Tooth Decay: Causes, Stages, Treatment, and Prevention

Tooth decay is one of the most common chronic diseases worldwide, affecting 60–90% of school-age children and most adults. Yet it is fully preventable when the mechanism is understood. In this guide, the ClinicsJo Editorial Team reviews the causes, stages, and treatment of each stage.

What Is Tooth Decay?

Tooth decay is a bacterial disease affecting the hard tissues of the tooth (enamel, dentin, cementum). Specialized bacteria — primarily Streptococcus mutans — consume sugars and carbohydrates from food residue and produce acids that gradually dissolve the tooth's minerals, forming a cavity.

Causes of Tooth Decay

  • High intake of sugars and carbohydrates, especially sticky sweets and soda.
  • Poor oral hygiene and plaque buildup.
  • Dry mouth — saliva naturally protects teeth.
  • Insufficient fluoride.
  • Teeth with deep fissures (hard to clean).
  • Aging and gum recession exposing roots.
  • Children sleeping with a bottle of milk or juice (Baby Bottle Tooth Decay).
  • Certain medications causing dry mouth.

Stages of Tooth Decay

1. White Spot (Initial Demineralization)

The first sign, appearing as a chalky white spot on the enamel. Reversible with fluoride and good hygiene — no drilling needed.

2. Enamel Decay

A cavity begins forming in the enamel. Still painless; needs a small filling.

3. Dentin Decay

The decay penetrates the enamel and reaches the dentin. Sensitivity to hot, cold, and sweets begins. Larger filling required.

4. Pulpitis

Decay reaches the nerve. Sharp, continuous pain. Root canal needed.

5. Abscess

Infection spreads to the jawbone. Swelling, pus, fever. Urgent treatment with antibiotics plus root canal or extraction is required.

Symptoms of Tooth Decay

  • A white, brown, or black spot on the tooth.
  • A visible or tongue-detectable hole.
  • Intermittent pain with sweets or cold drinks.
  • Pain when biting down.
  • Bad breath from the affected tooth.
  • Unpleasant taste in the mouth.

Diagnosis

Dentists use:

  • Visual exam with a fine probe.
  • Bitewing X-rays for decay between teeth.
  • Laser caries detectors (DIAGNOdent) for early lesions.
  • 3D imaging (CBCT) for complex cases.

Treatment

Stage 1 (White Spot)

  • High-concentration in-office fluoride application.
  • Fluoride-rich daily toothpaste.
  • ICON technique (special resin) — advanced no-drill solution.

Enamel and Dentin Decay

  • White composite filling: most common option, natural-looking.
  • Ceramic inlay/onlay: for larger decay, lab-fabricated.
  • Crown: if more than 50% of the tooth structure is lost.

Pulpitis or Abscess

  • Root canal + crown.
  • Antibiotics if there is active infection.
  • Extraction if the tooth cannot be saved.
Medical tip: Don't wait for pain. Early-stage decay causes no pain, and a check-up every 6 months allows your dentist to catch it early and treat it simply.

Prevention

  • Brush twice daily with toothpaste containing 1,000–1,500 ppm fluoride.
  • Floss daily.
  • Reduce sweets and soda, especially between meals.
  • Drink water after meals to rinse the mouth.
  • Eat foods rich in calcium and vitamin D.
  • For children: apply fissure sealants on permanent molars.
  • Visit your dentist for check-ups and cleaning every 6 months.
  • Avoid eating after brushing at night.

Tooth Decay in Children

Children in Jordan have high decay rates, driven by sugar consumption and limited awareness of cleaning baby teeth. Start cleaning your baby's gums even before the first tooth erupts. Use a small infant brush with rice-grain-sized fluoride toothpaste for children under 3, and pea-sized for older kids.
