About Parathyroid Surgery
Parathyroid surgery removes one or more of the parathyroid glands — four small glands usually lying behind the thyroid in the neck, whose job is to regulate blood calcium through parathyroid hormone. One distinction is worth stating clearly, because it is widely confused: these glands are not the thyroid and do not regulate metabolism or weight. They regulate calcium and bone, so parathyroid surgery does not treat an underactive or overactive thyroid.
The commonest indication is primary hyperparathyroidism, most often caused by a benign tumour in a single gland that secretes hormone without control, raising blood calcium and drawing it out of bone. This presents with recurrent kidney stones, osteoporosis and fractures, bone pain, or less specific symptoms such as thirst, frequent urination, fatigue, low mood and poor concentration; it may also be discovered on a routine blood test in someone with no symptoms. Secondary hyperparathyroidism accompanies advanced kidney disease and is managed medically first, with surgery reserved for failure of medical treatment.
Two stages come before any operation. First the diagnosis is confirmed biochemically — calcium, parathyroid hormone, vitamin D, kidney function and urinary calcium. Then the culprit gland is localised with imaging: neck ultrasound, nuclear medicine scanning or dedicated CT. Operating without localisation means a wider neck exploration, and a raised calcium level itself has other causes — certain medicines, excess vitamin D and other diseases — which must be excluded first.
The limits deserve emphasis. Surgery corrects the calcium level and stops ongoing bone loss, but it does not restore bone density immediately: recovery is gradual over months to years and requires calcium, vitamin D and monitoring. It does not dissolve existing kidney stones or repair kidney damage that has already occurred, though it reduces the formation of new stones. It does not guarantee that non-specific symptoms such as fatigue and poor concentration will disappear in every patient. And it is neither neck cosmetic surgery nor a treatment for thyroid enlargement.
Whether you are a candidate, and how extensive the operation should be — a focused single-gland excision or bilateral neck exploration — is determined by clinical assessment, laboratory results and imaging, in a joint decision with your endocrinologist, and not by reading a page.
