The Texas technique, often marketed as jawline widening, is a commercial term for defining the jawline, sharpening its angle and strengthening the chin. It is usually performed without surgery by injecting cohesive fillers along the border of the lower jaw, at its angle and at the chin, sometimes combined with injections that relax an enlarged chewing muscle. It is not the name of a single standardised operation, so do not assume that what was done for someone else is what will be done for you. Because the name is commercial rather than scientific, the first things to ask are: exactly which material will be injected, its type and origin, at which levels and sites, who will perform the injection, how long it lasts, and what the plan is if the result does not satisfy you. A vague answer to any of these is reason enough to stop. Assessment separates three different causes of a poorly defined jawline: a deficient bony framework, sagging skin and soft tissue, and fat accumulation under the chin. Injection adds volume and visually defines the bony border in the first situation only. It does not treat the second or the third. The limits must be stated plainly. Injection does not remove a double chin and does not dissolve fat. It does not tighten loose skin, and its weight can actually make the contour look worse when sagging is the real problem. It does not change bone, and it does not correct a genuine skeletal discrepancy between the jaws, which is the field of orthognathic surgery. The result is temporary, usually lasting from several months to a year or more depending on the material, the site and your own tissues, and it needs repeating. Complications range from swelling, bruising, asymmetry, lumps and product migration to infection, and to a rare but serious event in which injected material blocks a blood vessel. For that reason injections should only be given by a qualified doctor in a medical setting able to manage an emergency. Never accept material of unknown origin, permanent fillers, or injections outside a clinic.

The doctor assesses the bony framework, tissue thickness, skin elasticity, submental fat, and the position of the teeth and bite, to establish whether you need volume, tightening, fat reduction or skeletal correction, which are entirely different options.

Explaining the options and their limits
You are told what injection can and cannot achieve, and when the correct choice is a different procedure or no procedure at all. A realistic goal is agreed, with baseline photographs so the outcome can be judged afterwards.

Mapping the sites and antisepsis
The skin is cleaned and disinfected, injection points and vascular danger zones are marked, topical or injected local anaesthetic is used, and the appropriate depth is chosen, on the bone or in the planned tissue layer.

Gradual injection with reassessment
Material is placed in small incremental amounts with repeated checks and comparison of symmetry between the two sides, sitting and lying down, because excess and haste are the commonest causes of unnatural results.

Review after about two weeks
Once the swelling has settled, the final contour is reviewed, a small corrective touch may be added if needed, and you are told how long the result should last, when a repeat is expected, and which signs need urgent review.
Before the procedure
Tell your doctor about all medicines and supplements, especially blood thinners, aspirin, anti-inflammatory drugs and supplements that increase bruising, and never stop a prescribed medicine on your own. Report any previous facial injections, when they were done and which material was used if you know it, and disclose any permanent or unidentified material previously injected, because it completely changes the decision. Mention autoimmune disease, skin inflammation, active acne, any infection in the area, or a history of recurrent cold sores. If you are pregnant or breastfeeding the procedure is postponed. Schedule injections at least two weeks before an important event, avoid alcohol and strenuous exercise beforehand, and arrive with a clean face without make-up.
After the procedure
Expect swelling and possibly bruising for some days, with a firm feeling at the site that eases gradually. Do not massage or press the area and do not sleep face down for the first two nights. Avoid intense heat, saunas, strenuous exercise and alcohol for 24 to 48 hours, and postpone laser, peels and heat based facial treatments until your doctor allows them. Use cold packs gently, cleanse without rubbing, and delay make-up for at least a day. Do not judge the result before two weeks. **Seek care immediately if you develop severe pain out of proportion to the procedure, blanching or grey or blue mottling of the skin, any change in vision or pain around the eye, swelling that increases after day three with fever, ulceration or spreading discolouration, or a painful enlarging lump.**
Expected duration
The session itself usually takes 20 to 45 minutes after the topical anaesthetic has worked, initial swelling settles within two to five days, and the final contour is assessed at about two weeks. The result generally lasts from several months to a year or more depending on the material and site.
