Is acupuncture painful?

Acupuncture can cause a needling sensation or mild pain when a needle is inserted or manipulated, but the intensity varies between people, techniques, and treatment sites. Sources also describe a sensation known as “de qi,” which may involve heaviness, numbness, or tingling. Importantly, the sensation experienced during acupuncture, whether needling, heaviness, or tingling, is not by itself established evidence that the treatment is effective or working through its intended mechanism. What might a person feel during a session?

Most acupuncture methods use thin needles inserted into the skin at particular points. Depending on the technique, the needles may be moved or rotated, or combined with other forms of stimulation. The National Cancer Institute states that some people may experience a needling sensation known as “de qi,” which may be described as heaviness, numbness, or tingling. Sources also state that soreness or pain at the treatment site can occur as a side effect. Therefore, there is no single sensation that everyone should experience. [1] [2]

Does pain mean that acupuncture is working?

Not necessarily. The intensity of needling, pain, or the presence of a “de qi” sensation should not be used as a test of whether a session is successful. NCCIH states that how acupuncture works is not fully understood and that its effects may involve the nervous system and tissues, as well as nonspecific effects related to factors such as a person’s expectations and the practitioner-patient relationship. Studies in different conditions have also found that the difference between real and sham acupuncture can be small or unclear for some outcomes. [2]

What side effects and risks are possible?

Sources state that acupuncture may cause soreness or pain at the needle sites, fatigue, lightheadedness, or sleepiness. Infection and other complications can also occur, particularly when needles are not sterile or are inserted incorrectly. This makes the use of sterile, single-use needles and safe practice important. Safety considerations may be especially relevant for people with health conditions or treatments that weaken immunity, so acupuncture should not automatically be assumed to be appropriate for everyone without considering the person’s health situation. Is acupuncture effective for every condition?

No. NCCIH reports evidence of possible benefit for some pain conditions, including certain types of back and neck pain, osteoarthritis pain, and headaches, but the strength of evidence varies by condition, and some studies do not show a clear advantage over sham acupuncture. The NCI also states that some uses in people with cancer may help relieve particular symptoms, such as treatment-related nausea and vomiting, while results are mixed for other conditions. Acupuncture should therefore be viewed as a complementary approach in specific contexts rather than as a general substitute for established medical treatment.
