Retention is the phase of orthodontic care in which appliances called retainers are used to hold the teeth in their corrected positions after fixed braces are removed or clear aligner wear is completed. It belongs to the treatment plan from day one and is not a separate service added after the braces come off. When a tooth is moved, its position inside the bone changes, and the ligaments around its root together with the surrounding bone need a long period to reorganise around that new position. During this time the teeth are primed to drift back, and the mouth keeps changing with age even in people who never had orthodontic treatment. **Wearing a retainer after braces is not optional. Skipping it lets the teeth relapse gradually, and this is the most common reason a successful orthodontic result is lost years after treatment ended.** Retainers fall into two families. A fixed retainer is a thin wire bonded to the inner surface of the front teeth; the patient cannot remove it, which suits cases at higher risk of relapse or patients whose compliance cannot be relied upon. A removable retainer is either a clear tray covering the teeth or a plate with a front wire, taken out for eating and cleaning and worn on a schedule set by the orthodontist. Choosing between them, or using both together, depends on the type of movement that was corrected, the estimated risk of relapse and how reliably the patient can follow instructions. Suitability is decided by clinical examination, not by reading about it. The limits of retention should be stated plainly. A retainer preserves an existing result; it does not correct a new problem and it cannot pull back teeth that have already relapsed. If noticeable relapse has happened, the usual answer is a short course of re-treatment rather than a stronger retainer or longer wear. A retainer also gives no protection against decay or gum disease; it actually raises the need for careful daily cleaning, especially around a bonded wire where calculus builds up easily.
The orthodontist checks the bite and the stability of the corrected teeth, with radiographs when needed, to decide the retainer type and the relapse risk before appliances are removed.

Clean your teeth thoroughly before the visit and attend any gum-cleaning session you were asked to have, since inflamed gums make bonding a wire harder and less durable. Mention any known sensitivity to bonding materials or metals, and tell the clinician about any night guard you wear for teeth grinding so the retainer design can be coordinated with it. Arrange your schedule so the impression or scan is taken on the same day the appliances come off rather than later, and avoid hard foods immediately before the visit if your teeth feel tender. Finally, arrive with a realistic expectation: this is the start of a new phase of commitment, not the end of treatment.
Follow the prescribed wear hours exactly and do not decide on your own to cut them back because the teeth look straight. Remove a removable retainer for eating and for hot drinks, clean it with a soft brush and lukewarm rather than boiling water, and always keep it in its case; retainers are lost in paper napkins more often than they are broken. Clean around a bonded wire every day with floss or an interdental brush, because calculus collects on it quickly. Contact your orthodontist promptly if the fixed wire comes loose or you feel a sharp edge, or if a removable retainer no longer seats properly and needs force to insert. **Seek care immediately for swollen gums, pus or throbbing pain around a tooth carrying the wire, if you swallow a piece of the retainer, or if a front tooth becomes visibly mobile.** Never rely on an old retainer you have not worn for months; have it checked before using it again.
