Hyperhidrosis is a medical condition — not merely social embarrassment — in which the body sweats more than needed for temperature regulation, most often in the armpits, palms, soles or face, and it can interfere with work, writing and handling objects and carry a genuine psychological burden.

Botulinum toxin injection is one effective option in localised cases: it is injected superficially into the skin in a grid of closely spaced points, temporarily blocking nerve signalling to the sweat glands in the injected area so sweat production there falls markedly. The effect is not permanent — it begins within days to two weeks and usually lasts months before repeating.

Before injection two types must be distinguished: primary hyperhidrosis, which usually starts at a young age and is localised, symmetrical and without an evident underlying disease, and secondary hyperhidrosis, caused by another condition — such as an overactive thyroid, hormonal changes, infection or certain medications — or which is generalised or nocturnal. The second requires investigating the cause first, because treating the symptom alone may mask a disease needing treatment.

The usual pathway begins with strong medical topical antiperspirants, then injections are considered when those are insufficient, with further options discussed by the physician in resistant cases. Palm injections are more painful and need local anaesthesia or cooling, and may be accompanied by slight temporary weakness of grip strength, which should be known in advance. Injection is excluded in pregnancy and breastfeeding and with certain neuromuscular diseases, and the decision follows medical assessment.
