Turbinate coblation in Jordan

كي القرنيات الأنفية

Turbinate reduction, still widely called turbinate cautery, is a procedure that shrinks an enlarged inferior nasal turbinate. The turbinates are normal ridges inside each nostril covered by a richly vascular lining, and their job is to warm, humidify and filter inhaled air. When they stay chronically swollen — because of allergy, non-allergic rhinitis, or overuse of decongestant nasal drops — the airway narrows and the patient complains of blockage that never fully clears. Today the procedure is usually performed with radiofrequency energy or coblation, using a slim probe passed inside the turbinate tissue so that it shrinks from within while the surface lining and its function are preserved. That submucosal approach is preferred over the older surface cautery, and the aim is volume reduction rather than burning the surface. What the procedure does not do matters just as much. It does not correct a deviated septum, so a patient whose blockage comes from the septum needs a different or a combined operation. It does not cure the underlying allergy: sneezing, itching and watery discharge can continue and medical treatment is still needed. It does not restore a lost sense of smell. And the turbinate is never removed completely, because aggressive removal can lead to chronic dryness and the distressing empty-nose sensation. **The benefit may also fade after some years and swelling can return**, particularly if the triggers persist. The decision is made by clinical examination and nasal endoscopy after an adequate trial of organised medical treatment — typically a steroid nasal spray, antihistamines and allergen control — not simply because a patient reports a blocked nose. It may be carried out under local anaesthesia in a procedure room, or under general anaesthesia when combined with other nasal surgery. The specialty concerned is ear, nose and throat surgery.

Procedure steps

  1. 1

    Assessing the cause of blockage

The surgeon examines the nose endoscopically and compares turbinate size before and after a decongestant spray; a blockage that eases with the spray points to swelling rather than a bony septal problem. Allergy history and the medicines already tried, and for how long, are reviewed as well.

  2. 2

    Anaesthesia

In most cases topical anaesthetic applied to the lining followed by an injection into the turbinate is enough, and the patient stays awake with little discomfort. When it is combined with septal correction or sinus surgery it is performed under general anaesthesia in theatre.

  3. 3

    Delivering energy inside the tissue

A slim probe is introduced into the turbinate through a tiny entry point, and energy is applied along one or more tracks for a few seconds each. The controlled heat injures a limited volume of tissue beneath the surface, which then contracts gradually over the following weeks.

  4. 4

Checking the airway and the other side

The nose is re-examined endoscopically to confirm a wider passage and no active bleeding, then the other side is treated if it is enlarged too. The surgeon deliberately leaves enough tissue to keep humidifying the air and avoids over-reduction.

  5. 5

    Finishing and short observation

Nasal packing is usually not required, though an ointment or a thin splint may be used in selected cases. The patient is observed briefly and goes home the same day with saline-rinse instructions and a follow-up appointment.

Before the procedure

Tell your doctor about every medicine you take, especially blood thinners, aspirin, anti-inflammatory painkillers and herbal supplements, and never stop a medicine on your own. Mention drug allergies and any clotting disorder or family history of frequent nosebleeds. If the procedure is under general anaesthesia, follow the anaesthetist's fasting instructions; if it is under local anaesthesia you may usually have a light meal unless told otherwise. Stop decongestant nasal drops under medical supervision, since dependence on them is a common cause of turbinate swelling. Do not stop a prescribed allergy spray unless instructed. Stop smoking, and postpone the appointment if you have acute rhinitis or sinusitis. Bring saline nasal rinse, and arrange a driver if you are given sedation or a general anaesthetic.

After the procedure

Expect the nose to feel more blocked during the first days because of swelling and crusting — that is the normal course, not a failure. Real improvement usually begins after one to two weeks and settles over one to two months. Use saline rinses several times a day; this is the single most useful step for clearing crusts. Do not blow your nose forcefully and do not pick crusts off. Avoid dust, smoke and strong fumes, and skip strenuous exercise, heavy lifting and steam rooms for one to two weeks. Continue your prescribed allergy treatment, since controlling the underlying inflammation is what protects the result long term. **Seek care immediately for: heavy or repeated nosebleeds that do not stop with pressure, high fever, pain that increases instead of easing, a strong foul odour with purulent discharge, or marked dryness with a sensation that air passes without any feeling of breathing.**

Expected duration

Usually 10 to 25 minutes for both sides, with same-day discharge.
