What is Long COVID?

Long COVID (or post-COVID) is a term describing a group of symptoms that persist or appear weeks or months after a coronavirus (COVID-19) infection, beyond the usual acute illness period. It may affect even those whose initial infection was mild, and varies greatly from person to person in the type, severity, and duration of symptoms.

The most prominent symptoms of long COVID are fatigue and severe exhaustion that worsens after exertion, shortness of breath, brain fog and difficulty with concentration and memory, in addition to heart palpitations, sleep disturbance, muscle and joint pains, loss or change of smell and taste, headache and dizziness, and anxiety and depression. Several body systems may be affected, making it a complex condition.

There is no single specific treatment for long COVID yet, and its management relies on a comprehensive evaluation to rule out other causes and treat each symptom within a multidisciplinary approach (pulmonary, cardiac, neurology, rehabilitation, mental health). In cases of severe post-exertional fatigue, it is important to follow the principle of careful graded activity and avoid overexertion that worsens symptoms. The reassuring news is that the majority improve gradually over time with supportive care, and that COVID vaccination reduces the risk of developing long COVID. Detection and medical follow-up are important to improve quality of life.

Symptoms

  • Severe fatigue and exhaustion that worsens after exertion.
  • Shortness of breath and palpitations.
  • Brain fog and difficulty with concentration and memory.
  • Sleep disturbance and muscle and joint pains.
  • Loss or change of smell and taste, and anxiety and depression.

Causes

  • A persistent effect of COVID-19 infection on several body systems.
  • Persistent inflammation, an immune response, and small-vessel problems are thought to play a role.
  • It may affect even those whose initial infection was mild.

Diagnosis

  • Clinical assessment of symptoms persisting after COVID.
  • Tests to rule out other causes and assess affected systems.
  • Multidisciplinary referral by the predominant symptoms.

Treatment

Symptom management

  • Treating each symptom separately (breathing, heart, sleep, mood) within a multidisciplinary approach.

Prevention

  • COVID-19 vaccination reduces the risk of long COVID.
  • Preventing infection and avoiding reinfection.

When to see a doctor
