A cholecystectomy is an operation to remove the gallbladder, a small sac beneath the liver that stores bile and empties it into the intestine after a fatty meal. The gallbladder is removed when it becomes a source of repeated symptoms or complications, most often because of gallstones and inflammation. The typical symptom is pain that starts in the upper right abdomen or below the breastbone after a rich meal, steady and pressing, usually lasting from tens of minutes to a few hours, and often radiating to the back or the right shoulder with nausea and vomiting. Repeated attacks, acute cholecystitis, a stone that has migrated into the bile duct causing jaundice and dark urine, or gallstone pancreatitis are all strong indications for surgery. Silent stones with no symptoms at all do not automatically mean an operation; suitability is decided by clinical examination and imaging, not by reading about it. Most operations are performed laparoscopically through four small incisions, and the surgeon takes care to define the anatomy clearly before dividing any structure, so as to avoid injury to the bile duct. The bile duct may be imaged during surgery when a stone inside it is suspected, and an endoscopic procedure on the duct may be needed before or after the operation. Conversion to an open incision remains possible in severely inflamed cases and is a safety decision. Limits of the procedure: removing the gallbladder removes the source of stones. It does not treat irritable bowel syndrome, heartburn and acid reflux, hiatal hernia or gastritis. If your main complaints are bloating, burning and wind rather than clear biliary colic, symptoms may persist after surgery, and the true cause should be sought first. No medicine or herb reliably dissolves gallstones, and shock-wave fragmentation is not used the way it is for kidney stones. An important myth to correct: the so-called gallbladder flush with olive oil and lemon juice does not expel stones; what appears in the stool is not gallstone material, and the mixture can push a real stone into the bile duct, causing obstruction or pancreatitis. Life after gallbladder removal is normal, although some people need weeks to months to settle their fat intake, and temporary loose stools can occur.

    Diagnosis and bile duct assessment

    Assessment starts with clinical examination and the story of the pain, together with abdominal ultrasound and liver and pancreas blood tests. If there is jaundice or abnormal liver chemistry, more detailed imaging of the bile ducts is requested, because a stone inside the duct needs to be dealt with before or during surgery.

    Preparation and general anaesthesia

    Your medicines and chronic conditions are reviewed, and blood thinners are stopped only on your doctor's instruction. You usually fast for six to eight hours, an intravenous antibiotic is given when indicated, and general anaesthesia with a breathing tube is administered.

    Laparoscopic access and defining the anatomy

    The abdomen is insufflated with medical gas and the camera plus three instruments are inserted. The fundus of the gallbladder is retracted upwards and tissue is cleared until the cystic duct and cystic artery are unmistakably seen, the step that protects the main bile duct from injury.

    Detaching and retrieving the gallbladder

    The cystic duct and artery are clipped and divided, the gallbladder is separated from its liver bed and removed through one of the port sites inside a bag. The area is checked for bleeding or bile leakage, and a temporary drain may be left in inflamed cases.

    Recovery, discharge and follow-up

    The port sites are closed with simple sutures, and walking and drinking fluids the same day are encouraged. Many elective cases go home the same day or after one night, with a follow-up appointment to check the wounds and review the histology of the gallbladder.

Before the procedure

Tell your doctor about all your medicines, especially blood thinners, antiplatelet drugs, diabetes and blood-pressure medicines and herbal supplements, and never stop a medicine on your own. Report drug allergies, any difficult past anaesthesia and previous abdominal operations, along with heart or breathing conditions. Bring earlier reports, ultrasound images, blood tests and your insurance card. Follow the fasting instructions given by your team, usually six to eight hours before the appointment, and avoid rich fatty meals in the preceding days. Shower on the morning of surgery, remove jewellery, nail polish and contact lenses, wear loose clothing, and arrange someone to drive you home, as you must not drive after anaesthesia.

After the procedure

Take short, frequent walks from the first day, drink enough fluids, and start with light low-fat food, widening your diet gradually over about two weeks. Expect right shoulder discomfort and mild bloating from the gas used in laparoscopy; this usually settles within days. Take pain relief as prescribed, apply nothing to the wounds other than what your doctor advises, and avoid soaking in a bath or pool until permitted. Avoid heavy lifting and strenuous activity for roughly two to four weeks. Loose stools after fatty meals are common and usually temporary. **Seek care immediately if you develop: yellowing of the eyes or skin, dark urine or pale stools, high fever or shivering, severe worsening abdominal pain, persistent vomiting, distension with no passage of gas, yellow fluid leaking from a wound, redness or pus at a wound, shortness of breath or chest pain, pain and swelling in the leg.**

Usually 45 to 90 minutes laparoscopically, longer with severe inflammation or when the bile duct is imaged; discharge is typically the same day or after one to two nights.
