How does insurance prior authorization work?
An insurer may require prior authorization for certain procedures. The doctor establishes the clinical need, then the clinic or the party handling the claim contacts the insurer and supplies the documents required by the policy.
These may include a medical report, tests or images and a cost estimate. Ask for the request number, current status and any missing documents. There is no universal response time; it depends on the insurer, procedure and completeness of the information.
Check the scope, validity period and copayment attached to an approval. Authorization does not always mean every expense is covered. Do not delay emergency care while waiting for routine paperwork. Use the insurance directory for practical provider information, and contact the clinic and insurer.
