Thyroid surgery removes part or all of the gland. The thyroid sits at the front of the neck and secretes hormones regulating metabolism, and the decision to remove it is taken only for a clear indication, because total removal means lifelong dependence on hormone replacement.
Usual indications: a nodule or mass suspected or proven malignant; a large nodule compressing the trachea or oesophagus causing breathlessness or difficulty swallowing; enlargement extending into the chest cavity; or overactivity not controlled by medication or radioactive iodine.
Before any surgery, steps that cannot be skipped must be completed: assessment of gland function, ultrasound to characterise nodules precisely, and fine-needle biopsy of suspicious nodules — many nodules are benign and need no surgery at all, and the biopsy is what distinguishes them. Vocal cord assessment is also done with hoarseness or previous neck surgery.
Two risks specifically characterise this operation and must be stated before consent, not after: the recurrent laryngeal nerve, which passes behind the gland and moves the vocal cord — injuring it causes hoarseness that may be temporary or permanent, so its integrity is monitored during surgery; and the four small adjacent parathyroid glands that regulate calcium — damaging them causes low calcium presenting as numbness around the mouth and fingers and cramps, which may be temporary or permanent and needs supplements.
Recovery is usually quick with a horizontal neck scar that fades over time. Follow-up afterwards is not a formality: adjusting the replacement hormone dose by periodic blood tests determines the patient’s quality of life, and the dose is adjusted by testing, not by how one feels.
