — all expected immune responses rather than complications. Offer extra fluids, breastfeed on demand, apply a cool compress to the site, and do not rub it or apply pastes or herbs. Let the arm or leg move normally. \*\*Seek emergency care for difficulty breathing or wheeze, swelling of the face, lips or tongue, or a rapidly spreading rash within minutes to hours\*\*; \*\*persistent unusual inconsolable crying for hours\*\*; \*\*marked lethargy, difficulty waking the child, or pallor and cold skin\*\*; \*\*any convulsion or twitching\*\*; \*\*persistent high fever, or any fever in an infant under three months\*\*; or \*\*an increasingly red, hot site with discharge after the second day\*\*. Keep the card updated and fix the next appointment before you leave.
