A facet joint injection is an interventional procedure that places local anaesthetic, sometimes with an anti-inflammatory medicine of the corticosteroid class, either inside the small joints between the vertebrae, known as facet joints, or around the tiny nerves that supply them, known as medial branches. It is carried out under X-ray or ultrasound guidance and serves both diagnostic and pain-relieving purposes. These joints are a common source of mechanical back or neck pain: localised pain that worsens on prolonged standing, arching backwards and twisting, that may spread to the buttock or back of the thigh without travelling down to the foot, and that eases on bending forward or sitting. No scan can prove on its own that the joint is the source, because degenerative facet changes are common in people with no symptoms at all. This is why the injection has an important diagnostic role: temporary relief after a targeted anaesthetic injection makes that joint the likely source and opens the option of radiofrequency treatment of the medial branch for longer-lasting effect. The limits must be stated clearly. It does not treat a herniated disc or radicular pain running down to the foot, it does not widen a narrowed spinal canal, it does not repair spondylolisthesis or instability, and it does not remove existing arthritis in the joint. Its effect is usually temporary and variable, and it should not be repeated without documented benefit. Most importantly, it enables rehabilitation rather than substituting for it: trunk strengthening, correction of posture and loading patterns, improved hip mobility and weight management are what protect the joint long term, and the injection is a window in which to begin them. Candidacy is decided by clinical examination and by matching the pain pattern to the suspected level, while excluding conditions that need urgent assessment such as progressive weakness or serious nerve compression symptoms.
Procedure steps
- 1
Identifying the suspected level
The pain pattern and its aggravating and easing factors are assessed, spinal range of motion and level-by-level tenderness are examined, and imaging is reviewed to select the candidate joint, with pre-procedure pain scores recorded for comparison.
- 2
Positioning, antisepsis and guidance
You usually lie face down, the skin is disinfected, and the fluoroscope is angled to display the facet joint or the course of the medial branch precisely, with vital signs monitored.
- 3
Skin anaesthesia and needle placement
The skin is numbed, then a fine needle is advanced towards the joint capsule or to the bony landmark the medial branch crosses, with the angle corrected step by step under imaging while you stay awake and describe what you feel.
- 4
Injection and confirming spread
A very small volume is injected once the position is confirmed, sometimes with contrast to exclude intravascular spread. The small volume is deliberate, because a diagnostic block loses its value if the medicine spreads to neighbouring structures.
- 5
Documenting the response and starting rehabilitation
The percentage and duration of pain relief in the following hours are recorded carefully, since this is the result of the diagnostic test and determines the next step, and the exercise programme is then resumed during the period of relative relief.
Before the procedure
Tell your doctor about all your medicines, especially blood thinners, anticoagulants and aspirin, and do not stop them yourself but ask about safe timing. Mention diabetes, since steroids can temporarily raise blood sugar; pregnancy or the possibility of it; and any allergy to contrast, local anaesthetic or antiseptics, active infection or fever, clotting problems or reduced immunity. Bring your MRI and X-ray images and reports. Very important: do not take a strong painkiller in the hours immediately before a diagnostic block, because it blurs the result of the test, and ask your doctor which medicines to continue. Wear comfortable clothing, arrange for someone to drive you, and do not plan heavy work for the same day.
After the procedure
Record your pain level hour by hour on the first day, because this log is the most valuable output of a diagnostic block and decides whether radiofrequency treatment is a suitable option for you. Rest without lying down for long stretches, move gently, and avoid heavy lifting, forceful backward arching and prolonged sitting on the first day. Expect local soreness at the needle site for a day or two, and a slight increase in pain once the anaesthetic wears off before any added steroid takes effect. Monitor blood sugar if you are diabetic. Resume your exercise and trunk strengthening programme during the period of relief rather than simply resting, because the injection enables rehabilitation and does not treat the structural cause on its own. **Seek care immediately for fever or chills, redness, warmth or discharge at the injection site, new or worsening leg weakness, loss of bladder or bowel control, saddle numbness, or a severe headache that worsens on standing.**
Expected duration
Usually 15 to 30 minutes, with a short observation period of 20 to 45 minutes before discharge.
