What is Alcohol Withdrawal Syndrome?

Alcohol withdrawal syndrome (AWS) is a potentially life-threatening syndrome occurring in individuals with significant alcohol dependence who abruptly reduce or cease alcohol consumption. It results from CNS hyperexcitability following removal of alcohol's chronic CNS depressant effects. AWS spans a severity spectrum: mild (tremulousness, anxiety, insomnia — onset 6–24 hours), moderate (diaphoresis, tachycardia, hypertension, nausea/vomiting — hours 12–48), severe withdrawal seizures (6–48 hours after last drink), and the most dangerous complication — delirium tremens (DTs, hours 48–96).

The neurobiological basis: chronic alcohol use upregulates excitatory NMDA glutamate receptors and downregulates inhibitory GABA-A receptors as homeostatic adaptation to alcohol's dual CNS depressant effect (GABA-A potentiation + NMDA inhibition). Abrupt withdrawal removes this inhibition, leaving uncompensated CNS hyperexcitability — manifesting as tachycardia, hypertension, diaphoresis, tremor, seizures, and hallucinations. Unlike opioid withdrawal (distressing but not directly lethal), alcohol withdrawal can cause death directly from autonomic instability and status epilepticus.

Delirium tremens (DTs) — the most severe form — affects 5–8% of those hospitalized for withdrawal; it involves a triad of severe autonomic instability (fever, tachycardia, hypertension), agitation and confusion, and hallucinations (characteristically visual — seeing insects or animals). DTs mortality without treatment is 5–15%; with proper ICU-level management, mortality is <1%. The Clinical Institute Withdrawal Assessment for Alcohol (CIWA-Ar) scale is the standard tool for monitoring withdrawal severity and guiding treatment.

Symptoms

  • Mild withdrawal (6–24 hours after last drink): tremulousness (the classic 'shakes'), anxiety, insomnia, headache, palpitations
  • Moderate withdrawal (12–48 hours): diaphoresis, tachycardia, hypertension, nausea/vomiting, heightened reflexes
  • Withdrawal seizures (6–48 hours): generalized tonic-clonic seizures (rum fits); most occur within 24 hours; may be the first sign of withdrawal in unrecognized alcohol dependence
  • Alcoholic hallucinosis (12–24 hours): visual, auditory, or tactile hallucinations with intact consciousness (vs. DTs where consciousness is impaired)
  • Delirium tremens (48–96 hours): disorientation, agitation, visual hallucinations (insects, small animals), severe autonomic hyperactivity (fever >38.5°C, HR >120, diaphoresis), global confusion — medical emergency

Causes

  • Abrupt cessation or significant reduction of alcohol in a chronic heavy drinker
  • Physiological dependence: upregulated NMDA receptors and downregulated GABA-A receptors as adaptation to chronic alcohol exposure
  • Hospitalization or surgery that abruptly interrupts alcohol access — often unrecognized until withdrawal begins 12–48 hours post-admission
  • Risk factors for severe AWS: high daily alcohol consumption (>8 drinks/day), prior alcohol withdrawal seizures or DTs, concurrent medical illness, older age, prolonged drinking history, poor nutritional status

Diagnosis

Clinical diagnosis in context: alcohol use history + characteristic temporal symptom progression after last drink. CIWA-Ar (Clinical Institute Withdrawal Assessment for Alcohol, Revised): validated 10-item scale (0–67 points) assessing nausea, tremor, diaphoresis, anxiety, agitation, hallucinations, headache, orientation — guides medication titration; score >15 = severe withdrawal. Blood tests: blood alcohol level, electrolytes (hypomagnesemia, hypokalemia, hypophosphatemia common), blood glucose (hypoglycemia risk), LFTs, CBC, coagulation (coagulopathy from liver disease). EEG if seizure type unclear.

Treatment

Benzodiazepines are the pharmacological cornerstone — work synergistically with GABA-A receptors (same as alcohol) to prevent seizures and DTs. CIWA-Ar symptom-triggered therapy: more efficient than fixed dosing — give diazepam 5–10 mg IV or PO (or lorazepam 2–4 mg for hepatic failure) when CIWA-Ar ≥8–10; repeat every hour as needed. For severe/ICU withdrawal (DTs): high-dose diazepam loading (multiple doses until calm), mechanical ventilation if needed; phenobarbital or propofol for refractory cases. Thiamine 500 mg IV three times daily for 3 days BEFORE giving glucose — prevents Wernicke's encephalopathy (thiamine deficiency + glucose = Wernicke's precipitated). IV fluids: correct dehydration; supplement magnesium, potassium, phosphate. Antiepileptics (valproate, gabapentin, carbamazepine): adjuncts or alternatives in mild-to-moderate withdrawal. Beta-blockers and clonidine: control autonomic symptoms but do not prevent seizures — use only as adjuncts.

Complications

  • Delirium tremens: 5–15% mortality without treatment; <1% with appropriate management
  • Wernicke's encephalopathy from thiamine deficiency: ophthalmoplegia (CN VI palsy), ataxia, global confusion — untreated Wernicke's → irreversible Korsakoff's syndrome (anterograde amnesia, confabulation)
  • Generalized tonic-clonic seizures and status epilepticus
  • Severe hyponatremia from excessive free water intake during withdrawal
  • Post-acute withdrawal syndrome (PAWS): prolonged anxiety, depression, and sleep disturbance lasting weeks to months

Prevention

  • Medical supervision for alcohol cessation — heavy drinkers should never abruptly stop without medical support; scheduled benzodiazepine taper or inpatient detoxification prevents life-threatening withdrawal
  • Thiamine supplementation for all heavy drinkers, especially malnourished patients, to prevent Wernicke's
  • Alcohol use disorder treatment (naltrexone, acamprosate, behavioral therapy) for long-term sobriety
  • Healthcare providers should screen for alcohol dependence before planned surgery or hospital admission to anticipate and prevent withdrawal

Seek emergency medical care immediately for severe tremors, confusion, fever, hallucinations, or seizures occurring after stopping alcohol — these are signs of serious withdrawal requiring urgent IV benzodiazepines and thiamine. Do not attempt to stop heavy drinking alone without medical supervision — severe alcohol withdrawal can be fatal. Contact a medical detox facility or emergency department for safe supervised withdrawal management.
