Essential Tremor: Symptoms, Causes, Treatment
الرعاش الأساسي
Essential tremor causes involuntary shaking, most often while using the hands. Severity varies, and assessment distinguishes it from other causes of tremor.
What is Essential Tremor?
Shaking may be noticeable when holding a cup or writing, and can affect the head or voice. Tremor does not automatically mean Parkinson disease; its appearance alone cannot establish the diagnosis.

Symptoms
Shaking usually develops gradually during movement or sustained posture. Fatigue, anxiety and caffeine may make it more noticeable, affecting eating, writing or speaking.
Causes
The exact cause is not clear in everyone, although familial cases occur. Medicine effects, thyroid overactivity and low blood sugar cause other forms of shaking that may need separate evaluation.
Risk factors
Increasing age and family history raise the likelihood, although younger people can be affected. Stimulants and tiredness may worsen symptoms but do not establish the diagnosis or underlying cause.
Diagnosis
Diagnosis uses medical history, neurological examination and tasks such as writing, alongside medicine review. Selective blood tests or imaging exclude alternatives; no single blood test confirms essential tremor.
Treatment
Mild symptoms may need only monitoring and adapted utensils. Medicines such as propranolol or primidone are selected after contraindication assessment. Deep brain stimulation or focused ultrasound suit selected disabling cases and carry risks.
Complications
Eating, writing and work may become difficult, with embarrassment or social withdrawal. Occupational strategies and appropriate emotional support can improve participation without assuming anxiety is the sole cause.
Prevention
There is no established preventive method. Adequate sleep, reducing caffeine if it worsens symptoms and adapting work tools may help. Alcohol should not be used as treatment.
When to see a doctor
Arrange assessment for new tremor or disruption of everyday activities. Sudden shaking with one-sided weakness, speech changes or severe headache requires emergency evaluation.
