Why does the bottom of my heel hurt with the first morning steps?

Quick answer

Pain under the heel during the first steps after rest commonly occurs with plantar fasciitis, but the pattern alone is not diagnostic. Location, injury, activity load and other symptoms help distinguish tendon, bone or nerve problems.

Adjustments to try

Temporarily reduce activities that aggravate pain and wear comfortable supportive shoes rather than walking barefoot on hard floors. Gentle calf and sole stretches may help if they do not worsen pain. Build activity according to tolerance.

When to seek examination

Arrange review if pain persists despite self-care, recurs or limits walking, or if you have diabetes or numbness. A heel spur on an image is not necessarily the pain source. Injections or surgery are not required for everyone.

Do not delay after significant injury

Sudden pain after injury with inability to walk or rise onto the toes, marked swelling or deformity needs prompt evaluation. Redness and heat with fever also require urgent assessment.
