Hallux Valgus (Bunion): Symptoms, Causes & Treatment

إبهام القدم الأروح (Bunion)

Quick summary

Hallux valgus (bunion) is a deformity of the big-toe joint in which the toe leans toward the other toes and the joint protrudes sideways forming a painful bony bump. It worsens with tight shoes. It is managed with comfortable footwear and padding, and may need surgery in severe painful cases.

What is Hallux Valgus (Bunion)?

Hallux valgus, popularly known as a bunion, is a deformity affecting the joint at the base of the big toe. In it, the big toe gradually leans toward the other toes while the joint bone protrudes outward (the side of the foot), forming a clear bony bump that may be painful, red, and swollen.

This deformity develops slowly over years due to an imbalance of forces on the big-toe joint. Heredity and foot shape play a fundamental role, while tight, high-heeled, and pointed-front shoes contribute to speeding it up and increasing symptoms, so it is more common in women. Over time it may be accompanied by joint inflammation, stiffness, and the formation of an inflamed bursa over the bump, and the big toe may press on the neighboring toes and deform them.

The management plan for a bunion depends on the severity of symptoms, not just the size of the bump. In most mild to moderate cases, conservative measures that ease pain and slow progression suffice: wearing wide, comfortable shoes with a broad front, using protective padding and spacers, and pain relievers when needed. It is important to know that these measures do not return the toe to its normal position but improve comfort. Surgery is reserved for severe painful cases that impair walking and life and do not respond to conservative treatment, where the bone and joint position is corrected.

Symptoms

  • A prominent bony bump at the base of the big toe.
  • Pain, redness, and swelling in the area, especially with shoes.
  • The big toe leaning toward the other toes.
  • Stiffness and difficulty walking in advanced cases.

Causes

  • Genetic factors and foot shape.
  • An imbalance of forces on the big-toe joint.
  • Tight, pointed, high-heeled shoes (a contributing factor).
  • Some joint diseases such as rheumatoid arthritis.

Diagnosis

  • Clinical examination of the foot and joint shape.
  • X-rays to assess the degree of deformity and joint condition.

Treatment

Conservative measures (most cases)

  • Wide, comfortable shoes with a broad front and low heel.
  • Protective padding, spacers, and supports.
  • Pain relievers and compresses for inflammation.

Surgery

  • Reserved for severe, painful, disabling cases that do not respond to conservative treatment, to correct the bone and joint.

Complications

  • Bursitis over the bump.
  • Deformity and overlapping of the neighboring toes.
  • Joint inflammation, stiffness, and difficulty walking.

Prevention

  • Choosing comfortable, broad-fronted shoes and avoiding pointed high heels.
  • Early attention to symptoms, especially with a family history.
  • Using supports and padding to slow progression.

When to see a doctor

See a doctor for persistent pain or swelling in the big toe, difficulty finding comfortable shoes, or impaired walking. Early intervention eases symptoms and slows progression. Book an appointment with an orthopedic or foot-surgery specialist on ClinicsJo.

FAQs about Hallux Valgus (Bunion)
