Percutaneous embolization of varicocele in Jordan

علاج دوالي الخصية بالقسطرة (الانصمام عن طريق الجلد)

Percutaneous varicocele embolization is an interventional procedure that closes the refluxing spermatic vein responsible for dilated veins in the scrotum. Under local anaesthetic, an interventional radiologist introduces a fine catheter through a vein in the upper thigh or the neck, guides it into the internal spermatic vein and occludes it with small metal coils or a sclerosing agent, so reflux stops and congestion gradually settles. In routine cases there is no scrotal incision, no stitches and no general anaesthetic. Varicoceles are common and many of them need no treatment at all. Treatment is usually considered when there is chronic aching or heaviness that has not responded to simple measures, clear loss of testicular volume in adolescents and young men, or abnormal semen parameters in a man planning a pregnancy who has a varicocele that can be felt on examination. The decision rests on clinical examination, Doppler ultrasound and semen analysis, not on a scan report alone, since a varicocele that cannot be felt and is only seen on imaging is not usually treated. The limits of the procedure are important. Embolization treats venous reflux; it is not a guaranteed treatment for infertility. Semen parameters improve in some men over the following months and do not improve in others, and no result can be promised. It does not correct other causes of reduced fertility such as hormonal, genetic or obstructive problems, it does not reliably raise testosterone, and it does not always restore the volume of a testis that has already shrunk. Reflux can also recur later through collateral veins and needs fresh assessment. A common myth is that every varicocele causes infertility and every case needs urgent intervention. In reality many affected men father children without any treatment, and candidacy is determined by clinical examination and your doctor's assessment alongside test results. Discuss with your doctor how embolization compares with surgical ligation in terms of anaesthesia, recovery time and the chance of recurrence before choosing.

Procedure steps

  1. 1

    Assessment and diagnosis

The doctor examines both testes standing and lying, estimates their volume, arranges scrotal Doppler ultrasound to document reflux, requests semen analysis in men planning a pregnancy and may add hormone tests. Symptoms and findings are combined to decide whether intervention is genuinely justified.

  2. 2

Preparation, local anaesthetic and venous access

The puncture area in the groin or neck is cleaned, local anaesthetic is given and a thin sheath is placed in the vein. You normally remain awake and able to talk to the team, with vital signs monitored throughout the session.

  3. 3

Reaching the spermatic vein and confirming reflux

The catheter is advanced into the renal vein and then the internal spermatic vein, and contrast is injected to document the direction of flow and map collateral veins. This step matters because leaving a refluxing collateral behind is one reason varicoceles recur.

  4. 4

    Occluding the refluxing vein

The spermatic vein is closed with fine coils, a sclerosing agent or both, at more than one level if required, with check imaging after each step to confirm that reflux has stopped and healthy veins are preserved.

  5. 5

    Discharge and follow-up

The catheter is removed, pressure is applied to the puncture site and a dressing is placed; most men go home the same day and return quickly to desk work. Examination and Doppler are repeated at follow-up, and semen analysis is usually rechecked after three to six months because sperm production needs that long to reflect the change.

Before the procedure

Tell your doctor about all your medicines and supplements, especially blood thinners and antiplatelet drugs, and do not stop anything yourself but ask how it should be adjusted. Report any previous contrast reaction, any kidney disease and any earlier surgery in the abdomen, groin or scrotum. Bring your Doppler report, semen analysis and hormone results if you have them. Blood tests for kidney function and clotting are usually requested. Fasting is normally four to six hours, with sips of water and approved tablets allowed, and because the procedure uses X-rays a protective shield is used according to local protocol. Wear supportive, comfortable underwear and arrange for someone to drive you home, as driving on the same day is not advised.

After the procedure

Rest for the remainder of the day, keep the dressing clean and dry for the first day, and avoid heavy lifting, vigorous sport, cycling and baths for about three to seven days, resuming intercourse according to your doctor's advice. Mild groin and scrotal aching, heaviness and a small bruise at the puncture site for a few days are expected; lying down, supportive underwear and the prescribed painkiller help. Relief of heaviness and pain develops over weeks, and semen analysis is repeated after months rather than days. **Go to the emergency department immediately if you develop a high fever with shivering, severe or increasing scrotal pain, swelling and redness, chest pain, shortness of breath or palpitations, continued bleeding or a rapidly expanding swelling at the puncture site, or severe flank pain.** Contact your doctor if pain persists for weeks or the veins become visible again.

Expected duration

The procedure usually takes 30 to 60 minutes, with a short period of observation afterwards and same-day discharge in most cases.
